Pression des nouvelles mentalités sur le DSM -le cas des problèmes religieux ou spirituels

International audienceSuccessive editions of the Diagnostic and Statistical Manual (DSM) punctuate the history of psychiatry since 1980, with each time a revision of classifications. The new axes and categories appear to vary depending on negotiations with social groups rater than on a scientific basis. The DSM then operate as a psychological mirror of the society in which it develops. This permeability to social field expectations and to pressures of new attitudes is examined through the category “religious or spiritual problems” created in the edition of the DSM-IV 1994. Pressed by the arguments of proponents of Transpersonal Psychology, the American Psychiatric Association has defined a category of distressing religious or spiritual experience that may appear as a mental disorder when out of its context, but is merely a “normal reaction” not to evaluate as a pathology. The arguments of transpersonal psychologists are here the subject of a critical analysis. When defining normality in relation to religions and spiritualities, the DSM has opened the door to a series of experiences with blurred contours in a very important reversal of the relation between psychiatry and beliefs. But there is concern that this approach of these experiences without pathology could encourage a psychic economy with confusing subjectivity and a narcissistic focus on fascinating experiences.Les éditions successives du Diagnostic and Statistical Manual of Mental Disorders (DSM) rythment l’histoire de la psychiatrie depuis 1980, avec à chaque fois une refonte des classifications. Les axes supplémentaires et les nouvelles catégories semblent davantage varier en fonction de négociations avec des corps sociaux que sur des bases scientifiques. Le DSM fonctionnerait alors comme le miroir psychologique de la société dans laquelle il se développe. Cette perméabilité aux attentes du champ social et aux pressions des nouvelles mentalités est examinée à travers la catégorie « problèmes religieux ou spirituels » créée dans l’édition du DSM-IV de 1994. Pressée par les arguments des tenants de la psychologie transpersonnelle, l’American Psychiatric Association a défini une catégorie d’expérience religieuse ou spirituelle pénible pouvant apparaître comme un trouble mental si elle est sortie de son contexte, mais qui ne serait qu’une « réaction normale » à ne pas attribuer à une pathologie. Les arguments des psychologues transpersonnels font ici l’objet d’une analyse critique. En définissant la normalité dans le rapport au religieux et au spirituel, le DSM a ouvert la porte à un ensemble d’expériences aux contours très flous dans un renversement important du rapport de la psychiatrie aux croyances. Mais on peut craindre que cet abord « dépathologisé » de ces expériences vienne conforter une économie psychique floutant la subjectivité au profit d’une centration narcissique sur des vécus fascinants
