The contribution of clinical and laboratory findings to the clinician's decision to perform a lumbar puncture with a suspicion of meningitis

It has not been yet answered definitely in the literature which clinical and/or laboratory findings differentiate the bacterial from the viral meningitis with safety. The aim of the present study was to identify if there are such clinical and /or laboratory variables. The bacterial etiology of meningitis in the present study was determined by using PCR. It was found that headache, vomiting and neck rigidity significantly increase the possibility that the patient has meningitis irrespective of its type (bacterial or viral). However, the lumbar puncture should not be postponed in the absence of these parameters when the clinician’s decision is reasonable as bacterial meningitis is potentially lethal disease and the sensitivity of the abovementioned variables for the identification of meningitis is lower than 100%.Στη βιβλιογραφία δεν έχει απαντηθεί οριστικά το ερώτημα ποιες κλινικές και εργαστηριακές παράμετροι διαφοροδιαγιγνώσκουν με ασφάλεια τη βακτηριακή από την ιογενή μηνιγγίτιδα. Η παρούσα μελέτη σχεδιάστηκε με σκοπό την αναζήτηση αυτών των παραμέτρων σε συνδυασμό με την ταξινόμηση της αιτιολογίας της νόσου με τη χρήση της PCR για βακτηριακούς παράγοντες και για εντεροϊούς. Διαπιστώθηκε ότι η κεφαλαλγία, οι έμετοι και η αυχενική δυσκαμψία αυξάνουν την πιθανότητα τα ευρήματα της οσφυονωτιαίας παρακέντησης να είναι συμβατά με μηνιγγίτιδα, χωρίς όμως με βάση την παρούσα μελέτη να συνιστάται η αναβολή της εύλογης απόφασης του κλινικού ιατρού για τη διενέργεια οσφυονωτιαίας παρακέντησης επί απουσίας τους, διότι η βακτηριακής αιτιολογίας νόσος είναι δυνητικά θανατηφόρος και οι ανωτέρω δείκτες δεν έχουν ευαισθησία 100% για την ανίχνευση της μηνιγγίτιδας
