Exploración vestibuloespinal

The vestibular labyrinth does not differentiate between the inclination of the body and the head. The most important sensory systems that solve this problem are the visual and somatosensory system, which associated with vestibular sensory organs detect if the body position coincides with the appropriate posture. On the other hand, the central nervous system automatically sends an order to obtain the necessary postural variation, being carried out by the vestibular-spinal reflex. Considering that the two labyrinths are in a situation of vestibular compensation, a sudden destruction in one of them originates a dominant tone in the muscles of the healthy side. Method: Review. Results: There are two types of tests to assess this reflection: static and dynamic. Among the statics is the Romberg test, with its sensitized variant, and the dynamic tests that include the Babinski-Weil walk test, the Barany test and the Unterberger-Fukuda test (or the simulated walk). Conclusions: In the peripheral vestibular syndromes segmental deviations are towards the hypovalent side (harmonic syndromes), while in the central syndromes can be to any side (disharmonic syndromes).El laberinto vestibular no es capaz de distinguir entre la inclinación del cuerpo y la de la cabeza. Dada esta situación, los dos sistemas sensoriales más importantes que contribuyen a la resolución de este problema son el sistema visual y el sistema somatosensorial, que en conjunto con los órganos sensoriales vestibulares permiten detectar si la posición corporal coincide con la postura adecuada o no. De lo contrario, el sistema nervioso central emite automáticamente una orden para obtener la variación postural necesaria, siendo llevaba a cabo por el reflejo vestibuloespinal, teniendo en cuenta que los dos laberintos están en situación de compensación vestibular, una destrucción brusca en uno de ellos va a originar un tono dominante en los músculos del lado sano. Método: Revisión narrativa. Resultados: Existen dos tipos de pruebas para valorar dicho reflejo: estáticas y dinámicas. Entre las estáticas se encuentra la prueba de Romberg, con su variante sensibilizada. Por otra parte, las pruebas dinámicas incluyen la prueba de marcha de Babinski-Weil, prueba de los índices (Barany) y prueba de Unterberger-Fukuda (o de la marcha simulada). Conclusiones: En los síndromes vestibulares periféricos las desviaciones segmentarias son hacia el lado hipovalente (síndromes armónicos), mientras que en los centrales pueden ser hacia cualquier lado (síndromes disarmónicos)
