Influence of anesthesia depth on cerebral oxygenation during major abdominal operations

Background/ Aims: Data on cerebral oxygenation during different levels of general anesthesia is scarce. In this prospective pilot study we examined the effects of two different depths of anesthesia on cerebral oxygenation (rSO2, SjVO2) in a cohort of patients undergoing major abdominal surgery.Methods: Twenty five patients undergoing major abdominal, non-vascular surgery (>2 hours duration) were assigned to sevoflurane cerebral state monitor (CSM) - guided anesthesia: Group A had a cerebral state index (CSI) value of 40-42 and group B had a CSI value of 50-52. SjVO2 and rSO2 were measured at different time points along with jugular PO2, glucose and lactate. Results: rSO2 was not significantly changed during the study time points in any of the two groups and remained stable during the follow up. A significant reduction in SjVO2 was found from first hour to second hour only in group A (p=0.025), while SjVO2 increased significantly from third hour to recovery only in group B (p=0.025). At recovery group B had significantly greater values of SjVO2 compared to group A. Jugular PO2, lactate and glucose were not significantly different between groups. Conclusion: There were no significant differences in cerebral oxygenation between patients undergoing major non-vascular abdominal surgery with CSI values of 41 and 52 respectively.Σκοπός: Σε αυτή την προοπτική πιλοτική μελέτη εξεταστηκε το αποτελεσμα της αναισθησιας σε 2 βαθη, στην οξυγόνωση του εγκεφαλου.Μεθοδοι: 25 ασθενείς που υπεβληθησαν σε κοιλιακη επεμβαση και αναισθησία με sevofluarane διαχωρίστηκαν σε δυο ομάδες: Α με CSI 40-42 και Β με CSI 50-52. Μετρήθηκαν τα SjVO2 και rSO2 καθως και Po2, Glu, lactate στο σφαγιτιδικό αίμα σε διαφορες χρονικές περιόδουςΑποτελέσματα: το rSO2 δεν μεταβληθηκε στατιστικως σημαντικά σε καμία χρονική στιγμή μεταξυ των δυο ομάδων. Υπήρχε σημαντική μειωση του SjVO2 από την πρωτη στην δευτερη ωρα στην ομαδα Α και σημαντική αυξηση από την τριτη ωρα στην ανανηψη στην ομαδα Β. Δεν βρεθηκαν σημαντικές διαφορές στο σφαγιτιδικο PO2, Glu, Lactate μεταξυ των 2 ομάδων. Συμπερασμα: δεν υπηρχαν διαφορες στην εγκεφαλικη οξυγονωση μεταξυ δυο ομαδων με CSI 41 και 52
