Fístula carótido–cavernosa: Presentación de un caso insólito

Carotid-cavernous fistula is an abnormal communication between the carotid artery and the cavernous sinus. It is not a common reason for consultation, which can be difficult to diagnosis by presenting a clinical picture similar to the others, therefore the objectives are to describe the characteristics with which arose the entity in a patient attended in the ophthalmological centre in Santiago de Cuba in the year 2014 and contribute to the dissemination of scientific information for future diagnostics and patients with carotid-cavernous arteriovenous fistula integrated management. Is presented the case of a patient of 49 years of age, white, male, with a history of arterial hypertension, which following a crisis hypertensive presented a proptosis of the right eye with redness. Studies are conducted that allowed the diagnosis of right arteriovenous carotid-cavernous fistula indirect of low flow. It was referred to the Hermanos Ameijeiras Hospital, where study and treatment was continued. Manual external compression of the carotid artery improved the condition. The presentation of this case allows describing the entity to know their features, as well as the disclosure scientific for contribute to them future diagnostics of this entity.La fístula carótido–cavernosa es una comunicación anómala que se establece entre la arteria carótida y el seno cavernoso. No es un motivo frecuente de consulta, por lo cual puede resultar difícil su diagnóstico al presentar un cuadro clínico similar al de otras entidades, por ello los objetivos trazados son describir las características con que se presentó la entidad en un paciente atendido en el Centro Oftalmológico de Santiago de Cuba en el año 2014 y contribuir a la divulgación de la información científica para futuros diagnósticos y manejo integral de pacientes con fístula arteriovenosa carótido–cavernosa. Se presentó el caso de un paciente de 49 años de edad, blanco, masculino, con antecedentes de hipertensión arterial, que a raíz de una crisis hipertensiva presentó una proptosis del ojo derecho con enrojecimiento. Se le realizaron estudios que permitieron el diagnóstico de fístula arteriovenosa carótido–cavernosa derecha indirecta de bajo flujo. Fue remitido al Hospital Hermanos Ameijeiras donde se continúo el estudio y tratamiento. La compresión manual externa de la carótida le reportó mejoría. La presentación de este caso permite describir la entidad para conocer sus características, así como la divulgación científica para aportar a los futuros diagnósticos de esta entidad
