Investigation of the significance of pig salmonellosis from stable to table

Salmonella spp. is one of the most important zoonotic pathogens. Human and animal salmonellosis remain a serious global problem, because of the direct economic costs resulting from morbidity and mortality, and, thus, the considerable losses in animal products. Within the species Salmonella enterica there are over 2,500 different serovars, many of which have great veterinary and medical significance, although they differ widely in their host range, within mammals and birds, and in the nature of disease that may cause. Most important are subclinically infected pigs, which represent the most likely route for Salmonella entering the food chain. These animals shed the microorganism into their environment, maintaining the infection in a farm by infecting pen mates. Thus, these animals should be targeted for successful control of infection, making surveillance of pig salmonellosis mandatory.The aim of this thesis was to study the epidemiology of Salmonella spp. in Greek pig farms, in order to investigate the significance of pork for Public Health.After extensive reviewing of the existing literature, the studying of pig salmonellosis focused at the slaughterhouse in an attempt to evaluate the role of carcass for Greek consumer infections.Initially the infection was investigated serologically. Serum samples examined were from 314 sows and 374 finishing pigs. Thirty nine farms of unknown Salmonella spp. status and five geographic regions of Greece (Thessaly, Central Greece, Ν. Greece, Epirus, Crete) participated in the initial study. For the serological investigation a commercial indirect mix-ELISA kit (Herd-Check Swine Salmonella Antibody Test Kit, Idexx Laboratories, Inc., Maine, USA) was used.The results showed that seropositivity was:• Among sows: 81.53% with a cut off OD≥10% and 20.38% with a cut off OD≥40%.• Among finishing pigs: 70.32% with a cut off OD≥10% and 15.5% with a cut off OD≥40%.• Among farms: from 0-100% with a cut off OD≥10% and 0-63.33% with a cut off OD≥40%.The seroprevalence of the 39 farms was dependent on the chosen cut off value of the ELISA. When the ELISA was compared with isolation, the best correlation was observed with an ELISA OD cut off value of 40%. Specifically, the Odds Ratio for a cut off value of 40% was OR 2.456 (1.004 – 6.007) (95% CI). Thus, an animal was 2.456 times more likely to be Salmonella spp. positive. The important role of sows in the maintenance of Salmonella spp. infection in a farm was statistically confirmed. The findings showed that with a cut off value of OD 40% the prevalence of Salmonella in sows and finishers was 20.38% and 15.5% respectively.After the preliminary seroprevalence study, the isolation of the pathogen and its association with seropositivity was limited in the area of Thessaly, where the rates of seropositive farms ranged between 0-92.6% with a cut off OD≥10% and between 0- 20% with a cut off OD≥40%.For the isolation and identification of the infecting serovars, the samples were examined following ISO 6579: 2002, Annex D for food and animal feeding stuffs (ISO 2002). Samples tested were lymph nodes, gallbladder, intestine, neck muscle and faeces from 123 carcasses of slaughtered pigs, representing 15 farms having approximately 2,500 finishers. Thus, about 5% of these animals were tested. Serum was also examined from each animal as described above. 615 tissue and faecal samples were cultured and 123 serum samples were tested for antibodies against Salmonella spp. Additionally to the pig samples, 378 environmental samples from the surfaces of the slaughterhouses (swab samples) were examined, in order to investigate their role in the contamination of the carcasses.The mean seroprevalence of these animals was overall 59.35% with a cut off OD≥10% and 20.33% with a cut off OD≥40%. The proportion of seropositive animals bacteriologically confirmed was 65.11% with a cut-off OD ≥ 10% and 30.23% with a cut-off OD≥ 40%. As for sensitivity and specificity of the mix-ELISA bacteriologically confirmed it was statistically significant (95% CI) with a cut-off OD≥ 10%. The bacteriological investigation with ISO 6579: 2002 recovered 265 Gram and oxidase negative isolates, belonging, thus, to the family of Enterobacteriaceae. These isolates were biochemically placed to species using the API 20E (Biomerieux, France) and MicrogenTM GnA + B-ID (Microgen Bioproducts Ltd, UK) Systems. Of the 265 isolates 101 were identified as Salmonella spp. with these systems and were tested positive with a polyvalent slide agglutination test detecting O- and H- antigens, were sent to the Greek National Reference Laboratory (NRL -Chalkida) for specific serotyping. Among them were 15 biochemically identified as S. enterica subsp. arizonae and one as S. enterica subsp. indica. Of the remaining 82 isolates (55 from carcasses and 27 from the slaughterhouse environment) were fully serotyped and 4 were untypeable. The serovars identified were the new emerging monophasic variants of S. Typhimurium, S. enterica subsp. enterica ser. 4,5, 12:i:- (16 isolates) και S. enterica subsp. enterica ser. 4,12:i:- (9 isolates), the monophasic S. enterica subsp. enterica ser. 6,7:k:- (7 isolates) and S. enterica subsp. enterica ser. 6,14,25:-:1,2 (one isolate) and the rarer serovars of subspecies enterica subsp. diarizonae 61:k:1,5, S. enterica subsp. salamae 38:b:1,2 and S. enterica subsp. houtenae 40:g,t:- (one isolate each). From the remaining isolates, 38 belonged to serovar Typhimurium and 12 to other serovars (S. Bredeney (3 isolates), S. Agona, S. Derby, S. Infantis, S. Meleagridis, S. Cerro). Although, serovar Typhimurium was the prevalent one (37.6%), the majority of the serovars were those considered emerging or "exotic" in terms of human salmonellosis. In order to investigate antimicrobial resistance with regards to Public Health, the 101 isolates identified as Salmonella spp. with the API 20E, MicrogenTM GnA+B-ID and serotyping were tested against 24 antimicrobial agents with the disk diffusion method. These agents were: amoxicillin (30 μg), amoxicillin/clavulanic acid (20/10μg), ampicillin (10 μg), ampicillin/sulbactam (10/10 μg), aztreonam (30 μg), cefotaxime (30 μg), cefoxitin (30 μg), ceftazidime (30 μg), ceftiofur (30 μg), ceftriaxone (30 μg), cefuroxime (30 μg), chloramphenicol (30 μg), colistin (50 μg), doripenem (10 μg), enrofloxacin (5 μg), erythromycin (15μg), gentamycin (10 μg), kanamycin (30 μg), nalidixic acid (30 μg), penicillin G (10 μg), rifampin (30 μg), sulfamethoxazole/trimethoprim (23.75/1.25 μg), tetracycline (30 μg) and tigecycline (15 μg). The findings showed high resistance to penicillin G (10 mg) (93%), rifampin (30 micrograms) (75.24%), tetracycline (30 ug) (68.31%), sulfamethoxazole / trimethoprim (23.75 / 1.25 g) (61.38%), ampicillin (10 ug) (54.45%), amoxicillin (30 mg) (53.46%) and smaller to chloramphenicol (30 ug) (22.77%), kanamycin (30 g) (14.85%) and colistin (50 mg) (11.88%).Due to the observed high resistance to commonly used antimicrobials, the antimicrobial activities of oregano, rosemary and thyme Essential Oils (EOs) were investigated at three graded undiluted concentrations (5, 15, 30 μl). Fifty nine Salmonella spp. isolates, showing multiple resistance were tested. Strong inhibitory effect was observed with oregano and thyme EOs, even for the amount of 5 μl, although without a statistically significant difference between them. The rosemary essential oil exhibited the least activity in relation to the oregano and thyme EOs. Comparison of the antibacterial effects of these EOs with commonly used antimicrobials (tetracycline,amoxicillin,ampicillin,sulfamethoxazole/trimethoprim) for the same strain revealed that the EOs have a better effect in controlling Salmonella, independent of the level of resistance to antibiotics. The results showed that pigs in Central Greece were infected by at least 14 different serovars. The large number of different serovars and the presence of important newly emerging ones is the first time observed in Greece.Thus, it can be concluded from the results of this investigation, that pork meat is a major source of human salmonellosis, not only for ordinary serovars, but also for newly emerging ones, showing, also, high resistance to commonly used antimicrobials. Additionally, the evidence showed that for the conditions in Thessaly the risk for man comes from the individual animal carrying the pathogen to its tissues and not from carcass cross-contamination during slaughtering. Hence, risk reactions for the region of Thessaly should be focused on reducing individual pig infection.When investigating the above, some difficulties were encountered in the isolation and biochemical characterization of enterobacteria. They need mention, because they affect the final determination of positivity and serovar prevalence. Among these were the presence of lactose-positive salmonellae, the sensitivity problems of the used media and the role of H2S in the recovery of Salmonella spp. from animals. These problems affect negatively the precise determination of carcass contamination rate, thus the size of the risk of pork meat for the consumer. This study showed the need for continuous monitoring of the pathogen’s prevalence in Greek pig farms, using tissue samples and not only faeces, due to the pathogen’s tissue tropism. It also became apparent that reliable systematic study of pig salmonellosis depends on staff well trained in bacteriology, thus, capable of identifying atypical strains escaping the internationally proposed methods.Το γένος Salmonella περιλαμβάνει παθογόνα βακτηρίδια με παγκόσμια σημασία στη Δημόσια Υγεία και την οικονομία, λόγω του άμεσου οικονομικού κόστους που προκύπτει από τα ποσοστά θνησιμότητας και θνητότητας, καθώς και των ανυπολόγιστων απωλειών σε ζωοκομικά προϊόντα. Στο είδος Salmonella enterica ανήκουν πάνω από 2.500 ορότυποι με σημασία στη Δημόσια Υγεία και τη ζωική παραγωγή. Οι ορότυποι αυτοί διαφέρουν σημαντικά ως προς το εύρος των ξενιστών τους, καθώς και ως προς τη φύση της νόσου που είναι ικανοί να προκαλέσουν. Ο χοίρος και τα προϊόντα του αποτελούν σημαντική δεξαμενή διασποράς του παθογόνου στην τροφική αλυσίδα, λόγω του ότι τα είδη του γένους Salmonella είναι ευρέως διαδεδομένα στα συστήματα παραγωγής χοίρων παγκοσμίως. Οι υποκλινικές λοιμώξεις του χοίρου με Salmonella spp. έχουν ζωονοτικό χαρακτήρα με μεγάλη σημασία στην ασφάλεια των τροφίμων, συνεπώς και στη χοιροτροφία, καθιστώντας την επιτήρηση των σαλμονελλώσεων απαραίτητη. Σκοπός της παρούσας διατριβής ήταν η μελέτη της επιδημιολογίας των Salmonella spp. σε ελληνικές εκτροφές χοίρου, ώστε να διερευνηθεί η σημαντικότητα του χοιρινού κρέατος για τη Δημόσια Υγεία.Μετά από εκτενή μελέτη της κείμενης διεθνούς βιβλιογραφίας, αποφασίστηκε η διαδικασία της ερευνητικής μελέτης της σαλμονέλλωσης του χοίρου σε επίπεδο σφαγείου, ώστε να καθοριστεί η σημασία της μόλυνσης στη Δημόσια Υγεία.Αρχικά η μόλυνση διερευνήθηκε ορολογικά με δείγματα αίματος 314 συών και 374 χοίρων τελικής πάχυνσης από 39 εκτροφές, αγνώστου επιπέδου μόλυνσης με Salmonella spp., σε πέντε γεωγραφικές περιοχές της Ελλάδας (Θεσσαλία, Στερεά Ελλάδα, Β. Ελλάδα, Ήπειρος, Κρήτη). Η ορολογική διερεύνηση έγινε με τη χρήση εμπορικά διαθέσιμης έμμεσης mix-ΕLISA (Herd-Check Swine Salmonella Antibody Test Kit, Idexx Laboratories, Inc., Maine, USA).Τα αποτελέσματα έδειξαν ότι η οροθετικότητα ήταν:•Μεταξύ των χοιρομητέρων 81,53% με cut off OD≥10% και 20,38% με cut off OD≥40%•Μεταξύ των χοίρων τελικής πάχυνσης 70,32% με cut off OD≥10% και 15,5% με cut off OD≥40% •Μεταξύ των εκτροφών από 0-100% με cut off OD≥10% και από 0-63,33% με cut off OD≥40%.Η οροθετικότητα των 39 εκτροφών ήταν εξαρτημένη από την επιλεγείσα τιμή cut off της ELISA. Αυτή η διερεύνηση έδειξε ότι η καλύτερη συσχέτιση μεταξύ ορολογικών μεθόδων και απομονώσεων παρατηρήθηκε για τιμή OD cut off 40%. Συγκεκριμένα το Odds Ratio για την τιμη του cut off 40% ήταν OR 2,456 (1,004 – 6,007) (95% CI), δηλαδή ένα ζώο είχε 2,456 φορές πιθανότητες να βρεθεί Salmonella spp. θετικό. Επιπλέον, η στατιστική ανάλυση επιβεβαίωσε τον σημαντικό επιδημιολογικό ρόλο των χοιρομήτερων στη διατήρηση και διασπορά της μόλυνσης με Salmonella, δηλαδή τον ρόλο τους στη μετάδοση του παθογόνου σε χοίρους τελικής πάχυνσης. Με βάση αυτά τα δεδομένα για cut off της OD 40% η συχνότητα εμφάνισης (επιπολασμός) της Salmonella στις χοιρομητέρες σε σχέση με τους χοίρους τελικής πάχυνσης ήταν 20,38% και 15,5% αντίστοιχα.Με δεδομένα τα ευρήματα της προκαταρκτικής οροδιερεύνησης ο σχεδιασμός της απομόνωσης του παθογόνου και της συσχέτισής του με τα ποσοστά οροθετικότητας περιορίστηκε στη Θεσσαλία, όπου τα ποσοστά των οροθετικών εκτροφών ήταν μεταξύ 0-92,6% με cut off OD≥10% και μεταξύ 0-20% (cut off OD≥40%).Για την απομόνωση και ταυτοποίηση των οροτύπων τα επιλεγμένα δείγματα εξετάστηκαν με βάση το ISO 6579:2002, Annex D για τρόφιμα και ζωοτροφές (ISO 2002). Τα δείγματα ήταν αίμα, ιστοί και κοπράνα από 123 σφάγια χοίρων τελικής πάχυνσης, που προήλθαν από 15 εκτροφές με 2.500 περίπου ζώα τελικής πάχυνσης. Δηλαδή ελέγχθηκε ορολογικά και μικροβιολογικά περίπου το 5% αυτών των ζώων. Για τον μικροβιολογικό έλεγχο των εκτροφών διερευνήθηκαν 615 δείγματα ιστών και κοπράνων. Η οροθετικότητα των εξετασθέντων ζώων συνολικά ήταν 59,35% με cut off OD≥10% και 20,33% με cut off OD≥40%, ενώ το ποσοστό των οροθετικών ζώων με βακτηριολογική επιβεβαίωση ήταν 65,11% με cut-off OD≥ 10% και 30,23% με cut-off OD≥ 40%. Διαφαίνεται ότι η ευαισθησία και η ειδικότητα της mix-ELISA είναι στατιστικά σημαντική (95% CI) με cut-off OD≥ 10%.Επιπλέον των δειγμάτων από σφάγια χοίρων, εξετάστηκαν μικροβιολογικά 378 δείγματα επιφανειών (swab samples) του σφαγείου, ώστε να διερευνηθεί ο ρόλος τους στην επιμόλυνση των σφάγιων.Από τη μικροβιολογική διερεύνηση με το ISO 6579:2002 απομονώθηκαν 265 στελέχη. Τα στελέχη αυτά ήταν Gram και οξειδάση αρνητικά, άρα ανήκαν στην οικογένεια των Εντεροβακτηριοειδών και ταυτοποιήθηκαν βιοχημικά με το API 20E (Biomerieux, France) και το MicrogenTM GnA+B-ID (Microgen Bioproducts Ltd, UK). Χαρακτηρίστηκαν ως Salmonella spp. 101 στελέχη, τα οποία ελέγχθηκαν με πολυδύναμους αντι-Ο, αντι-H και αντι-Vi ορούς και στάλθηκαν στο Εθνικό Εργαστήριο Αναφοράς Σαλμονελλών (Χαλκίδα) για πλήρη οροταυτοποίηση. Τελικά οροταυτοποιήθηκαν πλήρως 82 στελέχη (55 προερχόμενοι από ζώα και 27 από τις επιφάνειες του σφαγείου), 4 χαρακτηρίστηκαν ως ατυποποίητα, 14 αναγνωρίστηκαν βιοχημικά ως S. enterica subsp. arizonae και ένα ως S. enterica subsp. indica. Μεταξύ των αναγνωρισμένων οροτύπων ήταν οι νεοαναδυόμενοι μοναφασκοί τύποι του οροτύπου Typhimurium, S. enterica subsp. enterica ser. 4,5, 12:i:- (16 στελέχη) και S. enterica subsp. enterica ser. 4,12:i:- (9 στελέχη), οι μονοφασικοί S. enterica subsp. enterica ser. 6,7:k:- (7 στελέχη) και S. enterica subsp. enterica ser. 6,14,25:-:1,2 (ένα στέλεχος) και οι πιο σπάνιοι ορότυποι των υποειδών enterica subsp. diarizonae 61:k:1,5, S. enterica subsp. salamae 38:b:1,2 και S. enterica subsp. houtenae 40:g,t:- (ένα στέλεχος ο καθένας). Από τα λοιπά στελέχη, 38 ανήκαν στον ορότυπο Typhimurium και 12 σε άλλους οροτύπους (S. Bredeney (3 στελέχη), S. Agona, S. Derby, S. Infantis, S. Meleagridis, S. Cerro). Παρόλο που ο ορότυπος Typhimurium ήταν ο επικρατέστερος (38), η πλειοψηφία του συνόλου των λοιπών οροτύπων ανήκαν σε εκείνους που θεωρούνται αναδυόμενοι ή «εξωτικοί» όσον αφορά τη σαλμονέλλωση του ανθρώπου. Για τη διερεύνηση της σημασίας της αντιβιοαντοχής στη Δημόσια Υγεία, τα 101 στελέχη αναγνωρισμένα με τα API 20E, MicrogenTM GnA+B-ID και την οροταυτοποίηση ως Salmonella spp. διερευνήθηκαν με τη μέθοδο διάχυσης των δίσκων σε 24 αντιμικροβιακές ουσίες. Οι ουσίες ήταν: amoxicillin (30 μg), amoxicillin/clavulanic acid (20/10μg), ampicillin (10 μg), ampicillin/sulbactam (10/10 μg), aztreonam (30 μg), cefotaxime (30 μg), cefoxitin (30 μg), ceftazidime (30 μg), ceftiofur (30 μg), ceftriaxone (30 μg), cefuroxime (30 μg), chloramphenicol (30 μg), colistin (50 μg), doripenem (10 μg), enrofloxacin (5 μg), erythromycin (15μg), gentamycin (10 μg), kanamycin (30 μg), nalidixic acid (30 μg), penicillin G (10 μg), rifampin (30 μg), sulfamethoxazole/trimethoprim (23.75/1.25 μg), tetracycline (30 μg) και tigecycline (15 μg). Τα ευρήματα έδειξαν υψηλά ποσοστά ανθεκτικότητας σε: penicillin G (10 μg) (93%), rifampin (30 μg) (75,24%), tetracycline (30 μg) (68,31%), sulfamethoxazole/trimethoprim (23.75/1.25 μg) (61,38%), ampicillin (10 μg) (54,45%), amoxicillin (30 μg) (53,46%) και μικρότερα σε chloramphenicol (30 μg) (22,77%), kanamycin (30 μg) (14,85%) και colistin (50 μg) (11,88%).Λόγω της παρατηρούμενης υψηλής ανθεκτικότητας και της επιβολής περιορισμών στη χρήση των αντιμικροβιακών ουσιών, διερευνήθηκε η ευαισθησία 59 στελεχών με πολλαπλή ανθεκτικότητα εκ των οροταυτοποιηθέντων Salmonella spp. που απομονώθηκαν από σφάγια χοίρων, σε τρεις αναραίωτες ποσότητες (5, 15 και 30 μl) αιθέριων ελαίων από ρίγανη, δενδρολίβανο και θυμάρι. Τόσο το ριγανέλαιο όσο και το θυμαρέλαιο είχαν ισχυρή ανασταλτική επίδραση, ακόμη και για την ποσότητα των 5 μl, χωρίς στατιστικά σημαντική διαφορά μεταξύ τους, ενώ το έλαιο δενδρολίβανου δεν είχε καμία ή παρουσίασε μικρότερη αντιμικροβιακή δράση σε σχέση με το ριγανέλαιο και το θυμαρέλαιο. Η σύγκριση των αντιμικροβιακών ιδιοτήτων αυτών των αιθέριων ελαίων με τα ευρέως χρησιμοποιούμενα αντιβιοτικά για το ίδιο στέλεχος (tetracycline, amoxicillin, ampicillin, sulfamethoxazole/trimethoprim) αποκάλυψε ότι τα αιθέρια έλαια έχουν καλύτερη αντιμικροβιακή δράση από τα χρησιμοποιηθέντα αντιβιοτικά. Τα αποτελέσματα έδειξαν ότι στην Κεντρική Ελλάδα οι χοίροι μολύνονται από τουλάχιστον 14 διαφορετικούς οροτύπους. Ο μεγάλος αριθμός διαφορετικών οροτύπων και η παρουσία σημαντικών νεοαναδυόμενων παγκοσμίως είναι η πρώτη φορά που παρατηρείται στην Ελλάδα.Συμπεραίνεται από τα αποτελέσματα αυτής της διερεύνησης ότι το κρέας του χοίρου είναι μια σημαντική πηγή σαλμονέλλωσης του ανθρώπου, όχι μόνο από συνηθισμένους οροτύπους, αλλά και από νεοαναδυόμενους, οι οποίοι εμφανίζουν υψηλή ανθεκτικότητα σε ευρέως χρησιμοποιούμενες αντιμικροβιακές ουσίες. Επιπλέον, η μελέτη έδειξε ότι ο κίνδυνος για τον άνθρωπο προέρχεται από το συγκεκριμένο ζώο που φέρει το παθογόνο στους ιστούς του και όχι λόγω διασταυρούμενης επιμόλυνσης του σφάγιου που μπορεί να συμβεί κατά τη σφαγή. Κατά τη μελέτη των ανωτέρω προέκυψαν δυσκολίες στην απομόνωση και τον βιοχημικό χαρακτηρισμό των Εντεροβακτηρίων, που χρήζουν αναφοράς, αφού επηρεάζουν τα ποσοστά θετικότητας και τον τελικό προσδιορισμό των οροτύπων που μολύνουν τον χοίρο. Μεταξύ αυτών ήταν η παρουσία λακτόζη θετικών salmonellae, τα προβλήματα ευαισθησίας των χρησιμοποιηθέντων υποστρωμάτων, καθώς και ο ρόλος του H2S στην απομόνωση Salmonella spp. από τα ζώα. Αυτά τα προβλήματα επηρεάζουν αρνητικά τον ακριβή καθορισμό του ποσοστού μόλυνσης των σφάγιων, άρα και τον καθορισμό του μεγέθους της επικινδυνότητας του κρέατος χοίρου για τον καταναλωτή.Η παρούσα μελέτη υποδεικνύει την ανάγκη συνεχούς επιτήρησης της συχνότητας εμφάνισης του παθογόνου στις ελληνικές χοιροτροφικές μονάδες, καθώς και της προσπάθειας απομόνωσής του με δειγματοληψίες κυρίως ιστών και κοπράνων, αφού το παθογόνο δείχνει τροπισμό και εγκατάσταση στον λεμφικό ιστό κυρίως. Επιπλέον, διαφάνηκε ότι η συστηματική αξιόπιστη μελέτη των σαλμονελλώσεων απαιτεί άριστα εκπαιδευμένο στη βακτηριολογία επιστημονικό προσωπικό, ώστε να εντοπίζονται και τα άτυπα στελέχη, που δεν εμπίπτουν στους κανόνες των προτεινόμενων διεθνών μεθόδων
