Implante percutáneo de prótesis valvular aórtica

Due to the progressive aging of the population, the symptomatic severe aortic stenosis is a pathology that has an increased prevalence in our environement. Although the election treatment is surgery, these very elderly patients present associated co-morbidity which may increase the surgical risk. We present results of our experience and describe our own care protocol. Between 2008 April of and 2009 January, we made percutaneous implantation of aortic prosthesis (Corevalver ®) in 29 patients. The selection criteria were clinical and anatomical (diameters of the plane to valvular and iliofemoral access way). In 27 patients the chosen access was femoral arteria and in 2 cases the access was made through left subclavia. In the cases made by femoral route the access was percutaneous with 18F and sutured with arterial suture Prostar. The patients were under sedation and local anaesthesia. All the patients needed implants for a transcutaneous pacemaker. The mean age was of 77 years [50-92], 12 patients were men and 17 women. The complications related to the puncture were: failure in the Prostar closing and a flight of iliac artery; both needed vascular surgery. A hearth attack within the procedure, caused by a dissection of mammary artery, needed treatment with ACTP and stent. A 40% showed AV blockage, needing transcutáneo pacemaker. It did not have, in any case, problems of implants (migration or coronary obstruction). The aortic valve percutaneous implantation could be a very useful alternative in patients which were rejected for surgery due an elevated risk of death or adverse effects, providing them a solution for their health problem. Actually, this procedure is easy and cost effective.• Introducción: Debido al envejecimiento de la población, la estenosis aórtica severa sintomática es una patología cada vez más prevalente y aunque el tratamiento de elección es la cirugía suelen, estos pacientes de edad muy avanzada, presentar comorbilidad asociada que incrementa el riesgo quirúrgico. • Objetivos: Presentar resultados de nuestra experiencia y describir protocolo de actuación. • Material y Método: Entre abril de 08 y enero de 09 se realizó implante percutáneo de prótesis aórtica Corevalve en 29 pacientes. Los criterios de selección fueron clínicos y anatómicos (diámetros del plano valvular y vía de acceso iliofemoral). En 27 pacientes la vía de acceso elegida fue femoral derecha o izquierda y en 2 casos el acceso se realizó por subclavia izquierda, mediante presentación y cierre quirúrgico. En los casos realizados por vía femoral el acceso fue percutáneo con 18F y el cierre con sutura arterial Prostar. Los implantes se realizaron bajo sedación del paciente y con anestesia local. Precisaron implante de marcapasos transcutáneo. • Resultados: N=29. La edad media fue de 77 años [50-92], 12 hombres y 19 mujeres. Las complicaciones relacionadas con la punción fueron: un fallo en el cierre Prostar y un caso de fuga de arteria iliaca, ambas precisaron cirugía vascular. Un infarto periprocedimiento por disección de arteria mamaria, precisó ser tratada con ACTP y stent. Un 40% presentó bloqueo A-V, precisando marcapasos transcutáneo. No hubo, en ningún caso, problemas de implante (migración u obstrucción coronaria). • Conclusiones: La implantación percutanea de la válvula aortica podría ser una alternativa en pacientes con estenosis aórtica severa con alto riesgo quirúrgico
