Context Pancreatic cancer resection represents the only chance of cure but it can be performed only in localized cancer. Herein we report the case of a patient with locally advanced pancreatic cancer who underwent neoadjuvant chemo-radiation therapy followed by surgical resection with no residual tumour (R0). Case report A 50-year-old man was referred to our team for the presence of jaundice. A CT showed a mass in the head of the pancreas with an encasement of at least 50% of the circumference of the superior mesenteric vein (SMV). The involvement of the SMV was considered a relative contraindication to surgery. The patient underwent an ERCP and a biliary stent was left in the common bile duct.
