Les soins palliatifs sont-ils encore un humanisme ?

International audienceThe French palliative movement emerged in the early 1980s from an ethical revolt denouncing the end-of-life conditions considered unworthy reserved for our fellow citizens at the end of life, thus challenging biomedicine in its instrumental drifts (unreasonable obstinacy, no -control of symptoms, therapeutic abandonment or euthanasia) and carried by the explicit desire to change the attitude of medicine and society as a whole in the face of death, dying and mourning. Thirty years later, the palliative movement has become standardized and institutionalized, succeeding in establishing itself in the French health scene as a health actor recognized by both biomedicine and the French public authorities. But this institutionalization was also done at the cost of medicalization (alignment of care on medicine, de-objectification, professionalization and increased technicization of care and support practices) and bureaucratization of the palliative movement, thus losing innovative excitement and the subversive audacity of its beginnings. Since then, would palliative care not have denied over time their original humanistic ideals by wanting to embody a legitimate medical specialty that is now caught between technoscientific norms and financial pressures? Can they embody a credible alternative to the backbone of euthanasia and physician-assisted suicide? These questions deserve to be asked and considered collectively.Le mouvement palliatif français a émergé au début des années 1980 en partant d’une révolte éthique dénonçant les conditions de fin de vie jugées indignes réservées à nos concitoyens en fin de vie, en contestant ainsi la biomédecine dans ses dérives instrumentales (obstination déraisonnable, non-contrôle des symptômes, abandon thérapeutique ou euthanasie) et porté par la volonté explicite de changer l’attitude de la médecine et de la société tout entière face à la mort, au mourir et au deuil. Trente ans plus tard, le mouvement palliatif s’est normalisé et institutionnalisé, en réussissant à s’imposer dans le paysage sanitaire français comme un acteur sanitaire reconnu tant par la biomédecine que par les pouvoirs publics français. Mais, cette institutionnalisation s’est aussi faite au prix d’une médicalisation (alignement des soins sur la médecine, désubjectivation, professionnalisation et technicisation accrues des pratiques de soins et d’accompagnement) et d’une bureaucratisation du mouvement palliatif, perdant ainsi l’effervescence innovante et l’audace subversive de ses débuts. Dès lors, les soins palliatifs n’auraient-ils pas renié au fil du temps leurs idéaux humanistes originels en voulant incarner une spécialité médicale légitime qui se retrouve aujourd'hui prise en étau entre normativités technoscientifiques et pressions financières? Peuvent-ils incarner une solution de rechange crédible face à la lame de fond de l’euthanasie et du suicide médicalement assisté? Ces questions méritent d’être posées et réfléchies collectivement
