Long-term oncologic outcomes after radical cystectomy

Aim of the study: to evaluate long term survival rate after radical cystectomy. Purpose: 1. Evaluate incidence of complication events according to Clavien-Dindo classification. 2. Evaluate 90-day mortality rate after radical cystectomy. 3. Evaluate 5-year overall survival and cancer-specific survival rates after radical cystectomy. 4. Evaluate prognostic factors for survival. Materials and methods: This retrospective study included 209 patients who had bladder cancer and underwent radical cystectomy from 2000 March to 2017 December. Statistical analysis was performed while using SPSS 22.0 statistical program. Predictive factors were analyzed while using univariate and multivariate analysis of Cox’s regression model. Survival was estimated while using Kaplan-Meier method. A p value of <0,05 was consisted statistically significant. Results: 169 (80,9 %) men and 40 (19,1 %) female were enrolled into the study. The average age was 65,3 (±SD 9,24) (from 38 to 82) years old. Most often pathological stage diagnosed was pT2 (36,4 %). Lymph node involvement was found in 82 patients (41 %). 90-day mortality rate was 6,7 percent (14 patients). After radical cystectomy, 46,4 % had at least one complication. According to Clavien-Dindo classification, 1-2° complication rate was 22 %, 3-5° - 24,4 % of all patients. 5-year and 10-year overall survival rate was 35,9 % and 30,5 %. 5-year and 10-year cancer specific survival rate was 61,9 %. Lymph node involvement, pathological tumor stage and complication rate were statistical predictors for long-term survival. Conclusions.
