Development of causal models in the investigation of the relationship of social, psychological and nutritional parameters in the impact of cardiovascular disease: 10 - year follow - up of the ATTICA study (2002 - 2012)

Aim: investigating the effect of socio-economic, psychological, behavioral and nutritional parameters on the 10-year incidence of cardiovascular disease (CVD) in the context of the ATTICA study (2002–2012). Materials-Methods: from May 2001 to December 2002, 1514 men and 1528 women (>18 years) without any clinical evidence of CVD or any other chronic disease, at baseline, living in greater Athens area, in Greece, were enrolled. From the initial sample of 3042 participants, 853 (453 men, 45±13 years and 400 women 44±18 years) underwent psychological evaluation using valid self-reporting psychometric tools. Educational years and average annual income were used to define the socioeconomic status (SES). The level of adherence to the Mediterranean dietary pattern (MD) was assessed using the MedDietScore. During 2011-12, the 10-year follow-up was performed and the new CVD events were recorded. Multiple logistic regression was applied for the estimation of the odds ratio (OR) of developing CVD, Path analysis for detecting mediation mechanisms in CVD development and και Item Response Theory (IRT) to estimate the combined measure of anxiety and depression (“Psychological distress”) and the Health and Functionality Index (HFI). The Healthy Aging Index (HAI) was also calculated evaluating the presence of hypertension, hypercholesterolemia, diabetes mellitus, obesity, depression, the Body Mass Index category, physical activity and the level of adherence to MD. Results: the 10-year CVD incidence was 5.1% (n=43 events). Depression was associated with almost 3.5-fold elevated CVD incidence (adjusted OR =3.6, 95%CI: 1.3-11, p=0.020), anxiety was also associated positively with CVD at borderline (adjusted OR=1.03, 95%CI: 1.0-1.1, p=0.080), MD negatively (p<0.001) while the individuals of low SES exhibited almost double CVD incidence (adjusted OR =1.9, 95%CI: 1.2-3.1, p=0.008) compared to the high SES group. Additionally, 14% (i.e., 1/7 units) increase in HAI was associated with 53% reduction in 10-year CVD incidence (adjusted OR=0.47, 95%CI: 0.28-0.80, p=0.005). Finally, when investigating the indirect pathways (mediating effects): a) “Psychological distress” through lower physical activity (p<0.001), then through elevated C-reactive protein levels (p=0.045) and finally through metabolic syndrom (p<0.001), eventually led to increased CVD incidence (p=0.049), b) “Psychological distress” mediated the SES-CVD association: individuals of moderate or high SES had lower levels of “Psychological distress” (p<0.001), which served as protection against CVD and γ) ο HFI mediated the observed association of educational and financial status with mortality (p=0.020 και p<0,001) be reducing their impact on mortality. Conclusions: the integration of both the mental health parameters that are risk factors for CVD and the socio-economic determinants of health, in the intervention strategies of the health authorities aimed at reducing the risk of the disease, could help to improve life expectancy by promoting well-being as a primary goal.Σκοπός: η διερεύνηση της επίδρασης κοινωνικο-οικονομικών, ψυχολογικών-συμπεριφοριστικών και διατροφικών παραμέτρων στη δεκαετή επίπτωση της καρδιαγγειακής νόσου (ΚΑΝ) στα πλαίσια της μελέτης ΑΤΤΙΚΗ (2002–2012). Υλικό-Μέθοδος: από το Μάιο του 2001 έως το Δεκέμβριο του 2002, 1514 άνδρες και 1528 γυναίκες άνω των 18 ετών, που κατοικούσαν στην ευρύτερη περιοχή της Αθήνας και ήταν ελεύθεροι καρδιαγγειακής νόσου και άλλων χρόνιων νόσων, αποτέλεσαν το δείγμα της μελέτης. Από το συνολικό δείγμα των 3042 ατόμων, 853 συμμετέχοντες (453 άνδρες, 45±13 ετών και 400 γυναίκες 44±18 ετών) υποβλήθηκαν σε ψυχολογική αξιολόγηση με τη χρήση έγκυρων ψυχομετρικών εργαλείων αυτο-αναφοράς. Τα έτη εκπαίδευσης και το μέσο ετήσιο εισόδημα χρησιμοποιήθηκαν για τον ορισμό του κοινωνικο-οικονομικού επιπέδου (ΚΟΕ). Ο βαθμός προσκόλλησης στη Μεσογειακή διατροφή (ΜΔ) μετρήθηκε με το MedDietScore. Κατά τα έτη 2011-12, πραγματοποιήθηκε ο 10ετής επανέλεγχος και καταγράφηκαν τα νέα περιστατικά ΚΑΝ. Η πολλαπλή λογιστική παλινδρόμηση χρησιμοποιήθηκε για την εκτίμηση των σχετικών λόγων ανάπτυξης ΚΑΝ, η Ανάλυση Διαδρομών (Path analysis) για την ανίχνευση διαμεσολαβητικών μηχανισμών στην ανάπτυξη ΚΑΝ και η Θεωρία Απόκρισης Αντικειμένων (IRT) για την εκτίμηση του συνδυασμένου μέτρου άγχους και κατάθλιψης (“Ψυχολογική Δυσφορία”) και του Δείκτη Υγείας και Λειτουργικότητας (ΔΥΛ). Επίσης κατασκευάστηκε ο Δείκτης Υγιούς Γήρανσης (ΔΥΓ) αξιολογώντας την παρουσία υπέρτασης, υπερχοληστερολαιμίας, σακχαρώδη διαβήτη, παχυσαρκίας, κατάθλιψης, την κατηγορία Δείκτη Μάζας Σώματος, τη φυσική δραστηριότητα και το βαθμό προσκόλλησης στη ΜΔ. Αποτελέσματα: η 10ετής επίπτωση της ΚΑΝ ήταν 5,1% (n=43 περιστατικά). Η κατάθλιψη συσχετίστηκε με κατά 3,5 περίπου φορές υψηλότερη επίπτωση ΚΑΝ (προσαρμοσμένος ΣΛ=3,6, 95%ΔΕ: 1,3-11, p=0,020), το άγχος επίσης συσχετίστηκε θετικά με την ΚΑΝ οριακά (προσαρμοσμένος ΣΛ=1,03, 95%ΔΕ: 1,0-1,1, p=0,080), η ΜΔ αρνητικά (p<0,001) ενώ τα άτομα χαμηλού ΚΟΕ παρουσίασαν σχεδόν διπλάσια επίπτωση ΚΑΝ (προσαρμοσμένος ΣΛ=1,9, 95%ΔΕ: 1,2-3,1, p=0,008) σε σχέση με τα υψηλού. Επίσης, 14% (δηλαδή 1/7 μονάδες) αύξηση στον ΔΥΓ συσχετίστηκε με 53% μείωση στη 10ετή επίπτωση ΚΑΝ (προσαρμοσμένος ΣΛ=0,47, 95%ΔΕ: 0,28-0,80, p=0,005). Τέλος, κατά τη διερεύνηση των έμμεσων μηχανισμών διασύνδεσης (διαμεσολαβήσεις): α) η “Ψυχολογική Δυσφορία” μέσω της μειωμένης φυσικής δραστηριότητας (p<0,001), εν συνεχεία μέσω των αυξημένων επιπέδων C-αντιδρώσας πρωτεΐνης (p=0,045) και τέλος μέσω του μεταβολικού συνδρόμου (p<0,001), οδήγησε τελικά σε αυξημένη καρδιαγγειακή επίπτωση (p=0,049), β) η “Ψυχολογική Δυσφορία” διαμεσολάβησε τη σχέση ΚΟΕ-ΚΑΝ: τα άτομα μέσου ή υψηλού ΚΟΕ παρουσίασαν χαμηλότερα επίπεδα “Ψυχολογικής Δυσφορίας” (p<0,001), γεγονός που έδρασε προστατευτικά έναντι της ΚΑΝ και γ) ο ΔΥΛ διαμεσολάβησε την παρατηρούμενη σχέση μορφωτικού και οικονομικού επιπέδου-θνησιμότητας (p=0,020 και p
