Impact of physical activity on the cognitive function of the elderly: interaction between skeletal muscle and nervous system

Normal aging is characterized by subtle changes in cellular homeostasis such as decreased antioxidant capacity and accumulation of dysfunctional proteins. These disorders progressively result in physiological deterioration, especially in tissues with high energetic dependence, such as the brain and skeletal muscle. Physical exercise contributes to healthy aging and reduces the morbidity rates of various age-related diseases. Being an important modifiable behavioral factor, more and more health promotion activities in all ages, alert to the importance of the practice of physical exercise, properly and safely. A search was performed in the Medline (Pubmed) database using combinations of the terms exercise, brain, muscle, aging and cognitive, including articles from the last 5 years, with the objective of: (i) describing cellular and physiological changes associated with skeletal muscle and nervous system aging, (ii) to demonstrate the protective effect of physical exercise, and (iii) to discuss the possible mechanisms mediating the endocrine action of skeletal muscle on the central nervous system. In addition to the beneficial effect of physical exercise on motor skills and fragility, several recent studies have revealed the positive impact of exercise practice on aging in the nervous system, being a protective factor against cognitive decline. Stimulation of neurogenesis and synaptic plasticity, as well as structural modifications of dendritic morphology, prevention of neuroinflammation, normalization of cerebral vascular perfusion and stimulation of energy metabolism seem to be essential avenues for the effects of physical exercise on learning and memory. Although the interaction between the central nervous system and the skeletal muscle is focus of research, the mechanisms linking skeletal muscle physiology and function with cerebral neuroplasticity are not yet clear. Skeletal muscle is an endocrine organ, releasing humoral factors in response to muscle contractions. Recent studies suggest that in response to exercise, myokines such as irisin and the neurotrophic factor derived from the brain act as systemic mediators that trigger the process of structural and functional adaptation of the brain to exercise.O envelhecimento da população humana é inquestionável, com estimativas mundiais a prever que em 2050, mais de 2 biliões de pessoas terão acima de 65 anos de idade. O envelhecimento normal caracteriza-se por alterações subtis na homeostase celular como por exemplo diminuição da capacidade antioxidante e acumulação de proteínas disfuncionais. Progressivamente, estas perturbações resultam em deterioração fisiológica, especialmente em tecidos com elevada dependência energética, como o cérebro e o músculo esquelético. O exercício físico contribui para o envelhecimento saudável e reduz as taxas de morbilidade de várias doenças relacionadas com a idade. Sendo um importante fator comportamental modificável, cada vez mais atividades de promoção de saúde em todas as idades, alertam para a importância da prática de exercício físico, de forma adequada e segura. Foi feita uma pesquisa na base de dados da Medline (Pubmed) usando combinações dos termos exercise, brain, muscle, aging e cognitive, incluindo artigos dos últimos 5 anos, com o objetivo de: (i) descrever as alterações celulares e fisiológicas associadas ao envelhecimento no músculo esquelético e no sistema nervoso, (ii) evidenciar o efeito protetor da prática de exercício físico e (iii) discutir os possíveis mecanismos mediadores da ação endócrina do músculo esquelético sobre o sistema nervoso central. Para além do efeito benéfico da prática de exercício físico, ao nível da capacidade motora e fragilidade, diversos estudos recentes têm revelado o impacto positivo da prática de exercício no envelhecimento ao nível do sistema nervoso, sendo evidente um fator de proteção contra o declínio cognitivo e demência. A estimulação da neurogénese e da plasticidade sináptica, assim como modificações estruturais da morfologia dendrítica, prevenção da neuroinflamação, normalização da perfusão vascular cerebral e estimulação do metabolismo energético parecem ser vias essenciais para os efeitos do exercício físico na aprendizagem e memória. Apesar da interação entre o sistema nervoso central e o músculo esquelético ser foco de investigação, não estão ainda claros os mecanismos que relacionam a fisiologia e a função do músculo esquelético com a neuroplasticidade cerebral. O músculo esquelético é um órgão endócrino, libertando fatores humorais em resposta às contrações musculares.
