Il danno osseo da eccesso di cortisolo si determina direttamente attraverso la riduzione dell\u2019apposizione e l\u2019incremento del riassorbimento ossei ed indirettamente attraverso il ridotto assorbimento intestinale e l\u2019aumentata escrezione urinaria di calcio e l\u2019ipogonadismo. Nell\u2019ipercortisolismo conclamato o sindrome di Cushing si verifica osteoporosi complicata da fratture vertebrali in circa il 70% dei casi, anche nei soggetti eugonadici e nel sesso maschile. La guarigione dalla sindrome di Cushing sembra normalizzare il rischio di frattura gi\ue0 dopo 1 anno, mentre la normalizzazione della massa ossea pu\uf2 non essere completa e richiede diversi anni.
