Lupus eritematoso sistémico asociado a parvovirus B19

Introduction: Systemic lupus erythematosus (SLE) is an autoimmune disease. Some scientific evidence exists implicating infectious agents as triggers of the disease. The parvovirus B19 has also been implicated in rheumatoid arthritis in adults, juvenile idiopathic arthritis (associated with HLA B27), vasculitis, lupus, histiocytic necrotizing lymphadenitis (Kikuchi's disease), systemic sclerosis, uveitis, myositis, and other rheumatologic diseases. Case Report: A 12-year-old male patient presented at 21 days of symptoms of intermittent fever, loss of appetite, weight loss, paleness, and cutaneous rash, predominantly of the upper body. Upon examination the patient showed hypotrophy and a distended abdomen as well as data indicating mild dehydration, with no splanchnomegaly found on palpation. Laboratory results showed normocyticnormochromic anemia, leukopenia with lymphopenia, elevated acute phase reactants, normal liver and kidney function, and negative microbiology and serology. Tomography showed bilateral pleural effusion and ascites, while echocardiography showed pericardial effusion. Bone marrow aspirate showed hemophagia, positivity for antinuclear and anti-dsDNA antibodies, and C3 and C4 consumption, while serology (IgM) was positive for parvovirus B19.Therapy was initiated with prednisone, with remission of fever at 48 hours. At week 4, nephrotic proteinuria developed, cycles of cyclophosphamide were added to treatment, and the patient was referred to Boston (USA), where renal biopsy was performed and the final diagnosis of class IV (WHO) diffuse proliferative glomerulonephritis with active and chronic lesions was reached. Progress following treatment was satisfactory, with resolution of proteinuria, normalization of the hemolytic complement, and negative antinuclear and anti-dsDNA antibody findings. Discussion: The patient presented a profile of infection by parvovirus B19, while we believe the SLE profile was secondary to the infection. In our patient the nephrotic syndrome was preponderant. The presence of hypocomplementemia and elevated anti-dsDNA caused renal involvement, which creates a predisposition to nephropathy. The molecular mimicking of certain epitopes by infectious agents are sufficiently similar to those of the host to cause the immune response they trigger to misidentify host antigens.Introducción: El lupus eritematoso sistémico (LES o Lupus), es una enfermedad autoinmune. Existe alguna evidencia científica involucrando a agentes infecciosos como disparadores de esta enfermedad. Así, el parvovirus B19 ha sido implicado en Artritis Reumatoide en adultos, en Artritis Idiopática Juvenil (asociada al HLA B 27), Vasculitis, Lupus, Enfermedad de Kikuchi, Esclerosis Sistémica, Uveítis, Miositis y otras enfermedades reumatológicas. Caso Clínico: Varón de 12 años de edad, con padecimiento de 21 días de evolución, manifestado por: fiebre intermitente, pérdida del apetito, de peso, palidez y exantema cutáneo de predominio en hemicuerpo superior. A la exploración física, hipotrófico, con datos de deshidratación leve, abdomen distendido, no palpándose visceromegalias. El laboratorio clínico, anemia normocítica normocrómica, leucopenia con linfopenia, reactantes de fase aguda elevados, función hepática y renal normales, estudios microbiológicos y serológicos negativos. Tomografía con derrame pleural bilateral, ascitis, ecocardiograma con derrame pericárdico. Aspirado de medula ósea, con hemofagocitosis, ANA y Anti-DNA positivo, C3 y C4 consumidos, serología (IgM) positivo para parvovirus B19. Se inició terapia con prednisona, remitiendo la fiebre a las 48hs. A la 4ta. semana de la enfermedad desarrolla proteinuria en rangos nefróticos, se agrega al tratamiento ciclos de ciclofosfamida y se remite al paciente a Boston (EEUU), donde se le realizó biopsia renal con diagnóstico final de Glomerulonefritis proliferativa difusa clase IV (OMS) con lesiones activas y crónicas. La evolución posterior al tratamiento fue satisfactoria con resolución de la proteinuria, normalización del complemento hemolítico y negativización del ANA y Anti-DNA. Discusión: El paciente presentó un cuadro infeccioso por parvovirus B19, la instalación del cuadro de LES, suponemos fue secundario a la infección. En este caso el Síndrome nefrótico fue el preponderante. La presencia de hipocomplementemia y el aumento del Anti-dsDNA determinó la afección renal, lo que determina un factor de predisposición de la nefropatía. El mimetismo molecular de ciertos epítopos de agentes infecciosos son los suficientemente parecidos a los del huésped para que la respuesta inmunitaria provocada por aquéllos cruce con los antígenos propios del huésped
