Structural stability of two Y-TZP zirconia ceramic materials after laboratory milling procedures and cyclic loading: a Raman spectroscopy study

Introduction: The increasing need for more aesthetic dental restorations and to the development of CAD/Cam systems for restorations that can withstand occlusal forces at the posterior area, have led to Y-TZP zirconia ceramic materials. However crucial disadvantages of this material involve structural instability and transformation of the tetragonal grains to the monoclinic phase, with parallel increase of the grains volume. That structural transformation can be noticed in low temperatures with the presence of water and is known as low-temperature degradation, LTD. The transformation from the tetragonal to the monoclinic phase can also be activated by high temperatures or by various lab procedures such as grinding. The transformation zone is related to the failure of the restoration. If the transformation to the monoclinic phase extends not only on the surface of the zirconia ceramic but also in depth, the increase of the grains size that follows can lead to the formation of microcracks and the failure of the restoration.Purpose: The aim of this study was to identify and calculate of the monoclinic zirconia volume fraction (Vm%) using Raman spectroscopy in two different Y-TZP ceramic materials, commonly used for the fabrication of ceramic frameworks for fixed partial dentures (FPD’s).Material and methods: Two different Y-TZP ceramic materials were used: a) zirconia blocks ZrO2 with Y2O3 as stabilizer (In-Ceram YZ, Vita Zahnfabrik, Bad Sackingen, Germany) for use at a pre-sintered stage with Cerec InLab CAD/CAM system (Sirona Dental Systems GmbH, Bensheim, Germany) (groups YZ) and b) zirconia blocks (Hint-ELs TZP HIP, Hint-ELs GmbH Griesheim, Germany) used as sintered with the DentaCAD CAD/CAM system (Hint-ELs, Griesheim, Germany) (groups H) for the fabrication of three unit ceramic frameworks for FPD’s. The frameworks of all groups had an occlusal and axial wall thickness of 0,5 mm and connector dimentions of 3 Χ 3 mm between premolar and the pontic and 3 Χ 4 mm between the pontic and the molar. The mesio-distal width of the pontic was 11 mm. In total seven frameworks were fabricated using InCeram zirconia material and eight using HintELs. The frameworks were examined by using Raman spectroscopy before porcelain veneering in three different areas (occlusal O, connectors C and margins M) for the calculation of the fraction of the monoclinic phase (Vm%). Following porcelain veneering of the zirconia frameworks the fraction of the monoclinic phase (Vm%) was calculated again. Five of the InCeram ΥΖ and six Hint-ELs FPD’s were artificially aged through dynamic loading in water at a stable temperature of 37 0C with applied load of 80 N for 2.000.000 cycles at a frequency of 2 Hz (120 loads/min). A three point bending test followed with a crosshead speed of 0,5 mm/min.A Raman microscope was used operated under the following conditions: Ar+ laser (λ0=514,5 nm), 10 mW power at sample, 50X LWD objective, 2400 lines/mm grating, 1000 μm space confocal hole. In total 20 different groups for both zirconia ceramics materials were analyzed for the calculation of the fraction of the monoclinic phase (Vm%): at the surface of the ceramic frameworks (groups YZ and H), at the occlusal area (YZO, HO), at the connector area (YZC, HC) and at the marginal area (YZM, HM). After porcelain veneering the Vm% was calculated at both the external (YZE, HE) and internal (YZI, HI) areas of the zirconia frameworks. Finally the Vm% was calculated from the fractured surfaces (YZF, HF), the compressive sides of the fractured surfaces (YZFC, HFC), the middle sides (YZFM, HFM) and the tensile sides (YZFT, HFT) after cyclic loading.Results and conclusions: The results of this study were as follows:1)The percentage of the monoclinic volume that was found on the external surfaces of the zirconia frameworks was quite low (1,90%) on the Hint-ELs specimens but statistically higher than the percentage (0,15%) on the In-Ceram YZ specimens (p˂0,0001).2)The monoclinic phase that was calculated on the external surfaces for the frameworks made by both materials did not show a uniform pattern as there were found statistical differences of the Vm% between groups C, O and M (p˂0,05). At the Vita In-Ceram YZ frameworks measurements at the marginal area (group YZM, 0,28%) showed higher Vm% compared to the other two groups (YZC and YZO). On the contrary the Hint-ELs frameworks showed lower Vm% measurements on the marginal area (HM, 0,99%) compared to the other two groups (HC and HO).3)After porcelain veneering, the Raman analysis showed that the monoclinic phase was developed on both ceramic zirconia materials (p˂0,05). The Vm% was increased from 0,15% to 8,94% (group YZE) for the VITA In-Ceram YZ specimens and from 1,90% to 13,47% (group HE) for the Hint-ELs specimens.4)Confocal Raman spectroscopy showed that the Vm% in a depth of 25 μm for both Y-TZP materials was not higher that 2%.5)After cyclic loading of the veneered zirconia frameworks the fracture load for the Hint-ELs FPD’s (3851,75 N) was found statistically higher than the fractrure load of the In-Ceram YZ FPD’s (2720,05 N) (p˂0,05).6)The percentage of the monoclinic phase on the fracture surface after 3-point bending test was found to be significantly higher on the Hint-ELs ceramic (group HF, 18,52%) than the In-Ceram YZ (group YZF, 4,35%) (p˂0,05). The Vm% on the fractured surface did not show any statistically significant differences regardless of the areas (compressive, middle, tensile) (groups YZFC, HFC, YZFM, HFM, YZFT, HFT) (p˃0,05).Εισαγωγή: Οι αυξημένες ανάγκες για πιο αισθητικές αποκαταστάσεις και παράλληλα η εξέλιξη των συστημάτων CAD/CAM για οδοντιατρικές αποκαταστάσεις που μπορούν να ανταπεξέλθουν ακόμα και στις δυνάμεις που αναπτύσσονται στην περιοχή των οπίσθιων δοντιών έχουν οδηγήσει στην ευρεία χρήση κεραμικών ζιρκονίας Y-TZP. Ένα όμως βασικό μειονέκτημα που παρουσιάζει το συγκεκριμένο υλικό είναι αυτό της αστάθειας δομής, του μετασχηματισμού, δηλαδή, της σταθερής και επιθυμητής τετραγωνικής φάσης κρυστάλλων στην μονοκλινή. Αυτή η αλλαγή της δομής μπορεί να παρατηρηθεί σε χαμηλές θερμοκρασίες παρουσία νερού (low-temperature degradation, LTD), σε υψηλές θερμοκρασίες ή ύστερα από εργαστηριακές εργασίες. Η έκταση και η εξέλιξη του μετασχηματισμού συνδέεται άμεσα με την αποτυχία της αποκατάστασης καθώς αν η αλλαγή στην μονοκλινή φάση προχωρήσει σε βάθος και δεν περιοριστεί στην επιφάνεια του κεραμικού, τότε είναι πιθανό εξ’αιτίας της αύξησης του όγκου των κρυστάλλων να δημιουργηθούν καταστροφικές μικρορωγμές.Σκοπός: Σκοπός της ερευνητικής αυτής εργασίας ήταν ο ποσοστιαίος προσδιορισμός του όγκου της μονοκλινούς φάσης (Vm%) κρυστάλλων ζιρκονίας με φασματοσκοπική μικροανάλυση Raman, σε δύο διαφορετικά κεραμικά υλικά Υ-ΤΖΡ, που χρησιμοποιούνται για κατασκευή ολοκεραμικών σκελετών για ακίνητες προσθετικές αποκαταστάσεις.Υλικά και μέθοδοι: Χρησιμοποιήθηκαν δύο κεραμικά υλικά Υ-ΤΖΡ α) ράβδοι ζιρκονίας ZrO2 με Ύττριο Y2O3 (In-Ceram YZ, Vita Zahnfabrik, Bad Sackingen, Germany) προ-πυροσσυσωματωμένης μορφής για χρήση στη συσκευή Cerec InLab CAD/CAM (Sirona Dental Systems GmbH, Bensheim, Germany) (ομάδες ΥΖ) και β) ράβδοι ζιρκονίας (Hint-ELs TZP HIP, Hint-ELs GmbH Griesheim, Germany) η επεξεργασία των οποίων γίνεται στην τελική πυροσσυσωματωμένη τους μορφή με το σύστημα Hint-ELs DentaCAD CAD/CAM (Hint-ELs, Griesheim, Germany) (ομάδες Η) για την κατασκευή ολοκεραμικών σκελετών τριών τεμαχίων ακίνητων προσθετικών αποκαταστάσεων. Ο σχεδιασμός των ολοκεραμικών σκελετών περιελάμβανε πάχος υλικού 0,5 mm στην μασητική και στις αξονικές επιφάνειες και διαστάσεις συνδέσμων 3 Χ 3 mm μεταξύ προγόμφιου και γεφυρώματος και 3 Χ 4 mm μεταξύ γομφίου και γεφυρώματος. Το εγγύς-άπω εύρος του γεφυρώματος ήταν 11 mm. Συνολικά κατασκευάστηκαν επτά σκελετοί της εταιρείας InCeram και οκτώ της εταιρείας Hint-ELs. Οι σκελετοί εξετάστηκαν με φασματοσκοπία Raman πριν την όπτηση της πορσελάνης σε τρεις διαφορετικές περιοχές (μασητική O, συνδέσμων C και ορίων M), για τον προσδιορισμό του ποσοστού όγκου μονοκλινής ζιρκονίας (Vm%). Ακολούθησε όπτηση πορσελάνης επικάλυψης στους σκελετούς ζιρκονίας και μέτρηση με εφαρμογή φασματοσκοπίας Raman του Vm%. Στη συνέχεια σε πέντε ολοκεραμικές γέφυρες InCeram ΥΖ και έξι Hint-ELs εφαρμόστηκε μηχανική καταπόνηση σε νερό σε θερμοκρασία σταθερή στους 37 0C με δύναμη καταπόνησης 80 Ν για 2.000.000 κύκλους μάσησης και συχνότητα 2 Hz (120 loads/min). Ένα δοκίμιο από κάθε υλικό τοποθετήθηκε στη δεξαμενή νερού στους 37 0C χωρίς εφαρμογή δυνάμεων. Ακολούθησε δοκιμασία των δοκιμίων της αντοχής τους στην κάμψη με ταχύτητα φόρτισης 0,5 mm/min.Η μικροανάλυση με φασματοσκοπία Raman πραγματοποιήθηκε κάτω από τις εξής συνθήκες: Ar+ laser (λ0=514,5 nm), ένταση στο δείγμα 10 mW, μεγέθυνση 50 X LWD, φράγμα διασποράς 2400 lines/mm, κατά βάθος χωρική ανάλυση του δείγματος 1000 μm και χρόνο συλλογής 120 s ανά σημείο. Συνολικά ερευνήθηκαν στα δύο υλικά ΥΖ και Η για τη μέτρηση του Vm% 20 διαφορετικές ομάδες: πριν την όπτηση πορσελάνης (ομάδες ΥΖ και Η), στη μασητική επιφάνεια (ΥΖΟ, ΗΟ), στην επιφάνεια συνδέσμων (ΥΖC, HC) και στην επιφάνεια ορίων (ΥΖΜ, ΗΜ). Μετά την όπτηση πορσελάνης επικάλυψης στην εξωτερική επιφάνεια (ΥΖΕ, ΗΕ) και στην εσωτερική επιφάνεια (ΥΖΙ, ΗΙ). Στις επιφάνειες θραύσης (ΥΖF, HF), στις πλευρές συμπίεσης (ΥΖFC, HFC), στο μέσο των θραυσμένων επιφανειών (YZFM, HFM) και στις πλευρές εφελκυσμού (YZFT, HFT).Αποτελέσματα και συμπεράσματα: Τα αποτελέσματα της μελέτης αυτής κάτω από τις συγκεκριμένες πειραματικές συνθήκες καταλήγουν στα ακόλουθα συμπεράσματα:1)Το ποσοστό μονοκλινούς ζιρκονίας (Vm%) ολοκεραμικών σκελετών της ομάδας Η (1,90%) παρουσιάστηκε σημαντικά μεγαλύτερο (p˂0,0001) από αυτό που μετρήθηκε στην ομάδα ΥΖ (0,15%).2)Παρατηρήθηκαν στατιστικά σημαντικές διαφορές στα ποσοστά μονοκλινούς ζιρκονίας (Vm%) μεταξύ των περιοχών στις περιοχές που εξετάστηκαν (ομάδες C, O M) στις επιφάνειες των δύο τύπων σκελετών (p˂0,05). Στα δοκίμια VITA In-Ceram YZ βρέθηκε σημαντικά υψηλότερη τιμή Vm% στην περιοχή των ορίων (ομάδα ΥΖΜ) (0,28%) συγκριτικά με τις άλλες δύο ομάδες (ΥΖC και ΥΖO), ενώ αντίθετα για τα δοκίμια Hint-ELs το Vm% η περιοχή των ορίων (ομάδα HM) έδωσε σημαντικά χαμηλότερες τιμές (0,99%) συγκριτικά με τις άλλες δύο ομάδες (ΗC και HO). 3)Μετά την όπτηση πορσελάνης επικάλυψης αυξήθηκε το Vm% και στα δύο κεραμικά ζιρκονίας (p˂0,05). Από 0,15% σε 8,94% (ομάδα ΥΖΕ) στα δοκίμια VITA In-Ceram YZ και από 1,90% σε 13,47% (ομάδα ΗΕ) στα δοκίμια Hint-ELs.4)Η εφαρμογή εστιασμένης φασματοσκοπίας Raman κατά βάθος έδειξε ότι το Vm% σε βάθος στα 25 μm δεν ξεπερνά το 2% και στα δύο υλικά Υ-ΤΖΡ.5)Η αντοχή στη κάμψη μετά από μηχανική κόπωση των αποκαταστάσεων Hint-ELs (3851,75 Ν) ήταν σημαντικά μεγαλύτερη από αυτή των αποκαταστάσεων VITA In-Ceram YZ (2720,05 Ν) (p˂0,05). 6)Τα ποσοστά Vm στις επιφάνειες θραύσης ήταν σημαντικά μεγαλύτερα στο κεραμικό Hint-ELs (ομάδα HF) (18,52%) από ότι στο VITA In-Ceram YZ (ομάδα YZF) (4,35%) (p˂0,05).Το ποσοστό της μονοκλινούς ζιρκονίας στην επιφάνεια θραύσης για κάθε ένα υλικό δε διαφοροποιείται ανάμεσα στις τρεις πλευρές (ομάδες YZFC, HFC, YZFM, HFM, YZFT, HFT) (p˃0,05)
