Study of the structure and function of glomerular epithelial cells in an experimental model of lupus nephritis and in normal mice after administration of the macrolide rapamycine

Introduction: In the normal kidney, rapamycin is considered to be non-nephrotoxic based on initial studies in some animal species. In the diseased kidney, rapamycin may be beneficial or detrimental, depending on the type of renal injury. One explanation for the conflicting results maybe either the dose used, or the duration of rapamycin administration. The current study was undertaken a) to examine whether or not sirolimus is indeed non-nephrotoxic in normal Balb/c mice b) to define the clinical manifestations of the nephrotoxicity, proteinuria or decline of GFR c) in case that rapamycin is nephrotoxic to determine whether the toxicity is dose or time dependent d) in case that rapamycin induces glomerular proteinuria to study the molecular mechanisms of podocyte dysfunction Methods: Two months old Balb/c mice were given three different doses of rapamycin i.p for 1 week (LD:1, ID:1.5, and HD:3mg/kgBW/day, 4 mice in each group – dose model). An intermediate dose of rapamycin (1.5mg/kg) was given i.p for three different periods 1, 4 and 8 weeks (4 mice in each group, -time model). Six Balb/c mice were injected with the rapamycin solvent (DMSO) and served as controls. The 24h urine protein excretion and BW were estimated weekly. The 24h urine albumin excretion was measured at the beginning and the end of the study. Rapamycin and creatinine blood levels (Cr) were measured at sacrifice. Kidney morphology was examined by light microscopy, electron microscopy (EM) and immunofluorescence (IF for nephrin and podocin). WB & RT-PCR were performed to examine the expression of the slit diaphragm proteins nephrin and podocin. Glomerular Akt kinase expression and phosphorylation was examined by WB. Results: All treated animals reduced urine protein excretion throughout the course of the study while urine albumin was increased statistically significantly in the HD and 4-weeks groups. Renal function and body weight showed a mild to moderate decline especially in the HD group (Cr: 0.34±0.03 vs. 0.24±0.01mg.dl in controls, p<0.01). By EM, we noticed an increase in the mean foot process width on the first week (controls 335nm, LD 385nm, ID 420nm, HD 420nm), which was further deteriorated on the 4th week (456nm) and finally improved on week 8 (401nm). Podocin and Nephrin mRNA levels both showed a significant (40-60% lower vs. control, all p<0.001) and dose-independent decrease on the first week, which improved on the 4th week and was restored to normal on week 8. Nephrin and podocin protein levels showed a more delayed and less pronounced decline (relative to mRNA levels) while on IF they were normally expressed in the periphery of the capillary loops. Total Akt levels remained stable in both models but Ser473phospho-Akt increased slightly in the HD group and significantly (p<0.005) in the 4 and 8-weeks groups. Conclusions: In both models, significant alterations were noticed in kidney function, podocyte architecture, slit diaphragm proteins transcription and glomerular Akt activation. These alterations occured mainly during the first month of treatment while they were completely restored in the long term, indicating that a rapamycin “escape phenomenon” may exist. The observed increase in Akt phosphorylation may be related to the concomitant alterations of the slit diaphragm proteins or the induction of a member of the PDK2s family of kinases. It is not clear though, if it is part of the pathogenetic mechanism for the observed glomerular alterations especially in podocytes or it represents a compensatory mechanism eventually leading to resolution of the lesions. […]1. Μελέτη της επίδρασης της ραπαμυκίνης στη δομή και λειτουργία των επιθηλιακών σπειραματικών κυττάρων σε φυσιολογικά ποντίκια. Εισαγωγή: Στα φυσιολογικά νεφρά η ραπαμυκίνη θεωρείται μη νεφροτοξική αλλά η άποψη αυτή στηρίχθηκε σε ορισμένα μόνο είδη ζώων. Σε πάσχοντα νεφρά η ραπαμυκίνη είναι άλλοτε ωφέλιμη και άλλοτε βλαπτική, αναλόγως του τύπου της νεφρικής βλάβης. Μία πιθανή εξήγηση για τα αντιφατικά αποτελέσματα αποτελεί ίσως η δόση που χρησιμοποιείται καθώς και η διάρκεια χορήγησης. Ο στόχος της παρούσας μελέτης ήταν να εξετάσουμε: α) το αν η ραπαμυκίνη (σιρολίμη) είναι ή όχι νεφροτοξική σε φυσιολογικά ποντίκια. Balb/c. β) να καθορίσουμε τις κλινικές εκδηλώσεις της νεφροτοξικότητας, δηλαδή εμφάνιση πρωτεϊνουρίας ή μείωση της σπειραματικής διήθησης. γ) στην περίπτωση που η ραπαμυκίνη είναι νεφροτοξική να καθορίσουμε το αν η τοξικότητα είναι δοσοεξαρτώμενη ή χρονοεξαρτώμενη. δ) στην περίπτωση που η ραπαμυκίνη προκαλεί σπειραματική λευκωματουρία να μελετήσουμε τους μηχανισμούς δυσλειτουργίας των ποδοκυττάρων. Μέθοδοι: Σε ποντίκια Balb/c ηλικίας 2 μηνών χορηγήθηκαν 3 διαφορετικές δόσεις ραπαμυκίνης ενδοπεριτοναϊκά για 1 εβδομάδα (χαμηλή δόση ΧΔ: 1, ενδιάμεση δόση ΕΔ: 1.5, και υψηλή δόση ΥΔ: 3 mg/kg ΒΣ/ημέρα, 4 ποντίκια σε κάθε ομάδα – μοντέλο δόσης ΜΔ). Μια ενδιάμεση δόση ραπαμυκίνης (1.5mg/kg/ημέρα) χορηγήθηκε ενδοπεριτοναϊκά για 3 διαφορετικές περιόδους (σύντομη χορήγηση ΣυΧ: για 1 εβδομάδα, ενδιάμεση χορήγηση ΕνΧ: για 4 εβδομάδες και μακροχρόνια χορήγηση ΜαΧ: για 8 εβδομάδες, 4 ποντίκια σε κάθε ομάδα – μοντέλο χρόνου ΜΧ). Έξι ποντίκια έλαβαν μόνο το έκδοχο (DMSO) και χρησίμευσαν ως ομάδα ελέγχου (ΟΕ). Η πρωτεΐνη ούρων 24ωρου και το σωματικό βάρος προσδιορίστηκαν στην αρχή της μελέτης και ανά εβδομάδα. Η αλβουμίνη ούρων 24ωρου στην αρχή και στο τέλος της μελέτης. Τα επίπεδα ραπαμυκίνης και κρεατινίνης στο αίμα προσδιορίστηκαν κατά τη θυσία. Η ιστολογία των νεφρών εξετάστηκε με φωτονικό (ΦΜ) και ηλεκτρονικό μικροσκόπιο (ΗΜ) και με ανοσοφθορισμό (ΑΦ) για τις πρωτεΐνες νεφρίνη και ποδοσίνη. Επίσης έγινε WB & RT-PCR για τη μέτρηση της πρωτεϊνικής και γονιδιακής έκφρασης της νεφρίνης και ποδοσίνης. Τέλος η σπειραματική έκφραση της κινάσης Akt και η φωσφορυλίωσή της εξετάστηκαν με WB. Αποτελέσματα: Όλα τα ζώα που έλαβαν θεραπεία παρουσίασαν μείωση της αποβολή ολικής πρωτεΐνης στα ούρα κατά τη διάρκεια της μελέτης, ενώ παρουσίασαν αύξηση της αλβουμινουρίας στατιστικά σημαντική για τις ομάδες ΥΔ και ΕνΧ. Η νεφρική λειτουργία και το σωματικό βάρος παρουσίασαν ελαφρά-μέτρια μείωση κυρίως στην ομάδα της ΥΔ (Κρεατινίνη ορού: 0.34±0.03 vs. 0.24±0.01 mg/dl στην ΟΕ, p<0.01). Στο ΗΜ παρατηρήσαμε μια αύξηση της μέσης τιμής του πάχους των ποδικών εκβλαστήσεων την 1η εβδομάδα (ΟΕ 335nm, ΧΔ 385nm, ΕΔ&ΣυΧ 420nm, ΥΔ 420nm), η οποία επιδεινώθηκε περαιτέρω την 4η εβδομάδα χορήγησης (ΕνΧ 456nm) και τελικά βελτιώθηκε την 8η εβδομάδα (ΜαΧ 401nm). Τα επίπεδα mRNA της νεφρίνης και ποδοσίνης παρουσίασαν σημαντική πτώση την 1η εβδομάδα (40-60% χαμηλότερα από την ΟΕ, όλα τα p<0.001), ανεξαρτήτως χορηγούμενης δόσης, στη συνέχεια την 4η εβδομάδα βελτιώθηκαν και επανήλθαν στο φυσιολογικό την 8η εβδομάδα. Τα επίπεδα των πρωτεϊνών νεφρίνης και ποδοσίνης παρουσίασαν οριακή πτώση την 4η εβδομάδα και αποκαταστάθηκαν πλήρως την 8η εβδομάδα, ενώ στον ανοσοφθορισμό εκφράστηκαν φυσιολογικά στην περιφέρεια των τριχοειδικών αγκυλών. Τα επίπεδα της ολικής Akt παρέμειναν σταθερά και στα 2 μοντέλα αλλά η φωσφορυλίωσή της στη θέση Ser473 αυξήθηκε μέτρια στην ομάδα της ΥΔ και σημαντικά στις ομάδες ΕνΧ και ΜαΧ (p
