Psychological factors in essential hypertension

Clinical practice and research provide evidence indicating the involvement of psychological and psychophysiological factors in essential hypertension. According to previous studies personality traits play an important role in the development of cardiovascular disease. Moreover, little is known about the role of interoception (i.e. the ability of perceiving bodily signals) in essential hypertension. Ambulatory blood pressure monitoring (ABPM) provides a useful tool in the detection and evaluation of hypertension and in the assessment of abnormalities in blood pressure circadian pattern, such as a lack of nocturnal BP, related with increased cardiovascular risk. The aim of this study was 1) to compare newly diagnosed hypertensive patients and normotensive individuals concerning personality traits, such as depression, hostility, alexithymia, hypochondriasis, neuroticism, extroversion, psychoticism and somatosensory amplification, implicated in cardiovascular disease 2) to investigate the impact of personality traits on dipping status and 3) to assess interoceptive awareness in newly diagnosed-untreated hypertensives, by means of ambulatory blood pressure monitoring (ABPM) The study population consisted of 83 untreated newly diagnosed hypertensives (49.7±13.2 years) and 31 normotensives (48.2±10.4 years) matched regarding sex, bmi and prevalence of smoking. The study population was divided in terms of dipping status in two groups, “dippers” and “non dippers”. All participants underwent 24-hour ABPM (Spacelabs 90207). Personality traits were assesed by psychometric instruments. Cardiac interoceptive awareness was assessed by means of a heartbeat detection task. The statistical analysis revealed that hypertensives exhibited higher office blood pressure (BP) and heart rate (HR) levels (clinic systolic BP: 152±20 vs 139±19; p=0.001, clinic diastolic BP: 96±11 vs 87±11; p<0.001, clinic HR: 81±14 vs 74±10; p=0.017) as well as ambulatory measurements (systolic BP24: 135±10 vs 117±7; p<0.001, diastolic BP24: 86±8 vs 72±5; p<0.001, HR24: 76±9 vs 71±10; p<0.01) compared to normotensives. Furthermore, dippers presented significantly higher score of extrapunitiveness (13.2±4.9 vs 11.4±3.9, p=0.032), as well as significantly higher score of the dimension “urge to act out hostility (AH)” (4±3 vs 3±2, p=0.025) compared to non dippers. Multivariate regression analysis revealed extrapunitiveness as the only independent predictor of dipping status. The odds ratio (OR) for dipping status associated with each point increase in extrapunitiveness was 0.912 (95% CI: 0.832-0.992; p=0.048). Moreover, the analysis revealed an increased interoceptive awareness in hypertensives as compared to normotensives (odds ratiο: 10.5, 95% CI 1.3-85.2). A comparison within the hypertensive group between subjects with and without interoceptive awareness revealed that subjects with increased interoceptive awareness had higher office systolic (p= 0.03) and diastolic (p=0.046) blood pressure values, as well as mean ambulatory HR (p=0.047). The present data may suggest that psychological factors and psychophysiological parameters are associated with essential hypertension and blood pressure circadian profil. Especially, hostility and its features affect the circadian variation of blood pressure in hypertensive patients, providing a promising objective for future investigations linking psychological factors and dipping status in essential hypertension. Moreover, the better performance of hypertensive individuals compared to normotensives in a heartbeat detection task, give credence to the idea that interoceptive awareness may represent an enhanced cardiovascular reactivity involved in essential hypertension, even in its early stages. The present findings suggest that various stimuli that contribute to the development of hypertension are not independent of each other, but tend to interact in clusters. A final common pathway for many of these factors could be the sympathetic nervous system, which is involved in the development of essential hypertension, even in its early stages. Behavioral interventions and individualized approaches may be beneficial in the management of essential hypertensive patients. The cross-sectional nature of this study precludes causal inference, but provides valuable directions for future prospective investigations.Η κλινική πράξη και ένας μεγάλος αριθμός ερευνητικών δεδομένων υποδεικνύουν τη θετική συσχέτιση μεταξύ ψυχολογικών παραμέτρων και ιδιοπαθούς αρτηριακής υπέρτασης. Σύμφωνα με προηγούμενες μελέτες ορισμένα χαρακτηριστικά της προσωπικότητας παίζουν έναν σημαντικό ρόλο στην ανάπτυξη της καρδιαγγειακής νόσου. Επίσης σημαντικός φαίνεται να είναι ο ρόλος της ενδοδεκτικής αντίληψης, της αντίληψης δηλ. μεταβολών που συμβαίνουν εντός του σώματος, στην ιδιοπαθή ΑΥ, τα δεδομένα όμως είναι περιορισμένα. Η 24ωρη καταγραφή αποτελεί ένα χρήσιμο εργαλείο στη διάγνωση και την αξιολόγηση της ΑΠ καθώς και στις διαταραχές του κιρκαδικού προφίλ όπως η έλλειψη νυχτερινής κατάδυσης της ΑΠ, οι οποίες σχετίζονται με αυξημένο καρδιαγγειακό κίνδυνο. Σκοπός της παρούσας μελέτης ήταν να διερευνήσει με τη βοήθεια της 24ωρης καταγραφής ΑΠ 1) πιθανή διαφοροποίηση νεοδιαγνωσθέντων υπερτασικών ασθενών και νορμοτασικών ατόμων ως προς ορισμένα χαρακτηριστικά της προσωπικότητας όπως η κατάθλιψη, η εχθρικότητα, η αλεξιθυμία, η υποχονδρίαση, ο νευρωτισμός, η εξωστρέφεια, ο ψυχωτισμός και η σωματική ευαισθησία τα οποία έχουν συσχετισθεί με την καρδιαγγειακή νοσηρότητα 2) πιθανή συσχέτιση των παραπάνω ψυχολογικών παραμέτρων με διαταραχές στο κιρκάδιο προφίλ της ΑΠ 3) πιθανή διαφοροποίηση μεταξύ υπερτασικών και νορμοτασικών ατόμων ως προς μια ψυχοφυσιολογική παράμετρο, την ενδοδεκτική αντίληψη (interoception), η οποία εκφράζει την αντίληψη των μεταβολών που συμβαίνουν εντός του σώματος. Τον πληθυσμό της μελέτης αποτέλεσαν 83 νεοδιαγνωσθέντες υπερτασικοί ασθενείς, οι οποίοι δεν ελάμβαναν θεραπεία (49.7±13.2 έτη) και 31 νορμοτασικά άτομα (48.2±10.4 έτη). Οι δύο ομάδες δεν παρουσίασαν σημαντικές διαφορές ως προς την κατανομή του φύλου, το δείκτη μάζας σώματος και το ποσοστό των καπνιστών. Προκειμένου να αξιολογηθούν διαταραχές στο κιρκαδικό προφίλ της ΑΠ ο πληθυσμός της μελέτης κατηγοριοποιήθηκε σε δύο ομάδες με βάση την παρουσία (dippers) ή όχι (non dippers) νυχτερινής κατάδυσης της ΑΠ. Όλοι οι ασθενείς υποβλήθηκαν σε 24ωρη καταγραφή της ΑΠ με συσκευή Spacelabs 90207. Τα χαρακτηριστικά της προσωπικότητας αξιολογήθηκαν με κατάλληλα ψυχομετρικά εργαλεία. Η ενδοδεκτική αντίληψη αξιολογήθηκε με μια δοκιμασία διάκρισης των καρδιακών παλμών. Η στατιστική ανάλυση έδειξε ότι τα υπερτασικά άτομα είχαν σημαντικά υψηλότερες τιμές ΑΠ και καρδιακής συχνότητας (ΚΣ) στο ιατρείο (ΣΑΠ ιατρείου 152±20 έναντι 139±19; p=0.001, ΔΑΠ ιατρείου: 96±11 έναντι 87±11; p<0.001, ΚΣ ιατρείου: 81±14 έναντι 74±10; p=0.017) καθώς και τιμές ΑΠ και ΚΣ στην 24ωρη καταγραφή (ΣΑΠ 24ωρου: 135±10 έναντι 117±7; p<0.001, ΔΑΠ 24ωρου: 86±8 έναντι 72±5; p<0.001, ΚΣ 24ωρου: 76±9 έναντι 71±10; p
