Study o0f hormonal sexretion, metabolism and distribution of body fat in patients with obesity or type 2 diabetes mellitus - association with treatment that sensitizes insulin action

AIM OF THE STUDY The study of a) insulin secretion and action, b) accumulation, distribution and hormonal function of adipose tissue, and, c) markers of vascular endothelial dysfunction in men with Type 2 Diabetes (T2DM), and also, d) the impact of metformin on the above parameters.MATERIALS AND METHODS Thirty-three men with T2DM and 28 healthy men (mean age 60.45±7.56 and 57.39±7.67 years respectively, and mean BMI 31.67±4.37kg/m2 and 31.09 4.46 kg/m2 respectively) underwent an Oral Glucose Tolerance Test (OGTT) and an oral fat load, and were studied for 5 hours postprandially for: a) basal and postprandial levels of glucose, insulin, adiponectin, free fatty acids (FFAs), sICAM-1 and sVCAM-1, b) insulin resistance and sensitivity, by the use of HOMA and Matsuda index respectively, c) body fat accumulation and distribution by manual anthropometrics and by a set of axial MRI images of abdomen and hip.RESULTS The two groups did not differ significantly regarding manual and MRI indexes of fat distribution, except for higher total fat estimated by BIA in diabetics (p<0.035). Fasting adiponectin was lower, and fasting and postprandial FFAs were higher in diabetics (p=0.022, p=0.007 and p=0.077 respectively). Fasting and postprandial sICAM-1 and sVCAM-1 were higher in diabetics. Fasting and postprandial insulin levels were correlated negatively to fasting and postprandial adiponectin levels in both groups. In controls, postprandial glucose and insulin levels were correlated positively to postprandial FFAs. In diabetics, fasting and postprandial insulin was correlated positively to postprandial FFAs (r=0.415, p<0.05 and r=0.512, p<0.001 respectively). In the control group, insulin levels, fasting and postprandial, were positively correlated to fasting and postprandial sICAM-1 levels respectively (r=0.394 and r=0.446, both p<0.05). HOMA was positively correlated to MRI indexes of abdominal subcutaneous fat in the group of controls, visceral fat in the group of diabetics and subcutaneous hip fat in both groups. Matsuda was negatively correlated to MRI indexes of subcutaneous abdominal fat in the control group. The strongest correlations were observed at measurement site L3-L4 for both groups. Adiponectin levels, fasting and postprandial, were negatively correlated to MRI indexes of visceral, subcutaneous and total abdominal fat in the group of controls, the strongest one observed at measurement site L4-L5(r=-0.547, p<0.001). Postprandial FFAs levels were positively correlated to MRI indexes of visceral, particularly retroperitoneal fat, the strongest correlation observed at measurement sit L2-L3 (r=0.555, p<0.001), in the group of diabetics. Diabetics on metformin had higher MRI indexes of subcutaneous and total abdominal and subcutaneous hip fat, lower fasting FFAs levels (p=0.013) and lower postprandial sICAM-1 levels compared with drug-naïve patients.CONCLUSIONS 1.Insulin resistance and insulin sensitivity indexes are related to different fat compartments in diabetic and non-diabetic individuals with comparable indexes of body fat accumulation and distribution. HOMA is correlated positively with abdominal subcutaneous and hip subcutaneous fat, in the group of controls, whilst, in T2DM patients, it is correlated to the abdominal visceral and hip subcutaneous fat. The strongest associations with HOMA index were observed at measurement site L3-L4 in both groups.2. Adiponectin levels are negatively correlated with total, visceral and subcutaneous fat in the control group. FFAs levels are positively correlated to visceral, especially retroperitoneal fat, in T2DM. 3. Insulin levels are negatively correlated to fasting and postprandial adiponectin levels, and positively to postprandial FFAs levels in both groups.ΣΚΟΠΟΣ Η μελέτη α) της ινσουλινικής έκκρισης και δράσης, β) συσσώρευσης, κατανομής και της ορμονικής λειτουργίας του λίπους και γ) δεικτών ενεργοποίησης του αγγειακού ενδοθηλίου σε άτομα με ΣΔ2, και επίσης δ) των επιπτώσεων της μετφορμίνης στις παραπάνω παραμέτρους. ΥΛΙΚΟ ΚΑΙ ΜΕΘΟΔΟΙ 33 ανεπίπλεκτοι άρρενες ασθενείς με ΣΔ2 (μέση ηλικία 60.45± 7.56 έτη, μέσο BMI 31.67±4.37kg/m2) και 28 υγιή άτομα αντίστοιχης ηλικίας (57.39 7.67 έτη), φύλου και BMI (31.09 4.46 kg/m2) υποβλήθηκαν σε δοκιμασία OGTT και σε χορήγηση λιπαρού γεύματος για τη μελέτη: α) βασικών και μεταγευματικών επιπέδων γλυκόζης, ινσουλίνης, αδιπονεκτίνης, Ελεύθερων Λιπαρών Οξέων (ΕΛΟ) και δεικτών ενεργοποίησης αγγειακού ενδοθηλίου (s-ICAM-1, s-VCAM-1), β) ινσουλινικής αντοχής και ευαισθησίας, χρησιμοποιώντας τους δείκτες HOMA και Matsuda αντίστοιχα, γ) συσσώρευσης και κατανομής λίπους με συμβατικά μέσα και MRI κοιλίας και γλουτών.ΑΠΟΤΕΛΕΣΜΑΤΑ Δεν υπήρξε σημαντική διαφορά ως προς τους ανθρωπομετρικούς και MRI δείκτες συσσώρευσης και κατανομής λίπους μεταξύ των ομάδων, εκτός από το υψηλότερο ποσό λίπους μέσω της ΒΙΑ στους διαβητικούς (p<0.035). Τα επίπεδα αδιπονεκτίνης νηστείας βρέθηκαν χαμηλότερα και τα επίπεδα ΕΛΟ νηστείας υψηλότερα στα άτομα με ΣΔ2 (p<0.022 και p<0.007 αντίστοιχα). Τα επίπεδα sICAM-1 και sVCAM-1 ορού, νηστείας και μεταγευματικά, βρέθηκαν υψηλότερα στους διαβητικούς. Τα επίπεδα ινσουλίνης, νηστείας και μεταγευματικά, συσχετίστηκαν αρνητικά με τα αντίστοιχα αδιπονεκτίνης στις δύο ομάδες. Στους μάρτυρες, τα μεταγευματικά επίπεδα γλυκόζης και ινσουλίνης συσχετίστηκαν θετικά με τα αντίστοιχα των ΕΛΟ. Στους διαβητικούς, τα επίπεδα ινσουλίνης, νηστείας και μεταγευματικά, συσχετίστηκαν θετικά με τα μεταγευματικά επίπεδα ΕΛΟ (r=0.415, p<0.05 και r=0.512, p<0.001 αντίστοιχα). Τα επίπεδα ινσουλίνης, νηστείας και μεταγευματικά, σχετίστηκαν θετικά με τα αντίστοιχα sICAM-1 στους μάρτυρες (r=0.394, p<0.05). Ο ΗΟΜΑ σχετίστηκε θετικά με MRI δείκτες υποδόριου κοιλιακού λίπους στους μάρτυρες, σπλαχνικού λίπους στους διαβητικούς, και υποδόριου ισχίων στις δύο ομάδες. Αντίστροφα, ο δείκτης Matsuda συσχετίστηκε αρνητικά με MRI δείκτες υποδόριου κοιλιακού λίπους στους μάρτυρες. Οι ισχυρότερες συσχετίσεις παρουσιάζονται στο επίπεδο Ο3-Ο4 για τις δύο ομάδες.
