Expression and regulation of chemokines in polymorphonuclear neutrophil granulocytes in patients with rheumatoid arthritis

Polymorphkernige neutrophile Granulozyten (PMN) stellen den Hauptzelltyp in der entzündlichen Synovialflüssigkeit (SF) von Patienten mit rheumatoider Arthritis (RA) dar und sind, wie in Mausmodellen nachgewiesen, an der Induktion und Aufrechterhaltung der Gelenkentzündung beteiligt. Dabei ist unklar, welche Effektormechanismen der PMN zur Gelenkentzündung beitragen. Eine Hypothese ist, dass die SF PMN von RA Patienten im entzündeten Gelenk inflammatorische Chemokine synthetisieren und dadurch die Rekrutierung weiterer Entzündungszellen bewirken. Das Ziel dieser Arbeit war, die Chemokin Expression und Regulation in den SF PMN von RA Patienten mittels Mikroarray Analyse, qualitativer und quantitativer reverser Transkription-Polymerasekettenreaktion (RT-PCR) und Enzym-gekoppelten Antikörper Bindungs-Assays (ELISA) zu bestimmen. Die durchgeführten Expressionsanalysen identifizierten für die SF PMN von RA Patienten ein charakteristisches Expressionsprofil mit den hochregulierten mRNAs der Chemokine CCL3, CCL18, CXCL8 und CXCL10 und der Chemokinrezeptoren CXCR4 und CCRL2. Darüber hinaus lag die Mehrzahl der inflammatorischen Gene (86%) in den SF PMN der RA Patienten im Vergleich zu Blut PMN gesunder Spender herunterreguliert vor. Für das in dieser Arbeit erstmalig in PMN nachgewiesene Chemokin CCL18 zeigte sich eine auf mRNA- und Proteinebene unabhängige Regulation. Die Stimulation von Blut PMN mit Tumor Nekrose Faktor (TNF) alpha führte zur CCL18 mRNA Induktion, ohne dass CCL18 Protein nachweisbar war. Die Proteinsynthese von CCL18 in PMN fand durch zwei Mechanismen statt. Zum einen wurde CCL18 Protein durch Stimulation mit Interleukin (IL)-10 und Vitamin D3, unabhängig von exogenem TNF alpha, zum anderen durch den direkten Zell-Zellkontakt mit Endothelzellen (EC), abhängig von endogenem TNF alpha, induziert. Durch die insgesamt verringerte transkriptionelle Aktivität und die selektive Hochregulation von Chemokinen zeichnet sich ab, dass eine krankheitsfördernde Aktivität der SF PMN von RA Patienten darin besteht, inflammatorische Chemokine zu synthetisieren. Die Induktion von CCL18 findet vermutlich sowohl während der Extravasation in das entzündete Gewebe im Kontakt mit EC, als auch in der SF, durch Stimulation mit IL-10, statt. Diese Befunde schaffen neue Erkenntnisse über die Rolle der PMN in der Pathogenese der RA und können als Grundlage für die Entwicklung neuer Behandlungskonzepte dienen.Polymorphonuclear neutrophils (PMN) represent the predominant cell type in the inflammatory synovial fluid (SF) of patients with rheumatoid arthritis (RA) and, as demonstrated in mouse models, they participate in the initiation and progression of rheumatoid arthritis. However, the effector mechanisms of PMN, contributing to joint inflammation, remain unclear. One hypothesis is that SF PMN from RA patients synthesize inflammatory chemokines and thereby cause the recruitment of other inflammatory cells into the joint. The goal of this work was to determine the expression and regulation of chemokines in SF PMN from RA patients using microarray analysis, qualitative and quantitative reverse transcription-polymerase chain reaction (RT-PCR) and enzyme-linked immunosorbent assays (ELISA). The expression analysis of SF PMN from RA patients identified a characteristic expression profile with the highly up-regulated mRNAs of the chemokines CCL3, CCL18, CXCL8 and CXCL10 and the chemokine receptors CXCR4 and CCRL2. In addition, most of the inflammatory genes (86%) in SF PMN of RA patients were down-regulated when compared to blood PMN from healthy donors. The chemokine CCL18, for the first time detected in PMN, was regulated independently at levels of mRNA expression and protein synthesis. The stimulation of blood PMN with tumor necrosis factor (TNF) alpha resulted in the induction of CCL18 mRNA without detectable CCL18 protein. The protein synthesis of CCL18 in PMN involved two mechanisms. On one hand, CCL18 protein was induced by stimulation with interleukin (IL)-10 and vitamin D3, independently of exogenous TNF alpha. On the other hand, CCL18 was induced through direct cell-cell contact with endothelial cells (EC), requiring endogenous TNF alpha. The reduced overall transcriptional activity and selective up-regulation of chemokines suggest a pathological acitivity of SF PMN from RA patients caused by the production of inflammatory chemokines. It is likely to assume that the induction of CCL18 takes place both during extravasation into the inflamed tissue in contact with EC, as well as in SF, by stimulation with IL-10. These findings provide novel insights into the role of PMN in the pathogenesis of RA and can serve as a basis for the development of new treatment approaches
