Efeitos do treinamento fisíco sobre a rigidez arterial em pacientes com doença renal crônica em hemodiálise e em tratamento conservador

Introduction: Patients with CKD cardiovascular diseases are the leading cause of death. These patients have many cardiovascular risk factors, and among the emerging highlight the increased aortic arterial stiffness, measured by carotid-femoral PWV. On the other hand, it is known that aerobic capacity is reduced in renal patients, and this reduction is associated with increased mortality in those patients. Since physical inactivity is a potentially modifiable risk factor, conducting physical training protocols has been suggested as nondrug therapy for CKD. In the general population, physical training is associated with improved arterial stiffness. However, few studies have evaluated the impact of physical training on aortic compliance in chronic renal. Thus, the hypothesis of this study is that exercise training increases the maximum oxygen consumption and decreases arterial stiffness in patients with CKD. To assess the effects of an exercise training protocol on arterial stiffness, central blood pressure, thickness of the intima-media thickness of carotid and echocardiographic parameters in patients with chronic kidney disease on dialysis and conservative treatment, and the interaction of these factors with clinical and laboratory variables. Esses pacientes apresentam diversos fatores de risco CV, e dentre os emergentes destaca-se o aumento da rigidez arterial aórtica, avaliada pela VOP carótido-femoral. Por outro lado, sabe-se que capacidade aeróbica é reduzida em doentes renais e essa diminuição é associada a aumento da mortalidade nesses pacientes. Visto que a inatividade física é um fator de risco potencialmente modificável, a realização de protocolos de treinamento físico vem sendo sugerida como terapêutica não-medicamentosa para a DRC. Na população geral, o treinamento físico associa-se a melhora da rigidez arterial. Entretanto, poucos estudos têm avaliado o impacto do treinamento físico sobre a complacência aórtica em renais crônicos. Dessa maneira, a hipótese do presente estudo é que o treinamento físico aumenta o consumo máximo de oxigênio e diminui a rigidez arterial em pacientes com DRC. Objetivos: Avaliar os efeitos de um protocolo de treinamento físico sobre a rigidez arterial, pressão arterial central, espessura da camada íntima-média de carótida e parâmetros ecocardiográficos em pacientes com doença renal crônica em hemodiálise e em tratamento conservador, bem como a interação desses fatores com variáveis clínicas e laboratoriais. Sujeitos e Métodos: Indivíduos que realizam acompanhamento no ambulatório de insuficiência renal crônica ou em tratamento hemodialítico no Hospital das Clínicas da Faculdade de Medicina de Botucatu.
