Psychosocial and seizure frequency changes after surgical epilepsy treatment

Aim: to assess psychosocial and seizure frequency changes after surgical epilepsy treatment. Objectives: 1. To assess seizure frequency changes after surgical epilepsy treatment. 2. To assess the association between sex, age, underlying brain pathology, localisation of epileptogenic lesions in electroencephalography (EEG), operation type and seizure frequency after surgical epilepsy treatment. 3.To assess psychosocial changes after surgical epilepsy treatment. Materials and methods: A retrospective study was performed of patients who had received surgical epilepsy treatment in 2014-2018 in Lithuanian University of Health Sciences Kaunas Clinic. Demographical and clinical (the onset period of epilepsy, seizure frequency before and 1 year after surgery, underlying brain pathology, localisation of epileptogenic lesions in EEG, the number of antiepileptic drugs before and 1 year after surgery) data were collected. Psychosocial changes were assessed by PESOS questionnaire before and 1 year after surgery. Statistical analysis was performed, results were statistically significant if p < 0,05. Results: Before surgery the median of seizures was 1,5 seizures/per week (min – 1 seizure/per month, max – 50 seizures/per week). 1 year after surgery the median of seizures was 0 seizures/per week (min – 0 seizures/per week, max – 10 seizures/per week). The changes of seizure frequency before and 1 year after surgical epilepsy treatment were statistically significant (p ˂ 0,001). Sex, age, underlying brain pathology, localisation of epileptogenic lesions in EEG, operation type had no association with different seizure frequency groups 1 year after surgery (p ˃ 0,05). There was a significant improvement 1 year after surgery in the assessment of daily life restrictions (p = 0,016), fear associated with epilepsy (p = 0,016) compared to the assessment before surgery. Conclusions. 1. 59,1% of subjects were seizure free, 18,2% - had a reduction in seizure frequency, 22,7% - had no change in seizure frequency 1 year after surgical epilepsy treatment. 2. Sex, age during surgery, underlying brain pathology, localisation of epileptogenic lesions in EEG, operation type has no association with seizure frequency 1 year after surgery. 3. There was an improvement in daily life restrictions and fear associated with epilepsy 1 year after surgery
