Osteotomy for the treatment of altered passive eruption – case report with six months follow-up

Gummy smile is characterized by exposure of 3mm or more of gingiva during smile. One of the responsible factors is altered passive eruption that is characterized by the incomplete apical migration of the bone crest after the teeth have been into occlusion. In this case, the exposure of the crown is incomplete and result in a disproportion between the height and width of the tooth. The aim of this work was to report the treatment of Altered Passive Eruption with six months follow-up. Gengivectomy, osteotomy/osteoplasty and gengivoplasty were performed to correct the gummy smile and restore the harmony between the height and width of the anterior teeth. It can be concluded that the osteotomy in order to return the correct relationship between the cementum-enamel junction and bone crest is a safe and predictable option to obtain the harmony of the gingival margin of the anterior teeth.O sorriso gengival é caracterizado pela exposição de 3mm ou mais de gengiva ao sorrir e na maioria dos casos apresenta caráter multifatorial. Um dos principais fatores é a erupção passiva alterada que caracteriza-se pela migração apical incompleta da crista óssea após os dentes estarem em oclusão. Neste caso, a exposição da coroa do dente é incompleta, o que acaba gerando uma desproporção entre altura e largura do dente. O objetivo deste trabalho foi relatar o tratamento de Erupção Passiva Alterada com acompanhamento clínico de seis meses aonde foi realizada gengivectomia, osteotomia/osteoplastia e gengivoplastia para correção do sorriso gengival e devolver a harmonia entre a altura e largura dos dentes anteriores. Pode-se concluir que a osteotomia com intuito de devolver a correta relação entre a junção cemento-esmalte e a crista óssea demonstra ser uma opção segura e previsível para a obtenção da harmonia da margem gengival dos dentes anteriores
