Investigation of aggravating psychosocial factors on health and predictability of smoking and alcohol use in post adolescent students

Introduction: In modern industrialized countries is intense effect the engagement of young people with socially unacceptable activities, jeopardizing the long-term prosperity. Among the most important modifiable risk behaviors for health are smoking and alcohol use and abuse with early onset at a young age, which continues in young adulthood.Purpose: The purpose is to explore the psychosocial factors contributing to smoking behavior and alcohol use at post adolescent age focusing on provision of adoption and frequency, to identify both prospective users of tobacco and alcohol use and high-risk groups, i.e. those individuals who are driven to abuse, with the ultimate aim of establishing similar programs to prevent future health problems.The aim is to investigate : a) The modifiable factors in detail, which aggravate health, i.e. tobacco and alcohol use. b) Original recording methods of the existing behavior (quantity and frequency) drug use (tobacco and alcohol) in people of post adolescent age, of 18-25 years who are students (research phase a ') . c) Original forecasting methods through concrete models of health, the future use of tobacco and alcohol (research phase a and c). d) Formation of intermediate interventions impact on cognitive, emotional and behavioral part of tobacco use and alcohol use 1.with optical material and 2. focused psychological based on the Theory of Planned Behavior (research phase b and c) . e) Recording of intent and behavior in all groups with or without intervention (research phase c).Method: The main theoretical background of the research part of the thesis is the Theory of Planned Behavior (Ajzen, 1991), the axes of which relate to attitudes, subjective norms and perceived behavioral control, which influence the behavioral intention. The dimension of perceived behavioral control belongs the self efficacy. The intentions thus leading to the adoption of the behavior.After conducting two pilot studies, a qualitative and a quantitative, for research purposes was constructed and weighted research prototype tool - questionnaire for the first research phase, for the use of tobacco and alcohol , according to the blueprints of the aforementioned model health, the axes of which were investigated in detail.Except of the demographic parameters, behavioral attitudes were evaluated too, i.e. attitudes towards tobacco and alcohol use, subjective norms and perceptions of control, perceived behavioral control and self-efficacy. Furthermore the underlying intention was investigated to start or continue smoking or alcohol use in the future and their behavior was investigated. The research tool of the research phase a ' was administered to participants, randomly sampled in two ways : through a) Via Internet (for purposes of the method was created specific form of questionnaire and electronic base for data) and b) printed questionnaires.Subsequently, for the research phase b prototype intermediate interventions were constructed to change behavior in the population under investigation, in two types: optical material and focused mental health intervention based on the pillars of the "Theory of Planned Behavior". The three intermediate groups and subgroups and non-intervention, a groups elaborated by random sampling are as follows:a) Control group, where participants received no intervention.b) Group of optical material, with subgroups b1. Optical material for alcohol and b2. Optical material for smoking and alcohol.c) Group focused psychological intervention of health, with interview face to face via skype, with subgroups c1. focused on alcohol use and c2. focused on the use of tobacco and alcohol.To conduct the research phase c was constructed a new questionnaire on the use of tobacco and alcohol, which was based on questions from the questionnaire made for the first research phase. In this way the behavior and intentions of individuals recorded and studied to use tobacco and alcohol were involved in the three aforementioned groups.Sampling: The sample consists of students generally of post adolescent age , 18-25 years of both sexes, of all Faculties of the University of Peloponnese and the TEI of Kalamata (and annex in Sparta). In a’ research phase involved a total of 791 people. In the research phase b’ , the students were split randomly sampled in three groups : a. control group of 419 students, 262 in group b’ of optical material, c. 110 in the intervention group of focused psychological health interview. In the research phase c involved 184 people of the three aforementioned groups. These individuals were selected by simple random sampling.Results: For the analysis of the statistical results program IBM SPSS Statistics 21 was used. For the statistical analysis techniques of descriptive statistics were used, the multivariate technique of factor analysis, the x2 test of independence and the calculation of the odds ratio, the statistical tests t-test and ANOVA, o calculation of the correlation coefficient and the technique of logistic regression.There were many results detailed in the corresponding chapter. The reliability index for the scales on both alcohol consumption and smoking were quite high (Cronbach a >.85) . Factor analysis showed that the recommendations of the scales yielded loadings on a number of factors , explaining at least 53.1 % of the total variance . It is noteworthy that a distinct tendency for participants interim psychological health intervention to reduce smoking in the next month, as three cigarettes a day. Participants in the group of optical material intended to reduce smoking at about one cigarette per day.It seems that the interventions are most effective in overall alcohol consumption, regardless of beverage type, compared with smoking, although there was a downward trend also in smoking. Between the two types of interim intervention was superior in efficacy of psychological intervention of health, both on smoking and alcohol. Overall, it is distinguished to students a greater tendency to stop drinking despite quitting smoking habit.Conclusions: These results indicate the importance of investigating the modifiable behaviors that affect health. The application of research tools and intervention research would be useful in the context of best practices between primary, secondary and tertiary prevention of health in conjunction with the academic community. The target groups of the student population is both non- users of tobacco and alcohol so as to enhance their behavior and the potential users of tobacco and alcohol but also those who are already users, particularly those who belong to high risk groups.Keywords: Student population, smoking, alcohol use, attitudes, subjective norms, perceived behavioral control, intentions .Εισαγωγή: Tο κάπνισμα, η χρήση και η κατάχρηση αλκοόλ ανήκουν στις πιο σημαντικές τροποποιήσιμες επικίνδυνες συμπεριφορές για την υγεία.Σκοπός της παρούσας διδακτορικής διατριβής ήταν η διερεύνηση των ψυχολογικών και κοινωνικών παραγόντων οι οποίοι επιβαρύνουν την υγεία ατόμων μετεφηβικής ηλικίας, με επικέντρωση στη χρήση καπνού και αλκοόλ από τα ίδια, των τρόπων πρόβλεψης υιοθέτησης της εν λόγω συμπεριφοράς καθώς και της συχνότητάς της, ώστε να εντοπίζονται τόσο οι μελλοντικοί χρήστες καπνού και αλκοόλ όσο και οι ομάδες υψηλού κινδύνου, δηλαδή τα άτομα εκείνα που οδηγούνται σε καταχρήσεις και η υιοθέτηση κατάλληλων παρεμβάσεων πρωτογενούς, δευτερογενούς και τριτογενούς πρόληψης.Υλικό και Μέθοδος: Το δείγμα αποτέλεσαν 791 φοιτητές μετεφηβικής ηλικίας, 18-25 ετών όλων των Σχολών του Πανεπιστημίου Πελοποννήσου και του ΤΕΙ Καλαμάτας. Το κύριο θεωρητικό υπόβαθρο του ερευνητικού μέρους της διατριβής αποτέλεσε η Θεωρία της Προσχεδιασμένης Συμπεριφοράς (Ajzen, 1991). Κατασκευάστηκε και σταθμίστηκε πρωτότυπο ερευνητικό εργαλείο-ερωτηματολόγιο, για τη χρήση καπνού και αλκοόλ (Cronbach α > .85). Αξιολογήθηκαν, εκτός των δημογραφικών παραμέτρων, οι συμπεριφορικές αντιλήψεις, οι υποκειμενικές κανονιστικές αντιλήψεις και οι αντιλήψεις ελέγχου, η πρόθεση των υποκειμένων και η συμπεριφορά τους. Στη συνέχεια διαμορφώθηκαν πρωτότυπες ενδιάμεσες παρεμβάσεις αλλαγής της συμπεριφοράς στον υπό διερεύνηση πληθυσμό, δύο τύπων: οπτικού υλικού και εστιασμένης ψυχολογικής παρέμβασης υγείας. Στην τελική φάση διαμορφώθηκε νέο ερωτηματολόγιο για τη χρήση καπνού και αλκοόλ προκειμένου να γίνει επανέλεγχος σχετικά με την πρόθεση και τη συμπεριφορά των ατόμων, αξιολογώντας τις διαφοροποιήσεις συγκριτικά με την πρώτη ερευνητική φάση και την επίδραση των ενδιάμεσων παρεμβάσεων.Αποτελέσματα: Η στατιστική ανάλυση των δεδομένων έγινε με το πρόγραμμα IBM SPSS Statistics 21. Χρησιμοποιήθηκαν τεχνικές της περιγραφικής στατιστικής, η πολυμεταβλητή τεχνική της παραγοντικής ανάλυσης, ο χ2 έλεγχος ανεξαρτησίας και ο υπολογισμός των odds ratio, οι στατιστικοί έλεγχοι t-test και ANOVA, o υπολογισμός του συντελεστή συσχέτισης και η τεχνική της λογιστικής παλινδρόμησης.12Οι στάσεις είναι ο πρωταρχικός άξονας επίδρασης στη συμπεριφορά καπνίσματος και χρήσης αλκοόλ. Επομένως, σε προγράμματα παρέμβασης ιδιαίτερη εστίαση θα πρέπει να επιδειχθεί, σύμφωνα με τα αποτελέσματα της έρευνάς μας, στην αλλαγή στάσεων. Οι παρεμβάσεις υπήρξαν συνολικά πιο αποτελεσματικές στην κατανάλωση αλκοόλ, ανεξαρτήτως τύπου ποτού, σε σύγκριση με το κάπνισμα, αν και σημειώθηκε μια τάση μείωσης επίσης του καπνίσματος. Ανάμεσα στους δύο τύπους ενδιάμεσης παρέμβασης υπερτέρησε σε αποτελεσματικότητα αυτή της ψυχολογικής παρέμβασης υγείας, τόσο σχετικά με το κάπνισμα όσο και με το αλκοόλ.Συμπεράσματα: Η εφαρμογή των κατασκευασθέντων ερευνητικών εργαλείων και των παρεμβάσεων της έρευνας, καθώς και των νέων εξειδικευμένων παραγόντων που προέκυψαν από τη συγκεκριμένη μελέτη, θα ήταν χρήσιμη στο πλαίσιο βέλτιστων πρακτικών πρωτογενούς, δευτερογενούς και τριτογενούς πρόληψης για την υγεία σε συνδυασμό με την ακαδημαϊκή κοινότητα
