Patterns of antimicrobial resistance and genotyping of extended spectrum β-lactamase(ESBL

임상병리학과/석사[한글] 2003년 4월에서 12월까지 강원도 원주시에 소재하고 있는 원주기독병원에서 분리된 ESBL을 생산하는 E. coli와 Klebsiella spp.를 대상으로 VITEK system과 double disk synergy(DDS) test를 통하여 항생제에 대한 minimal inhibitory concentration(MIC)와 extended spectrum beta-lactamase(ESBL) activity를 검사하였다. 그리고 균주를 대상으로 분자생물학적인 방법인 PCR을 통하여 β-lactamase gene의 phenotype을 확인하였다. 그 결과, 17주의 E. coli 중 14균주(82%)에서 TEM type이, 5균주(29%)에서 CTX-M type이 확인되었다. 한편 40주의 Klebsiella spp.에서 30주(75%)에서 TEM type이, 38주(95%)에서 SHV type이 그리고 7균주(20%)에서 CTX-M type이 확인되었다. 이를 바탕으로 분자생물학적 유전자 분석기법인 ERIC(enterobacterial repetitive intergenic consensus sequence) PCR을 이용하여 ESBL을 생산하는 E. coli와 Klebsiella spp.를 대상으로 각각의 밴드 패턴의 유사성을 통하여 Dendrogram을 작성하여 균주간의 근연관계를 확인하였다. E. coli의 경우 유사도 84% 이상의 수준에서 크게 6개의 group을 형성하였고, 다시 세부적으로 유사도 92% 이상의 수준에서 유사성을 가지는 group이 형성되었다. Klebsiella spp.의 경우 유사도 50% 수준에서 크게 2가지로 분류가 가능하였다. 첫 번째와 두 번째 모두 다시 세부적으로 6개의 group을 형성하였다. 균주들 간의 근연관계와 항생제에 대한 내성여부를 비교해 본 결과 각각의 항생제 및 ESBL activity test결과들을 기준으로 ESBL을 생성하는 E. coli와 Klebsiella spp.의 분류가 가능하였다. [영문]Total 57 strains of extended spectrum β-lactamase (ESBL) producing E. coli and Klebsiella species were isolated from Wonju Christian Hospital during a 9 month-period from April to December, 2003. To determine the prevalence and genotypes of the ESBL producing clinical isolates, antibiotic susceptibility and ESBL activity test by VITEK system and double disk synergy (DDS) test, and PCR based genotyping were performed. Fourteen (82%) isolates of 17 ESBL producing E. coli were found to have blaTEM gene and 5 (29%) isolates were found to have blaCTX-M gene by polymerase chain reaction (PCR). Thirty (75%) isolates of 40 ESBL producing Klebsiella species with blaTEM gene, 38 (95%) isolates with blaSHV gene, and 7 (20%) isolates with blaCTX-M type gene were also identified. Enterobacterial repetitive intergenic consensus (ERIC) PCR and similarity index by dendrogram for genetical similarity to band pattern of each clinical isolates were examined. ESBL producing E. coli were grouped into 6 clusters up to 84% of similarity index and Klebsiella species were grouped into 12 clusters up to 76% of similarity index. In conclusion, ESBL producing clinical isolates were characterized with the results from antimicrobial resistance pattern and genetical similarity using ERIC PCR.ope
