Acute renal failure in patients with Hellp Syndrome

A síndrome HELLP ocorre em gestantes ou puérperas e é caracterizada pelo surgimento de hemólise, elevação de enzimas hepáticas e plaquetopenia. Desenvolve-se em aproximadamente 10% das gestantes com pré-eclampsia, sendo mais freqüente entre a 22ª e a 36ª semana de gestação. Insuficiência renal aguda é uma complicação freqüente e grave nessas pacientes. O objetivo deste estudo foi avaliar, em pacientes com síndrome HELLP, a prevalência de insuficiência renal aguda e estudar fatores a ela associados. Foram estudadas 49 gestantes com síndrome HELLP, internadas no Hospital de Clínicas de Porto Alegre, no período de janeiro de 1990 a fevereiro de 2000. A elevação da creatinina sérica acima de 1,5 mg/dL foi o critério utilizado para definir insuficiência renal aguda. As pacientes foram divididas em dois grupos: grupo 1 – pacientes que apresentaram insuficiência renal aguda; e grupo 2 – pacientes que não apresentaram alteração da função renal. O grupo 1 foi constituído de 23 pacientes (46,9%), dos quais 13 (26,6%) recuperaram gradualmente a função renal, e 10 (20,4%) necessitaram de tratamento dialítico. Dos pacientes submetidos à diálise, três recuperaram função renal, e três permaneceram em hemodiálise crônica (6,1%) por danos renais irreversíveis. Ocorreram seis óbitos (12,2%): cinco deles (10,2%) nas pacientes do grupo que desenvolveu insuficiência renal. Concluímos que a insuficiência renal aguda e crônica foi uma complicação freqüente e grave da síndrome HELLP nas gestantes estudadas. As pacientes submetidas à diálise apresentaram maior morbimortalidade e pior prognóstico. Essa situação pode levar à insuficiência renal terminal, com necessidade de diálise crônica indefinidamente, com graves repercussões clínicas, econômicas e sociais.HELLP syndrome occurs in pregnant and puerperal women and is characterized by hemolysis, elevated liver enzymes and low platelet count. It is present in approximately 10% of pregnant women with pre-eclampsia, being more frequent between the 22nd and 36th weeks of pregnancy. Acute renal failure is a severe and relatively common complication associated with this syndrome. The objective of this study was to evaluate, in patients with HELLP syndrome, the prevalence of and factors associated with acute renal failure. We studied 49 pregnant women with HELLP syndrome admitted to Hospital de Clínicas de Porto Alegre between January 1990 and February 2000. Increased serum creatinine levels higher than 1.5 mg/dL were the standard used to define acute renal failure. The patients were divided into two groups: group 1 – patients who developed acute renal failure; group 2 – patients who did not present changes in renal function. Group 1 was composed of 23 (46.9%) patients, of which 13 (26.6%) gradually recovered renal function, but 10 patients (20.4%) required hemodialysis. Among patients requiring dialysis, three recovered renal function and three remained in chronic hemodialysis (6.1%) due to irreversible renal damage. There were six deaths (12.2%), five (10.2%) of them in patients belonging to the group that developed renal failure. We concluded that acute and chronic renal failure was a frequent and severe complication of HELLP syndrome in the pregnant patients under investigation. The patients requiring dialysis presented a worse prognostic with higher morbidity and mortality rates. That situation can lead to terminal renal failure with need of chronic dialysis indefinitely, resulting in serious clinical, social and economic consequences
