People with severe mental illness or physical disabilities as victims of delinquent acts

People with disabilities are often confronted with the social’s exclusion problems,social’s neglect and unequal treatment. In addition, there are indications, that people withserious mental illness or physical disabilities are also victims of violent acts or delinquentbehavior.The issue of people’s with disabilities (severe mental disorders or physicaldisabilities) , has not been studied sufficiently, despite its medical and sociologicalimportance. To the contrary, the issue of psychiatric patients’ dangerousness has beenstudied thoroughly.The range of the phenomenon has not been registered in most countries, such as in Greece.Moreover, the risk factors, the typologies of delinquent acts against people with disabilitiesand patients as well as the ways of managing victims and preventing victimization are notsufficiently known.The present study aims at contributing to the research regarding victimizationbetween seriously mentally ill outpatients and people with physical disabilities matched tothem healthy individuals. In addition, the study aims at recording demographic risk factorsassociated with delinquency of people with disabilities. The present study aims at confirmingthe hypothesis that people with disabilities are more often ,victims of delinquent acts anddiscrimination than the general population.METHOD and SAMPLE: The sample comes from the catchment area of Thrace (witha population of about 250.000) and consists of 330 individuals (150 people with severemental illness, 30 people with physical disabilities and 150 healthy controls). All patientswere aged between 18-65 .The patients participated in the study were followed up by thesame psychiatric service. They had all been hospitalized in the past in the psychiatricdepartment of the area’s hospital (the University General Hospital of Alexandroupolis) and had received a diagnosis of schizophrenia, or bipolar disorder according to DSM-IV-TR. All ofthem were taking medication and were not in a state of relapse. A consultation with thepatients’ attendant doctor took place before the selection of the sample, in order tominimize the risk that acts of victimization mentioned by the patients,could be a product ofdelusions. The patients that were in a state of active ideation of the above mentioned kind(i.e. believed that were systematically poisoned or persecuted) were excluded from thesample.During the same chronological period a sample of 30 people with physicaldisabilities, who were members of the associations of people with disabilities in Komotiniand Xanthi, participated in the survey. During the same period, matched sample of controlswas formed.An ad hoc semi-structured interview for registering victimization incidents wascreated for this study. This interview was based on the following two interviews: a) theNational Crime Victimization Survey (NCVS), and b) the International Crime VictimizationSurvey (ICVS) that has been used in international research concerning the victimization ofgeneral population.There were two sets of statistical analyzes. The first set concerned the sample of300 people (150 people with severe mental illness and 150 controls) and the second setconcerned the sample of 127 people (30 people with physical disabilities, 51 people withsevere mental illness, and 46 controls).Descriptive analysis of the data was conducted withthe use of mean values ± standard deviations for the continuous variables and percentagesfor the categorical ones. The comparison of continuous variables among patients, peoplewith physical disabilities and non-patients was performed by using t-test. The comparison ofcategorical variables was conducted by using chi-square test or Fischer’s exact test for lowvalues. Logistic regression was applied in order to examine the influence of demographicparameters on overall victimization, assault/threat victimization and victimization as a result of discrimination (dependent variables).In the second sample financial exploitation, sexualassault and theft were used as dependents variables. Health status was included in allmodels as covariate. Age, sex, marital status, cultural identity, work and education wereconsidered as independent variables. Results with p values < 0.5 were consideredstatistically significant. The statistical package used were STATA 9.0 in the first sample andIBMSPSSv.20.0 in the second one .RESULTS: In the first sample, people with severe mental illness in comparison tocontrols, were more often victims of a discriminatory behavior (52% vs 24% - x2=24.958,p<.001) and of an act of overall delinquency as well (59.3% vs 46% - x2= 5.349, p= .02). Morespecifically, victimization rate is higher among patients in the cases of theft of personal items(20.7% vs 8 % - x2=9.800, p=.002) and assault/threat (28.7 % vs 8 % - x2 =21.395, p<.001)). Nosignificant differences were found in the rest of the offences between the two groups .The results of the logistic regression ,in univariate analysis, being a patient increasesthe possibility of being a victim of any delinquent act (OR=1.71, 95 % CI={1.08, 2.71}). Invarious models of multivariate analysis, the above influence of health state remains stable.Living in a village (OR=.56, 95 % CI={.33, .95}), and low educational level (OR=.44, 95%CI={.21, .96}) are protective factors.The results of logistic regression with dependent variables these of victimizationdue to assault/threat on the one hand and discriminative behavior on the other showed thatpatients are 4.6 times more in danger of being victim of the former crime (OR=4.62, 95%CI={2.32, 9.19)}) and 3.3 times (OR=3.34, 95% CI={2.1, 5.62}) in greater danger of beingvictims of the latter.In the second sample, the participants with a physical disability have been at leastonce victims of a delinquent act against them in 50% of the cases and of an act ofdiscrimination in 57% of the cases. Particularly people with physical disabilities are moreoften victims of theft or injury of a vehicle (13%), assault/threat (6,7 %), theft of personal items (6,7%). In the case of sexual assault/ threat the women participants’ distress is (13%).Patients, in comparison to people with physical disabilities , were more often victimsin the cases of theft of personal items(p=0.005), of an assault/threat (p<0.001), and of atheft of a vehicle (p<0,001).No significant differences were found , between the people withphysical disabilities and controls.The results of logistic regression showed that the patients are 8 times more indanger of being victim in overall victimization, than the people with physical disabilities.Especially the patients are 100 times more in danger of being victim in assault/threat, thanpeople with physical disabilities (OR=106.305, 95% CI={6.185- 1828.569)}. The people withphysical disabilities are 26 times more in danger of being victims in discrimination , thancontrols (OR=26.878, 95% CI={2.381- 303.434)}.CONCLUSION : This study compares victimization amongst patients suffering fromsevere mental illness, people with physical disabilities and matched controls. Results confirmthe hypothesis that the existence of a serious mental illness is an independent risk factor forvictimization and causes intense psychological distress to the victims. Results also confirmthe hypothesis that the existence of a disability is a risk factor of discrimination and hatecrimes. Findings show the large contribution of the patients’ family environment to theirvictimization. Consequently, the protection of patients from being victims of delinquent actsand especially preventive interventions to the patients’ families should be part of thepsychiatric/mental health services’ provision. It would also be useful if the society wassensitized about the behaviours of discrimination against individuals with disabilities, andsome specific programs were developed in order to protect victims of delinquent acts anddiscriminatory behavior.Τα άτομα με αναπηρία, είναι μία σημαντική κατηγορία του πληθυσμού, πουαντιμετωπίζει συχνά προβλήματα κοινωνικού αποκλεισμού, άνισης μεταχείρισης ήαδιαφορίας από το κοινωνικό περιβάλλον.Εκτός από τις συμπεριφορές διάκρισης εις βάρος τους, υπάρχουν ενδείξεις ότι ταάτομα με σοβαρή ψυχική διαταραχή ή με σωματική αναπηρία είναι επίσης θύματαπαραβατικών πράξεων ή βίαιων συμπεριφορών.Το ζήτημα της θυματοποίησης των ατόμων με αναπηρία (με σοβαρές ψυχικέςδιαταραχές ή σωματικές αναπηρίες), παρά τη σημασία του (ιατρική, κοινωνιολογική), δενέχει μελετηθεί ή συζητηθεί επαρκώς, ενώ έχει διερευνηθεί εκτενώς το ζήτημα τηςεπικινδυνότητας των ασθενών με σοβαρές ψυχικές διαταραχές.Στις περισσότερες χώρες, όπως και στην Ελλάδα, δεν έχει καταγραφεί η έκταση τουφαινομένου, ούτε είναι επαρκώς γνωστοί οι παράγοντες κινδύνου, οι τρόποι διαχείρισηςκαι πρόληψης της θυματοποίησης ούτε οι συνέπειές της στα άτομα με αναπηρίες.Η παρούσα έρευνα αποσκοπεί στο να συμβάλει στη διερεύνηση τηςθυματοποίησης των ατόμων με σοβαρές ψυχικές διαταραχές, των ατόμων με σωματικέςαναπηρίες και υγιών μαρτύρων με παρόμοια δημογραφικά χαρακτηριστικά προς αυτούς.που ζουν στην κοινότητα στη περιοχή της Θράκης. Επιπλέον, έχει σκοπό να καταγράψειδημογραφικούς παράγοντες κινδύνου που μπορεί να σχετίζονται με την παραβατικότητατων ατόμων με αναπηρίες.Η υπόθεση εργασίας αφορά στο ότι τα άτομα με σοβαρή ψυχική διαταραχή ήσωματική αναπηρία, είναι θύματα παραβατικών πράξεων ή συμπεριφορών διάκρισηςσυχνότερα από το γενικό πληθυσμό.Υλικό και μέθοδος: Το δείγμα αυτής της έρευνας προέρχεται από την περιοχή τηςΘράκης (με πληθυσμό περίπου 250.000 άτομα) και αποτελείται από 330 άτομα (150 άτομα με σοβαρή ψυχική διαταραχή, 30 άτομα με σωματική αναπηρία και 150 υγιείςμάρτυρες).Όλοι οι συμμετέχοντες ήταν ηλικίας 18-65 ετών.Οι ψυχικά ασθενείς που συμμετείχαν στην έρευνα παρακολουθούνταν από την ίδιαψυχιατρική υπηρεσία τουλάχιστον επί δυο χρόνια Όσοι ασθενείς παρουσίαζαν ενεργέςπαραληρητικές ιδέες (π.χ διωκτικού τύπου)αποκλείονταν από την έρευνα.Κατά την ίδιαχρονική περίοδο στην έρευνα συμμετείχαν 30 άτομα με σωματική κινητική αναπηρία,καθώς και ένα δείγμα ελέγχου υγιών μαρτύρων με παρόμοια δημογραφικάχαρακτηριστικά.Για τους σκοπούς αυτής της έρευνας δημιουργήθηκε ένα ad hoc ημιδομημένοερωτηματολόγιο καταγραφής συμβάντων θυματοποίησης, βασισμένο στις δύο ακόλουθεςσυνεντεύξεις: α) την NationalCrimeVictimizationSurvey, και β) την International CrimeVictimization Survey που έχουν χρησιμοποιηθεί σε διεθνείς έρευνες θυματοποίησηςγενικού πληθυσμού.Έγιναν δύο σειρές στατιστικών αναλύσεων. Η πρώτη αφορούσε σε δείγμα 300ατόμων (150 ψυχικά ασθενείς και 150 υγιείς) και η δεύτερη αφορούσε σε δείγμα 127ατόμων (30 άτομα με σωματική αναπηρία, 51 ψυχικά ασθενείς και 46 υγιείς). Έγινεπεριγραφική ανάλυση των δεδομένων με χρήση μέσων τιμών ± τυπικών αποκλίσεων για τιςσυνεχείς μεταβλητές και ποσοστών για τις κατηγορικές. Κατά την πρώτη στατιστικήανάλυση οι ψυχικά ασθενείς συγκρίθηκαν με τους υγιείς ως προς διάφορες παραμέτρουςθυματοποίησης ενώ κατά τη δεύτερη στατιστική ανάλυση τα άτομα με σωματική αναπηρίασυγκρίθηκαν ως προς τη θυματοποίησή τους τόσο με τους ψυχικά ασθενείς όσο και με τουςυγιείς. Οι συγκρίσεις συνεχών μεταβλητών μεταξύ ψυχικά ασθενών, ατόμων με σωματικήαναπηρία και υγιών ατόμων έγινε με τη χρήση του παραμετρικού t-test ή του μηπαραμετρικού Mann-Whitney U test για μη κανονικά κατανεμημένες μεταβλητές, ενώ γιατη σύγκριση κατηγορικών μεταβλητών χρησιμοποιήθηκε το χ2 τεστ ή το Fisher’s exact τεστγια μικρά νούμερα. Για τη μελέτη της επίδρασης των δημογραφικών παραμέτρων στη θυματοποίησητου δείγματος εφαρμόσθηκε λογιστική παλινδρόμηση. Ως εξαρτημένες μεταβλητέςχρησιμοποιήθηκαν: η συνολική θυματοποίηση, η θυματοποίηση από συμπεριφοράδιάκρισης και η θυματοποίηση από επίθεση/απειλή στο πρώτο δείγμα, ενώ στο δεύτεροδείγμα χρησιμοποιήθηκαν επιπλέον, η οικονομική εξαπάτηση, η σεξουαλική κακοποίηση,και η κλοπή προσωπικών ειδών. Ως ανεξάρτητες μεταβλητές χρησιμοποιήθηκαν ηκατάσταση της υγείας (ασθενής-υγιής), το φύλο, η ηλικία, ο τόπος κατοικίας, το επάγγελμα,η εκπαίδευση, η οικογενειακή κατάσταση και η πολιτισμική ταυτότητα. Τα αποτελέσματαθεωρήθηκαν στατιστικώς σημαντικά στο 5% επίπεδο στατιστικής σημαντικότητας.Αποτελέσματα: Στο πρώτο δείγμα, τα άτομα με ψυχική διαταραχή είναι συχνότεραθύματα από τους υγιείς τόσο όσον αφορά τις παραβατικές πράξεις γενικά (59.3 % vs 46%χ2=5.349, p=0.02) αλλά και όσον αφορά τις πράξεις διάκρισης (52% vs 24% χ2=24.958,p<0.001 ). Ειδικότερα, το ποσοστό θυματοποίησης, είναι υψηλότερο μεταξύ των ασθενώνόσον αφορά στην κλοπή προσωπικών ειδών (20.7 % vs 8% χ2=9.800 p=0.002) και στηνεπίθεση/ απειλή (28.7 % vs 8% , χ2=21.395 p<0.001). Όσον αφορά τα υπόλοιπα αδικήματαδεν υπάρχουν σημαντικές διαφορές μεταξύ ασθενών και υγιών.Σύμφωνα με τα αποτελέσματα της λογιστικής παλινδρόμησης το να είναι κανείςασθενής αυξάνει την πιθανότητα να καταστεί θύμα οποιασδήποτε παραβατικής πράξης(OR=1.71, 95 % CI={1.08, 2.71}) και η διαμονή στο χωριό μειώνει στο μισό τον κίνδυνοθυματοποίησης, σε σύγκριση με τη διαμονή στην πόλη(χωριό vs πόλη,OR.=0.56, 95%CI={0.32, 0.98}). Το χαμηλό μορφωτικό επίπεδο επιδρά και αυτό προστατευτικά σε σχέσημε την πανεπιστημιακή μόρφωση (OR=0.44, 95% CI={0.21 , 0.96}).Τα αποτελέσματα της λογιστικής παλινδρόμησης με εξαρτημένες μεταβλητές τηθυματοποίηση από επίθεση/ απειλή και συμπεριφορά διάκρισης έδειξαν ότι οι ασθενείςέχουν 4.6 φορές μεγαλύτερο κίνδυνο να είναι θύματα επίθεσης/ απειλής (OR=4.62, 95% CI={2.32, 9.19) και 3.3 φορές μεγαλύτερο κίνδυνο να είναι θύματα διάκρισης (OR=3,34, 95%CI={2.1, 5.62}) από ότι οι υγιείς.Στο δεύτερο δείγμα το 50% των ατόμων με σωματική αναπηρία, ανέφεραν ότι ήτανθύματα κάποιας, βίαιης ή μη, παραβατικής πράξης εις βάρος τους και το 57 % ότι ήτανθύματα κάποιας μορφής συμπεριφοράς διάκρισης. Οι συχνότερες μορφές θυματοποίησηςαφορούσαν στην κλοπή ή βλάβη του μεταφορικού τους μέσου (13%), και στις γυναίκες οιπράξεις σεξουαλικής παρενόχλησης (13%).
