Ética del reconocimiento recíproco en el ámbito de la salud

Objective. To introduce the concept of "reciprocal recognition" as a tool for the foundation of bioethics and for the analysis of problematic/difficult cases in bioethical reflection. Hypotheses. Ethical and legal positivism has revealed to be insufficient in order to understand and find solutions to the new problems emerged in the health context. Honneth�s bidirectional recognition -also called �mutual recognition� by Ricoeur and Cortina-, allows building up a model of justice, which can be useful in the health sector if applied to relations between patients and staff, among health professionals or between all of them and the health administration. Conflicts in clinical practice, in the management of health or in public health can be explained by the lack of bidirectional recognition. Applied to the analysis of conflicts, the concept of recognition allows transforming troubled relations into partnerships. It is a new approach to the issue, since mutual recognition has barely been considered as a foundation of bioethics up to now. Work methodology. First of all, after having collected ideas from a workshop of the University of Valencia�s Research Group in Bioethics (GIBUV) in 2009, and having reviewed the literature provided by the director of this group, a systematic search of publications in Dilemata, Isegoría and MEDLINE was conducted. In the first two journals the search was carried out by authorship and using the keyword "recognition", whilst in MEDLINE it was limited to articles published in the last ten years -in English, Spanish, French or Italian-, with the keyword groups "ethics, medical and principles and social values� and �recognition and ethics, medical". The article starts considering the state of matter concerning the concept of Honneth�s "mutual recognition" and taking into account other authors� contributions and of the relevant literature. The research is expected to highlight the idea of mutual recognition in the context of health care, with an aim to find an answer to the main issue: whether the recognition might or cannot become the basis on which a new theory of justice applicable to the health field is built.Objetivo. Introducir en la reflexión bioética el concepto de �reconocimiento recíproco�, como herramienta para la fundamentación de la bioética y el análisis de casos problemáticos. Hipótesis. Para entender y buscar soluciones a los nuevos problemas surgidos en el contexto de la atención a la salud, no resulta suficiente el positivismo jurídico ético. El reconocimiento bidireccional, concepto desarrollado por Honneth en su matiz de reciprocidad, denominado mutuo por Ricoeur y Cortina, permite construir un modelo de justicia aplicable al ámbito sanitario, tanto en lo relativo a relaciones entre pacientes y personal, cuanto a las relaciones entre los profesionales, o de todos ellos con la organización sanitaria. Su ausencia en la relación clínica, en la gestión sanitaria, o en la salud pública, es un buen factor explicativo de los conflictos. Aplicado al análisis de conflictos, el concepto de reconocimiento ofrece posibilidades para transformar las relaciones conflictivas en relaciones de cooperación. Todo ello es novedoso, ya que hasta ahora apenas se ha investigado sobre reconocimiento recíproco como fundamento de la bioética. Metodología de trabajo. En primer lugar, tras recoger ideas a partir de un seminario de trabajo (2009) del Grupo de Investigación en Bioética de la Universitat de València (GIBUV) y revisar bibliografía proporcionada por el director del grupo, se realizó búsqueda de lo publicado en las revistas Dilemata e Isegoría, por autor y por palabra clave �reconocimiento�, y en MEDLINE, acotando por artículos de revisión publicados en los últimos diez años (en inglés, español, francés o italiano), con los descriptores �Ethics, Medical and Principles and Social Values� y �Recognition and Ethics, Medical�. El artículo empieza planteando un estado de la cuestión sobre el concepto �reconocimiento recíproco�, partiendo del autor clave (Honneth) e incluyendo otros autores complementarios a éste, así como la bibliografía sobre el tema considerada significativa.
