The main risk factors of gallstones in patients with liver cirrhosis.

The aim of research: To investigate prevalence of gallstones in patients with liver cirrhosis ant to evaluate the main risk factors. Goals of research: To compare gallbladder stones manifestation between patients with liver cirrhosis and control group. To determine correlation between gender, age, BMI (body mass index), education, work, duration of liver cirrhosis and prevalence of cholelithiasis in patients with liver cirrhosis and control group. To find out in which liver cirrhosis etiology (toxic liver cirrhosis, cryptogenic liver cirrhosis, viral B and C liver cirrhosis, primary biliary cholangitis, metabolic liver cirrhosis) more frequently gallbladder stones manifest. Methods: Case-control study was performed. The original anonymous questionnaire was made to collect data. In 2018 june – 2019 march anonymous questionnaire was conduced. Patients of LHSU KC (Lithuanian Health and Science University Kauno Clinics) Gastroenterology department with liver cirrhosis and Radiology departments without liver cirrhosis was questioned. Analyzed factors: morbidity of liver cirrhosis ant cholelithiasis, etiology and duration of liver cirrhosis, age, gender, BMI, education, work. Results: 124 participants attended in the study. 50% was with liver cirrhosis and 50% was in control group. Liver cirrhosis is more frequent in men (p=0,031). In control group was 61,3% women. There was no statistically significant diferrence in prevalence of cholelithiasis between liver cirrhosis and control group patients (37,1% and 38,7%; p=0,853). Gallbladder stones was prevalent in 18,8% women with cirrhosis and in 23,4% women in control group (p=0,102). Average age of participants was 58,58 ± 13,769. In 65 years group 33,3% respondents in control group had gallbladder stones (p=0,009). In 40,7% patients with liver cirrhosis on 40-60 years subgroup and in 32,7% control group patients in >60 years subgroup gallbladder stones occured (p=0,003). Gallstones and higher BMI had 19,7% patients with liver cirrhosis and 21,1% patient in control group (p=0,839). 28,6% of respondents with primary education had cirrhosis and gallstones, but statistically significant difference was not found (p=0,491). 27,6% participants with sedentary work had liver cirrhosis and cholelithiasis (p=0,000). Gallbladder stones was found in 62,5% patients with liver cirrhosis, which lasts >5 years (p=0,020). Most often etiology of liver cirrhosis was cryptogenic (45,2%). Most common cause of liver cirrhosis in women was cryptogenic (53,8%), in men – cryptogenic (38,9%), viral (33,3%). But statistically significant correlation was not found (p=0,065). Gallstones most often was found in patients with primary biliary (66,7%), toxic liver cirrhosis (40%), but it was not statistically significant (p=0,688). Conclusions: Prevalence of gallstones in patients with liver cirrhosis and controul group patients is similar (38,7% and 37,1%). Prevalence of cholelithiasis in patients with liver cirrhosis is not associated with gender, BMI, education, etiology of liver cirrhosis and it is associated with age, work and duration of liver cirrhosis
