The Critically Ill Child in The Pediatric Emergency Service

Trabalho de Projeto do Mestrado Integrado em Medicina apresentado à Faculdade de MedicinaIntroduction: Critically ill patients in pediatric emergency departments (PED) are rare, even in tertiary care hospitals, however these departments should be prepared to identify and stabilize these patients, with a variety of ages and pathology. The objective of this strudy was to characterize the critically ill patient profile that presents to PED, in order to optimize the organization of the department and professionals skills.Methods: Retrospective study of 5 years (2011-2016), performed in a tertiary PED. All the episodes of children and teenagers (<18 years) sorted as priority I (red color), according to the portuguese adapted version of the Canadian Paediatric Triage and Acuity Scale (PaedCTAS) were included. Demographic variables, observation periods, place of origin and transportation, relevant pathologic background, major complaint and final diagnosis, management in the PED and outcome were analyzed. Statistical analysis with SPSS® 23.Results: 178 episodes were included, with increasing distribution over the years (median 37 cases/year). The median age was 6,2 years (AIQ 2,3-13,3), and 62,9% were male. Critically ill patients were admitted in every hour of the day, but the largest number of cases were during the afternoon period between 16 and 24 hours (53,4%).In 65,6% of cases medical transportation were used and 24,3% were bought by own means of transportation. The attribution of triage level I was determined by trauma in 33,7% of cases, respiratory distress in 23,0%, seizure in 22,5% and altered mental status in 10,7%. In 12 cases (6,7%) cardiopulmonary resuscitation maneuvers were performed. Trauma was most frequent final diagnosis (38,2%). There were 7 deaths (3,9%) in the PED, all with severe trauma. In 6 cases there were records of pre-hospital cardiopulmonary arrest (CPA). The majority had a good outcome, however 17 deaths (9,6%) were recorded in total.Diagnosis of trauma (p=0,001), medical transportation (p=0,003) and cardiopulmonary arrest (p<0,001) were the factors related with mortality. There were no significative differences in mortality related to age, sex and observation period.Discussion/Conclusions: Critically ill patients are rare in PED, however in this study, they appeared to be increasing. Trauma was the most frequent diagnosis. Mortality was low, but strongly associated with the diagnosis of trauma, medical transportation and CPA. It is important to implement measures of accident prevention in the community and optimize emergency teams, which should be multidisciplinary and have continuous training in resuscitation and trauma.Introdução: O atendimento de doentes críticos nos serviços de urgência pediátricos (SUP) é pouco frequente, mesmo em hospitais terciários, no entanto estes devem estar preparados para identificar e estabilizar este tipo de doentes, de todas as idades e com patologia variada. Foi objectivo deste estudo caracterizar o perfil do doente crítico observado num SUP, de forma a otimizar a organização e gestão do serviço e as competências dos profissionais.Métodos: Estudo retrospetivo de 5 anos (2011-2016), efectuado num SUP polivalente. Foram incluídos todos os episódios de crianças/adolescentes (<18 anos) triados com prioridade I (cor vermelha), de acordo versão portuguesa adaptada da escala de triagem Canadian Paediatric Triage and Acuity Scale (PaedCTAS). Foram analisadas variáveis demográficas, períodos de observação, proveniência e tipo de transporte, antecedentes patológicos, queixa principal e diagnóstico, orientação no SU e evolução clínica. Foi feita analise estatística com recurso a SPSS® 23.Resultados: Foram incluídos 178 episódios, com distribuição crescente ao longo dos anos (mediana 37 casos/ano). A idade mediana foi 6,2 anos (AIQ 2,3-13,3), e 62,9% eram do sexo masculino. Houve admissão de doentes críticos em todas as horas mas com maior número de casos no período da tarde, entre 16 e 24 horas (53,4%). Em 65,6% dos casos foi utilizado transporte medicalizado e em 24,3% meios próprios. A atribuição de triagem nível I foi determinada por trauma em 33,7% dos casos, dificuldade respiratória em 23,0%, convulsão em 22,5% e alteração do estado de consciência em 10,7%. Foram realizados exames complementares de diagnóstico em 86,0%. Em 12 casos (6,7%) foram efectuadas manobras de ressuscitação cardiopulmonar. O diagnóstico final mais frequente foi trauma (38,2%). Registaram-se sete óbitos (3,9%) no SU, todos com trauma grave. Em seis casos havia registo de paragem cardio-respiratória pré-hospitalar. A maioria dos casos teve evolução favorável, no entanto registaram-se 17 óbitos no global (9,6%).
