Integrins and Adhesion Molecules as Therapeutic Targets in Multiple Sclerosis

Inflammation it is a feature of this illness and it is the cause of the damage that the myelin suffers and by consequence, the axons. The circulating leukocytes in the bloodstream are recruited do the local of inflammation and among these the T cells are one of the cells that can cross the blood-brain barrier. After this the T cells are reactivated and trigger an inflammatory cascade from which results the release of cytokines, chemokines and other molecules. This crossing it is possible due to the α4 integrins, transmembrane proteins with different subunits like β1 and β7. The α4β1 integrin located at the surface of T cells bind VCAM-1 at the surface of the vascular endothelial cells and allow the mentioned crossing. This step of the inflammation it is crucial to reach the axon myelin. Given this, there is all the interest about acting on these transmembrane proteins, blocking its action in a way that can stop the passage through the central nervous system by the T cells. To compete for this purpose some possible drugs were studied and as a result of this, antibodies, small molecules and peptides emerged. The monoclonal antibody Natalizumab aroused on the previous decade as a therapeutic option after showing results of efficacy and safety at the clinical trials. Unfortunately, some reports of Multifocal Progressive Leukoencephalopathy were associated with it. Firategrast, a small integrin antagonist molecule, emerged after Natalizumab as an alternative to it with a smaller half-life and oral administration. Other examples of integrin antagonists are: CT301, CDP323 and HCA3551. Some of these have demonstrated potential to realize clinical trials and development. Given the written above it is possible to notice that some solutions were and still are being proposed as drugs to act on the integrins and it will be about those solutions that the following revision will be about.Afetando milhões de pessoas em todo o mundo, a Esclerose Múltipla (EM) apresenta-se como uma doença autoimune, crónica e degenerativa que afeta o Sistema Nervoso Central (SNC) provocando a perda de mielina nos axónios e afetando a matéria branca.A inflamação, característica da EM, é a causa do dano que é provocado na mielina e, por consequência, nos axónios. Os leucócitos circulantes no sangue são recrutados para o local da inflamação e, de entre estes, os linfócitos T são umas das células que têm a capacidade de atravessar a Barreira Hematoencefálica (BHE) sendo que, após isto, sofrem reativação desencadeando uma cascata inflamatória da qual resulta libertação de citocinas, quimiocinas e outras moléculas. A passagem para o SNC é possível devido às integrinas α4, proteínas transmembranares com diferentes subunidades como a β1 e a β7. A integrina α4β1 à superfície dos linfócitos T liga-se à molécula-1 de adesão celular vascular (VCAM-I) na superfície das células do endotélio capilar, permitindo a supracitada passagem. Este passo da inflamação é crucial para alcançar a mielina dos axónios.Deste modo existe o interesse em atuar sobre estas proteínas transmembranares, bloqueando a sua ação, por forma a impedir que ocorra a passagem da BHE. Para este objetivo foram pesquisados possíveis fármacos e, surgiram neste seguimento, anticorpos, pequenas moléculas e peptídeos.O anticorpo monoclonal Natalizumab surgiu na passada década como opção terapêutica após resultados de eficácia e segurança nos ensaios clínicos. No entanto, casos de Leucoencefalopatia Multifocal Progressiva (LMP) foram associados ao mesmo. O Firategrast, pequena molécula antagonista das integrinas, surgiu após o Natalizumab como alternativa ao mesmo com um menor tempo de semivida e administração per os em opção à via intravenosa. Existem outros exemplos de antagonistas das integrinas que foram sujeitos a ensaios clínicos: CT301, CDP323 e HCA3551 tendo algumas destas moléculas demonstrado potencial para realização de ensaios clínicos e desenvolvimento.Posto isto, é possível perceber que algumas soluções foram e ainda estão a ser propostas no que toca às integrinas como alvos terapêuticos e será sobre essas mesmas soluções que a seguinte revisão bibliográfica incidirá
