Interleukin 36α in the pathogenesis of inflammatory arthritis

Hintergrund: Interleukin (IL)-36α ist ein neues Mitglied der Interleukin-1 Familie, dessen pro-entzündliche Eigenschaften in der Psoriasis bereits erwiesen sind. Dies in Kombination mit dem Ergebnis unserer Vorprüfungen, in denen wir das neue Zytokin in Lymphgewebe nachweisen konnten, führte zu der Hypothese, dass IL-36α auch für die Pathogenese entzündlicher Arthritiden von Bedeutung sein dürfte. Zielsetzung: Ziel der vorliegenden Arbeit war es herauszufinden, inwieweit IL-36α in Psoriasisarthritis (PsA) im Vergleich zu rheumatoider Arthritis (RA) und Osteoarthritis (OA) exprimiert wird und welche Zellen dieses Zytokin produzieren. Methoden: Synovialgewebe von Arthritis-Patienten wurde mit Hilfe von Immunhistochemie und Immunfloureszenz zur Detektion von IL-36α, IL-36 Rezeptor (IL-36R) und IL-36R Antagonist (IL-36Ra) angefärbt. Ergebnisse: Die Expression des IL-36R und seiner Liganden IL-36α und IL-36Ra wurde im Bereich der ersten Zellschicht der Synovialmembran und in zellulären Infiltraten der Arthritisgewebe beobachtet. Die IL-36α-Expression war in PsA- und RA- signifikant höher als in OA-Synovium. Durch Immunfloureszenz konnten wir CD-138-positive Plasmazellen als wesentlichen zellulären Ursprung von IL-36α identifizieren, aber auch B-Zellen ließen sich mit CD20 und IL-36α doppelfärben. Interessanterweise wurden keine Unterschiede in der Expression von IL-36R und IL-36Ra zwischen PsA, RA und OA beobachtet. Die Ergebnisse dieser Arbeit wurden durch Frey et al publiziert. Schlussfolgerungen: Das neu entdeckte Zytokin IL-36α wird in PsA- und RA-Synovium hochreguliert und in erster Linie von Plasmazellen exprimiert. Ob die IL-36 Familie als potentielle Angriffstelle in der Therapie entzündlicher Arthritiden fungieren kann, muss in weiterführenden Studien getestet werden.Background: Interleukin (IL)-36α is one of the novel IL-1 cytokine family members, whose pro-inflammatory properties have already been proven for psoriasis. Own data on IL-36α expression in lymphoid tissue led us to the hypothesis that it could also play an important role in inflammatory arthritis. Objective: Aim of this study was to determine the IL-36α expression in psoriatic arthritis (PsA) compared to rheumatoid arthritis (RA) and osteoarthritis (OA) and to identify which cells express IL-36α. Methods: Immunohistochemical and immunoflourescent staining was performed for IL-36α, IL-36 receptor (IL-36R) and IL-36R antagonist (IL-36Ra) on synovial tissues obtained from arthritis patients. Results: IL-36R and its ligands IL-36α and IL-36Ra were expressed in the synovial lining layer as well as in cellular infiltrates of the arthritis tissues. However, IL-36α expression was significantly higher in PsA and RA than in OA synovium. Via immunoflourescence, we could identify CD138-positive plasma cells as the main cellular source of IL-36α, but also B cells were co-stained for both CD20 and IL-36α. Interestingly, the expression of IL-36R and IL-36Ra showed no differences between PsA, RA and OA. The results of this study were published by Frey et al. Conclusion: The cytokine IL-36α is up-regulated in PsA and RA synovium and predominantly expressed by plasma cells. Future studies will address if the IL-36 family plays a critical role in the pathogenesis of RA and PsA
