Methodische und technisch-experimentelle Untersuchungen zur Realisierung einer elektrophysiologischen Blaukanalstimulation

The investigation of the blue channel of the visual system is very important for different ophthalmologic diseases like glaucoma, diabetic retinal diseases, dominant-infantile opticus atrophy, side effects of drugs and intoxications. The acquisition and evaluation of visual evoked potentials (VEP) during isolated blue channel stimulation can offer the potential for an objective investigation of early blue channel damages. The subject of this work are methodical, technical and experimental contributions for the improvement of blue channel stimulation together with the acquisition of VEPs. The basics for this work are engineering methods together with the qualification of coloured stimuli within the physical, photometric and colorimetric layer. The main focus is the mathematical modelling of trichromatic colour perception (additive colour mixing). Methodical contributions will be given to the following aspects: the influence of background illumination, the use of trichromatic colour models for the realization of the silent substitution technique, optimization of spectral properties of light sources for background and stimulus, the expressiveness of photometric parameters for blue channel stimulation, the improvement of blue-on-yellow stimulation with background modulation, the context with equiluminant stimulation, the use of the Ganzfeld stimulation results for the optimization of localized stimulation, and the mandatory provision of spetroradiometric measurements. A research station has been realized within this work for Ganzfeld- and monitor based stimulation and simultaneous acquisition of VEPs. The coloured stimuli have been qualified within the physical, photometric and colorimetric layer. The retinal hazard potential has been estimated using international standards. The experimental contributions investigate the usefulness of the proposed methods. Responses of the blue channel should be identified clearly within the VEP. Important influences onto the shape of the VEP should be demonstrated. The following aspects will be investigated: basics of blue channel pattern stimulation, realization of flash stimulation with silent substitution technique, realization of multifocal blue channel stimulation, the influence of stimulus pulse width, substitution methods and equiluminant stimulation, the influence of rods onto the VEP, the stimulation with two different blue light sources within the physical, photometric and colorimetric layer. The discussion of the experimental contributions will result in recommendations for the use in clinical investigations.Die Untersuchung des Blausinns im visuellen System besitzt für verschiedene ophthalmologische Erkrankungen (Glaukom, diabetische retinale Funktionsschäden, dominant-infantile Optikusatrophien, Medikamentennebenwirkungen, Intoxikationen) eine große Bedeutung. Die Erfassung und Auswertung visuell evozierter Potentiale (VEP) bei isolierter Blaukanalstimulation kann das Potential für eine objektive Untersuchung zur Früherfassung von Blausinnschäden schaffen. Ziel der Arbeit ist es, methodische und technisch-experimentelle Beiträge für die Weiterentwicklung der Blaukanalstimulation und der simultanen VEP-Erfassung zu liefern. Ingenieurwissenschaftliche Ansätze bilden zusammen mit der Beschreibung grundlegender Prozesse der Farbwahrnehmung und der Kennzeichnung von Farbreizen in physikalischer, photometrischer und colorimetrischer Ebene die Grundlagen für die gesamte Arbeit. Die mathematische Modellierung des trichromatischen Farbensehens bildet hierbei einen Schwerpunkt. Methodische Beiträge werden zu folgenden Aspekten gegeben: Einfluss der Hintergrundbeleuchtung, Anwendung von Farbmodellen zur Realisierung der Silent-Substitution-Technik, Optimierung der Spektraleigenschaften der Lichtquellen für Hintergrund und Stimulus, Untersuchung der Aussagekraft photometrischer Kenngrößen bei Blaukanalstimulation, Verbesserung der Blau-auf-Gelb-Stimulation mit moduliertem Hintergrundlicht, Einordnung der äquiluminanten Stimulation, Nutzung der Ergebnisse der Ganzfeldstimulation für lokale Untersuchungen, die Unabdingbarkeit spektroradiometrischer Messungen. Eine konkrete Realisierung eines Forschungsmessplatzes für Monitor- und Ganzfelduntersuchungen zeigt die Kennzeichnung der verwendeten Farbreize in physikalischer, photometrischer und colorimetrischer Ebene. Anhand internationaler Normen wird eine Einschätzung der Netzhautgefährdung für diesen Messplatz gegeben. Die experimentellen Beiträge untersuchen die Realisierbarkeit und die Praktikabilität der vorgeschlagenen Methoden. Signalantworten des Blaukanals sollen im VEP-Signal eindeutig identifiziert und wichtige Einflussfaktoren demonstriert werden. Folgende Aspekte werden behandelt: orientierende Untersuchungen mit Musterstimulation, Realisierung einer Flash-Stimulation mit Silent-Substitution-Technik, Realisierung einer multifokalen Blaukanaluntersuchung, Einfluss der Pulsbreite, der Stimulus- und Hintergrundleuchtdichte bei Blau-auf-Gelb-Stimulation, partielle Substitutionsmethoden und äquiluminante Stimulation, Einfluss der Stäbchenaktivierung, Stimulation in Bezugsebenen mit zwei verschiedenen Blaulichtquellen (physikalisch, photometrisch, colorimetrisch).
