The study of expressed emotion in mothers of children and young adolescents with depression in relation to the course of their therapy

The discovery and therapeutic use of psychotropic drugs, during the fifties, allowed for many chronic hospitalized psychiatric adult patients to return to their families. The fact that some of them, despite their drug medication, soon after relapsed, while others did not, shifted the specialists’ focus to the family environment and how it might influence the course of the psychiatric illness. In consequence, the family atmosphere and its aspects were primarily studied in relation to adult schizophrenia and its relapses, but soon after, the research field was enlarged in order to include other psychiatric disorders.In the context of a particular research direction which examined the quality of the relations between patients and their relatives in relation to the course of the psychic disorder, the index of “Expressed Emotion” (EE) was accrued. EE indicates a number of central aspects of interpersonal relations. It includes measurements of critical attitude, hostility and emotional over-involvement on the part of a significant other, usually a family member, towards the psychiatric patient. Vast research work in this field has shown EE as a predictive index for the outcome of schizophrenia as well as for a number of other psychiatric and somatic conditions (depression, obsessive compulsive disorder, agoraphobia, anorexia nervosa, mental retardation, epilepsy, inflammatory bowel disease etc.)Regarding adult’s depression, the research findings indicate that high level of EE in the family is related to a negative outcome of the patient’s illness (prolonging of the depressive episode, relapses) as well as to the severity of the depressive episode. Comparatively, few studies have been conducted in samples of children and adolescents, though they also provide the same strong findings of correlation between parental EE and parental psychopathology (especially maternal depression) as well as depression in children. The investigation of the family factors in relation to the incidence or course of childhood depression has special clinical interest for two basic reasons: a) because children are psychically integrated within the family relationships and the connection between family dysfunction and the child’s psychic disorder is specifically strong, and b) because, in comparison to adults, the use and effectiveness of anti-depressive drugs is very restricted in the case of children and adolescents.It is in this scientific field that the present study attempts to contribute: that is, to investigate the family factors in relation to childhood depression. Its aim, in particular, is to study maternal EE and its correlation to the course of therapy in children and young adolescents (age 9-15), who suffer from Major Depressive Disorder or / and Dysthymic Disorder. This was investigated by means of a number of questions that examine variables from a relatively large field of research. That is, maternal EE was studied: a) in relation to mother’s clinical-psychiatric and social-demographic factors, b) in relation to the severity of the child’s clinical picture or social adjustment, and c) in relation to the changes of the child’s clinical picture in the course of its therapy.The study has been based on research material from a multi-center European research on the psychotherapy of children with depression, conducted by Trowell et al. (2007): “Childhood Depression: a place for psychotherapy: An outcome study comparing Individual Psychodynamic Psychotherapy and Family Therapy”. The children and young adolescents of the sample, together with their parents were randomly allocated to two different but equally established forms of psychotherapy: the Systems Integrative Family Therapy (SIFT) and the Focused Individual Psychodynamic Psychotherapy (FIPP), accompanied by Supportive - Counseling Therapy for the parents. The therapies lasted 9-12 months and the sample was re-evaluated after 6 months.In the present study, cross-sectional measurements were carried out of our variables at three phases of therapy (baseline - end of therapy - follow up) and also measurements were carried out of the changes in between. The 5MSS was used for the measurement of maternal EE, while for the rest of the variables, report questionnaires and diagnostic interviews.Results showed that high EE mothers, in comparison with low EE mothers: are of a lower educational level, attribute more severe depressive symptomatology to their children, are in more disagreement with their children’s self-evaluations, and their children -according to the teachers’ views- show more withdrawn/ internalizing behavior (introversion) in the school environment.Our results converge to an understanding of high maternal EE in the context of a disturbed mother-child interaction. Its correlation with an exaggerated evaluation of the child’s problems, imprints on the mother an attitude of bias in relation to her child. In the clinical research, the dimensions of high EE (“criticism”, “hostility”, “emotional over-involvement”), along with other characteristic attitudes that have been described of mothers (intrusiveness, anger, inflexibility, low tolerance and non-acceptance of the illness, causal attributions of the child’s behavior) can explain their less composed and objective attitude, a bias, regarding the severity of their children’s symptoms. Mothers’ educational level, which was found to correlate with their EE, might influence the formulation or transformation of these attitudes. University level studies, which have been correlated with low EE, seem to cultivate in them a composed scientific consideration of the illness and, consequently, a greater acceptance and tolerance of their children’s symptoms.The psychoanalytic approach to relatives’ high EE attitudes, based on the unconscious psychic mechanism of “projective identification”, can also explain the fact that high EE mothers attribute excessive depressive symptomatology to their children. Melanie Klein (1946) described projective identification as an early defense mechanism, which characterizes primary aggressive object relationships. Undesirable (painful, catastrophic, hostile etc) psychic contents are projected in fantasy from the subject (mother) to the object (child), exerting, at the same time, pressure, so that the object identifies with them. In this case, a fusion occurs of the self with the object, which ceases to be psychically independent. According to its definition, projective identification contains qualities of psychic functions comparable with high EE: aggressiveness - hostility, blurriness of limits - fusion - emotional over-involvement). Such psychic functions are also described in the clinical, as well as the theoretical psychological considerations of separation anxiety, at the core of which, a pathological relation of mutual hostile dependence between mother and child is noted. From this point of view, the correlation between high maternal EE and the child’s withdrawal and introversion in the school environment, according to the teachers, can also be explained.Η ανακάλυψη και η θεραπευτική χρήση των ψυχοφαρμάκων τη δεκαετία του ’50 επέτρεψε σε πολλούς χρόνιους ενήλικες νοσηλευόμενους ψυχιατρικούς ασθενείς να επιστρέψουν στις οικογένειές τους. Το γεγονός ότι, παρά την φαρμακευτική αγωγή, άλλοι υποτροπίαζαν σε σύντομο χρονικό διάστημα και άλλοι όχι, έστρεψε το ενδιαφέρον των ειδικών σε παράγοντες του οικογενειακού περιβάλλοντος, που μπορεί να επηρεάζουν την πορεία της ψυχικής νόσου. Έτσι, η οικογενειακή ατμόσφαιρα και οι πτυχές που τη συνθέτουν μελετήθηκαν αρχικά σε σχέση με τη σχιζοφρένεια των ενηλίκων και τις υποτροπές της, αλλά σύντομα το ερευνητικό πεδίο διευρύνθηκε, για να συμπεριλάβει και άλλες ψυχιατρικές διαταραχές.Στα πλαίσια μιας συγκεκριμένης ερευνητικής κατεύθυνσης, η οποία εξετάζει την ποιότητα των σχέσεων μεταξύ ασθενών και των συγγενών τους, σε σχέση με την πορεία της ψυχικής διαταραχής, προέκυψε ο δείκτης του «Εκφραζόμενου Συναισθήματος» (ΕΣ - “Expressed Emotion”). Το ΕΣ εκφράζει κάποιες κεντρικές πτυχές των διαπροσωπικών σχέσεων. Συμπεριλαμβάνει μετρήσεις της κριτικής στάσης, της εχθρότητας και της συναισθηματικής υπερεμπλοκής, από τη μεριά ενός σημαντικού προσώπου, συνήθως μέλους της οικογένειας, απέναντι στον ψυχικά ασθενή. Μέσα από την πλούσια ερευνητική εργασία, το ΕΣ έχει αναδειχτεί ως προγνωστικός δείκτης για την έκβαση τόσο της σχιζοφρένειας όσο και άλλων ψυχιατρικών και σωματικών καταστάσεων (κατάθλιψη, ιδεοψυχαναγκαστική διαταραχή, αγοραφοβία, ψυχογενής ανορεξία, νοητική καθυστέρηση, επιληψία, φλεγμονώδεις νόσοι του εντέρου κ.α.).Αναφορικά με την κατάθλιψη των ενηλίκων, τα ερευνητικά δεδομένα υποστηρίζουν, ότι το υψηλό επίπεδο του ΕΣ στην οικογένεια συσχετίζεται με κακή έκβαση της νόσου του ασθενούς (παράταση του καταθλιπτικού επεισοδίου, υποτροπές), αλλά και με την ίδια τη βαρύτητα του καταθλιπτικού επεισοδίου. Συγκριτικά, πολύ λίγες μελέτες έχουν διεξαχθεί σε δείγματα παιδιών και εφήβων, οι οποίες παρέχουν επίσης ισχυρά ευρήματα συσχέτισης του γονεϊκού ΕΣ, τόσο με ψυχοπαθολογία στους γονείς –και ιδιαίτερα μητρική κατάθλιψη– όσο και με κατάθλιψη στα παιδιά. Η διερεύνηση των οικογενειακών παραγόντων σε σχέση με την επίπτωση ή την πορεία της κατάθλιψης στα παιδιά έχει ιδιαίτερο κλινικό ενδιαφέρον για δύο βασικούς λόγους: α) διότι τα παιδιά συγκροτούνται ψυχικά μέσα στο πλέγμα των οικογενειακών σχέσεων και η σύνδεση μεταξύ οικογενειακής δυσλειτουργίας και ψυχικής διαταραχής στο παιδί είναι ιδιαίτερα ισχυρή, και β) διότι σε σύγκριση με τους ενηλίκους, η χρήση και η αποτελεσματικότητα των αντικαταθλιπτικών φαρμάκων στα παιδιά και στους εφήβους είναι εξαιρετικά περιορισμένη.Σ’ αυτό το πεδίο, επιχειρεί να συνεισφέρει η παρούσα μελέτη: της διερεύνησης, δηλαδή, των οικογενειακών παραγόντων σε σχέση με την παιδική κατάθλιψη. Σκοπός της, ειδικότερα, είναι η μελέτη του μητρικού ΕΣ και η συσχέτισή του με την πορεία της θεραπείας παιδιών και νεαρών εφήβων (9-15 ετών) που πάσχουν από Μείζονα Καταθλιπτική Διαταραχή ή/και Δυσθυμική Διαταραχή. Αυτό διερευνήθηκε μέσω επιμέρους ερωτημάτων, με εξέταση μεταβλητών από ένα ευρύ σχετικό ερευνητικό φάσμα. Έτσι, το μητρικό ΕΣ μελετήθηκε: α) σε σχέση με κλινικούς-ψυχιατρικούς και κοινωνικο-δημογραφικούς παράγοντες της μητέρας, β) σε σχέση με τη βαρύτητα της κλινικής εικόνας ή της κοινωνικής προσαρμογής του παιδιού και γ) σε σχέση με τις μεταβολές της κλινικής εικόνας του παιδιού κατά την πορεία της θεραπείας του.Η μελέτη βασίστηκε στο ερευνητικό υλικό της πολυκεντρικής ευρωπαϊκής έρευνας για την ψυχοθεραπεία στην παιδική κατάθλιψη των Trowell et al. (2007): Childhood Depression: a place for psychotherapy. An outcome study comparing Individual Psychodynamic Psychotherapy and Family Therapy. Τα παιδιά και οι νεαροί έφηβοι του δείγματος μαζί με τους γονείς τους κατανεμήθηκαν τυχαία σε δύο διαφορετικές αλλά εξίσου καθιερωμένες μορφές ψυχοθεραπείας: τη Συστημικά Απαρτιωμένη Οικογενειακή Θεραπεία (ΣΑΟΘ) και την Εστιασμένη Ατομική Ψυχοδυναμική Ψυχοθεραπεία (ΕΑΨΨ), συνοδευόμενη από Συμβουλευτική-Υποστηρικτική Θεραπεία γονέων. Οι θεραπείες διήρκεσαν 9-12 μήνες και το δείγμα επαναξιολογήθηκε μετά 6μηνο. Πραγματοποιήθηκαν συγχρονικές μετρήσεις των μεταβλητών μας στις τρεις φάσεις της θεραπείας (baseline - end of therapy - follow up) και μετρήσεις των μεταβολών τους ανάμεσα σε αυτές. Για τη μέτρηση του μητρικού ΕΣ χρησιμοποιήθηκε το 5MSS και για τις υπόλοιπες μεταβλητές ερωτηματολόγια αναφοράς και διαγνωστικές συνεντεύξεις.Από τα αποτελέσματα προέκυψε ότι, σε σύγκριση με τις μητέρες χαμηλού ΕΣ, οι μητέρες υψηλού ΕΣ έχουν χαμηλότερο μορφωτικό επίπεδο, αποδίδουν βαρύτερη καταθλιπτική συμπτωματολογία στο παιδί τους, βρίσκονται σε μεγαλύτερη ασυμφωνία με τις αυτό-αξιολογήσεις των παιδιών τους, και τα παιδιά τους, από την άποψη των δασκάλων, παρουσιάζουν περισσότερη απόσυρση / εσωτερικευόμενη συμπεριφορά (εσωστρέφεια), στο σχολικό περιβάλλον.Τα αποτελέσματά μας συγκλίνουν στην κατανόηση του υψηλού μητρικού ΕΣ στα πλαίσια μιας διαταραγμένης αλληλεπίδρασης μητέρας-παιδιού. Ο συσχετισμός του με μια υπερβάλλουσα εκτίμηση των προβλημάτων του παιδιού αποτυπώνει στη μητέρα μια στάση «προκατάληψης» (“bias”) σε σχέση με το παιδί της. Στην κλινική έρευνα, οι ίδιες οι διαστάσεις του υψηλού ΕΣ («κριτική», «εχθρότητα», «συναισθηματική υπερεμπλοκή»), μαζί και με άλλες χαρακτηριστικές στάσεις των μητέρων που έχουν περιγραφεί (παρεμβατικότητα, θυμός, ακαμψία, χαμηλή ανοχή και μη αποδοχή της νόσου, αιτιακές προσάψεις σε σχέση με τη νόσο και τον έλεγχο του παιδιού πάνω στα συμπτώματά του) μπορούν να ερμηνεύσουν μια στάση της μητέρας λιγότερο ψύχραιμη και αντικειμενική, μια προκατάληψη όσον αφορά τη σοβαρότητα των συμπτωμάτων του παιδιού της. Το μορφωτικό επίπεδο των μητέρων, το οποίο βρέθηκε να συσχετίζεται με το ΕΣ τους, μπορεί να επιδρά στη διαμόρφωση ή στην τροποποίηση των στάσεων αυτών. Η πανεπιστημιακή σπουδή, η οποία συσχετίσθηκε με χαμηλό ΕΣ, φαίνεται να καλλιεργεί στις μητέρες μια πιο ψύχραιμη, επιστημονική θεώρηση της νόσου, και μεγαλύτερη, κατά συνέπεια, αποδοχή και ανοχή των συμπτωμάτων του παιδιού.Η ψυχαναλυτική προσέγγιση των στάσεων των συγγενών υψηλού ΕΣ, βάσει του ασυνείδητου ψυχικού μηχανισμού της «προβλητικής ταύτισης» (“projective identification”), μπορεί επίσης να ερμηνεύσει την καθ’ υπερβολή απόδοση, από τις μητέρες υψηλού ΕΣ, καταθλιπτικής συμπτωματολογίας στα παιδιά. Η Melanie Klein (1946) περιέγραψε την προβλητική ταύτιση ως έναν πρώιμο αμυντικό μηχανισμό, που χαρακτηρίζει πρωταρχικές, επιθετικές σχέσεις αντικειμένου. Ανεπιθύμητα (οδυνηρά, καταστροφικά, εχθρικά κλπ) ψυχικά περιεχόμενα προβάλλονται φαντασιωσικά από το υποκείμενο (μητέρα) στο αντικείμενο (παιδί) ασκώντας ταυτόχρονα πίεση, προκειμένου το αντικείμενο να ταυτοποιηθεί με αυτά. Σε αυτή την περίπτωση συμβαίνει συγχώνευση του εαυτού με το αντικείμενο, το οποίο παύει να είναι ψυχικά ανεξάρτητο. Σύμφωνα με τον προσδιορισμό της, η προβλητική ταύτιση εμπεριέχει ποιότητες ψυχικής λειτουργίας (επιθετικότητα - εχθρότητα, σύγχυση ορίων – συγχώνευση – συναισθηματική υπερεμπλοκή), συγκρίσιμες με το υψηλό ΕΣ. Τέτοιες ψυχικές λειτουργίες περιγράφονται, επίσης, τόσο στην κλινική όσο και στην ψυχολογική θεώρηση του άγχους αποχωρισμού, στον πυρήνα του οποίου συναντάται η παθολογική σχέση αμοιβαίας εχθρικής εξάρτησης ανάμεσα στη μητέρα και το παιδί. Υπ’ αυτό το πρίσμα μπορεί να ερμηνευτεί επίσης ο συσχετισμός που βρέθηκε ανάμεσα στο υψηλό μητρικό ΕΣ και σε περισσότερη απόσυρση και εσωστρέφεια των παιδιών, από την άποψη των δασκάλων τους
