DESARROLLO Y VALIDACIÓN DE MÉTODOS DE DIAGNÓSTICO APLICADOS AL SEGUIMIENTO Y CONTROL DE LA TRIPANOSOMIASIS HUMANA AFRICANA EN REGIONES ENDÉMICAS Y NO ENDÉMICAS

The african trypanosomiasis is caused by the protozoan parasites of T.brucei subspecies, T.b. brucei, T.b.gambiense and T.b.rhodesiense, the first one affect only animals and is the primary cause of Nagana while the other two are the causative agents of human trypanosomiasis. In the last few years, the Human African Trypanosomiasis (HAT) or sleeping sickness has re- emerged in the endemic zones of Africa, that revealed the need for new methods of control which help in the reduction of the number of infected cases in endemic regions as well as the number of imported cases in non-endemic regions. One of the most important points that should be considered in the control of any disease is the application of a sensitive, specific methods of diagnosis specially in the case of sleeping sickness, as it is well known that the treatment available is highly toxic and sometimes fatal. Therefore, the need for new diagnostic methods that can be applied to diagnose the infected cases is urgently required. The objective of this study is the development and validation of new diagnostic methods that can be applied to diagnose correctly imported cases of sleeping sickness, control of cases in endemic areas as well as the follow up of the infection in experimental animals. In this study, methods based on genomic amplification as well as methods for the detection of antigens of the sleeping sickness were developed, optimized and validated. In addition to the validation of other serological methods for the diagnosis of HAT in endemic and non-endemic regions. The methods based on genomic amplification developed (nested PCR and real time PCR) have demonstrated elevated sensitivity and specificity for the diagnosis of T.brucei compared to other methods available. In addition, procedures for the control of the disease in endemic regions and the diagnosis and follow up of imported cases of the disease were establishedLa Tripanosomiasis africana es una parasitosis causada por protozoos del grupo de Trypanosoma brucei y transmitida por moscas del género Glossina. Tres subespecies han sido, hasta el momento, caracterizadas dentro del grupo de T.brucei como agentes causales de la enfermedad: Trypanosoma brucei brucei, T. b. gambiense y T. b. rhodesiense. La primera afecta sólo a animales mientras que las otras dos son las que dan lugar a la enfermedad en humanos.La enfermedad del sueño ha re-emergido con fuerza en las zonas endémicas de África, siendo necesario implementar nuevas medidas de control que permitan disminuir la incidencia de la enfermedad, tanto en las zonas endémicas como en los posibles casos exportados a otras regiones. El primer paso en el control de cualquier enfermedad es la realización de un diagnóstico precoz, sensible y específico que demuestre la presencia del patógeno, especialmente necesario en la enfermedad del sueño donde el tratamiento es altamente tóxico y en ocasiones mortal.El objetivo de este trabajo ha sido Implementar procedimientos de diagnóstico y control para la Tripanosomiasis Humana Africana (THA), o enfermedad del sueño, comprobando su eficacia en casos clínicos importados, en la búsqueda activa de casos en áreas endémicas y en el seguimiento de infección en animales de experimentación. Se desarrollaron, optimizaron y validaron métodos basados en amplificación genómica y detección antigénica, además de validar métodos de detección serológica, para el diagnóstico eficaz de la THA en áreas endémicas y no endémicas. Los métodos de amplificación genómica desarrollados, nested PCR y PCR a tiempo real para la detección específica de T.brucei han mostrado una alta sensibilidad y especificidad frente a otros métodos. Además se han propuesto procedimientos de actuación para el control de la enfermedad en zonas endémicas y el diagnóstico y seguimiento para casos importados de la enfermedad
