Decisão no parto por cesariana: estudo qualitativo exploratório

In this study we intend to understand how it settles the decision of cesarean delivery both a scheduled and non-scheduled, and identify causes and psychological implications of this decision. We opted for a qualitative methodology that would allow a detailed analysis of the data and identify the relationships between variables. Semi-structured interviews were conducted with women between 24 hours and 72 hours post-delivery, mothers undergoing elective and emergency cesarean delivery in a hospital in the Alentejo region in Portugal. Data were collected and analyzed according to grounded theory. Through an exercise in coding and creating interpretive categories, it was realized that the decision for cesarean delivery has emerged as a central category. The medical decision has greater impact, and the obstetrician presents itself as an agent of decision making. It was possible to understand that decision making in obstetrics are difficult to interpret because of the clinical character. Most women, when the decisions do not identify physiological and psychological risks to themselves, mainly focusing on the benefits and welfare of the baby, not being involved in decision making.Pretendeu-se compreender a forma como se estabelece a decisão de parto por cesariana tanto de forma programada como não programada e identificar causas e implicações psicológicas dessa decisão. Optou-se por uma metodologia qualitativa que permitisse uma análise aprofundada dos dados e a identificação das relações entre as variáveis. Realizaram-se entrevistas semiestruturadas entre as 24 e as 72 horas pós parto, a puérperas submetidas a cesariana electiva e de urgência num hospital da região Alentejo em Portugal. Os dados foram colhidos e analisados de acordo com a grounded theory. Através de um exercício de codificação interpretativa e da criação de categorias, percebeu-se que a decisão para parto por cesariana emergiu como categoria central. A decisão médica é que tem maior impacto e o obstetra apresenta-se como agente da tomada de decisão. Compreendeu-se que as tomadas de decisão em obstetrícia são difíceis de interpretar pelas devido ao carácter clínico. Na sua maioria, as mulheres, quando das tomadas de decisão não identificam para si próprias riscos fisiológicos e psicológicos, centrando-se sobretudo nos benefícios e no bemestar do bebé, não sendo envolvidas nas tomadas de decisão
