Angiogenetic factors expression in patients with non-small cell lung cancer

Background: Angiogenesis has been proven to be a process related to the migration, proliferation and metastasis of cancer cells. Its predominant moderator is the VEGF which imposes its angiogenetic action through the transmembran receptors VEGFR1 and VEGFR2. The prognostic value of the measurement of these factors in the blood serum of patients is still controversial while their evaluation in the washing has not been studied sufficiently. Aim: The aim of this study is to define the VEGF, VEGFR1 and VEGFR2 and the ratios of VEGF/VEGFR1 and VEGF/VEGFR2 in the blood serum and the washing of patients with newly diagnosed non-small cell lung cancer (NSCLC) and their correlation with the basic clinical characteristics of the patients and the tumors, the treatment response of those who had chemotherapy, the progression free survival (PFS) and the overall survival (OS) of the patients. We will also investigate the correlation of positive VEGF immunohistochemistry with concentration of angiogenic markers in serum and washing. Patients and methods: Forty patients with newly diagnosed NSCLC participated in the current study. The measurement of the circulating and washing levels of VEGF, VEGFR1 and VEGFR2 was carried out with the sandwich enzyme-linked immunosorbent assay (ELISA). For immunohistochemistry we have used specific anti- VEGF (Biogenex, San Ramon, CA). Results: Circulating VEGF levels is correlated with T descriptors in TNM staging system (Ρ=0.021), as well as the ratio VEGF/VEGFR2 in serum and washing (Ρ= 0.03. and Ρ=0.040 relatively). From those who were treated with chemotherapy, best responses were observed in lower concentrations of VEGF in serum and washing (Ρ<0.001). Higher concentrations of VEGF in washing are correlated with worse overall survival (HR 2.208, 95%CI 1.132-4.308, P=0.020) and PFS (HR 2.265, 95%CI 1.145-4.483, P=0.019). Similar results for OS and PFS were observed with high values of the VEGF/VEGFR2 ratio in washing (HR 3.056, 95%CI 1.483-6.297, P=0.002 for survival and HR 3.067, 95%CI 1.508-6.236, P=0.002 for PFS). Multivariate Cox analysis revealed as independent markers for overall survival VEGFR2 levels in serum and washing (Ρ=0.017 και Ρ=0.004 relatively), while for PFS independent markers were VEGFR1 (Ρ= 0.004), VEGF/VEGFR1 (Ρ=0.033) and VEGF/VEGFR2 (Ρ=0.007) in washing. No correlation was noted between IHC-VEGF and circulating or washing angiogenic markers. Conclusions: The circulating VEGF levels are controversial. Nevertheless, the definition of angiogenic markers in washing could recognize a high risk group of patients who could benefit from an aggressive initial therapeutic approach.Εισαγωγή: Η αγγειογένεση έχει αποδειχτεί ότι είναι μία διαδικασία που σχετίζεται με την αύξηση, τον πολλαπλασιασμό και το μεταστατικό δυναμικό των καρκινικών κυττάρων. Ο κύριος μεσολαβητής της είναι ο VEGF που ασκεί την αγγειογενετική δράση του μέσω σύνδεσης με τους διαμεμβρανικούς υποδοχείς VEGFR1 και VEGFR2. Η προγνωστική αξία της μέτρησης αυτών των δεικτών στον ορό ασθενών είναι αμφιλεγόμενη ενώ η αξιολόγησή τους στο βρογχικό έλπλυμα δεν έχει μελετηθεί επαρκώς. Σκοπός: Σκοπός αυτής της μελέτης είναι ο προσδιορισμός των VEGF, VEGFR1 και VEGFR2 και του λόγου VEGF/VEGFR1 καιι VEGF/VEGFR2 στον ορό και στα δείγματα βρογχικού εκπλύματος (washing) ασθενών με πρωτοδιαγνωσμένο μη μικροκυτταρικό καρκίνο πνεύμονα και συσχέτισή τους με κλινικά χαρακτηριστικά των ασθενών και των όγκων, την ανταπόκριση στην θεραπεία για τους ασθενείς που έλαβαν χημειοθεραπεία, το διάστημα επιβίωσης χωρίς υποτροπή της νόσου και την συνολική επιβίωση όλων των ασθενών. Παράλληλα θα εξεταστεί η σημασία της ύπαρξης θετικού ανοσοιστοχημικού αποτελέσματος για τον VEGF στα δείγματα ιστού των ασθενών και η σχέση του με τα ανιχνεύσιμα επίπεδα δεικτών στον ορό και στο βρογχικό έκπλυμα. Τέλος τα επίπεδα VEGF, VEGFR1 και VEGFR2 στον ορό των ασθενών θα συγκριθούν με τις αντίστοιχες τιμές από υγιείς εθελοντές. Ασθενείς και μέθοδοι: Σαράντα ασθενείς με πρωτοδιαγνωσμένο μη μικροκυτταρικό καρκίνο του πνεύμονα συμμετείχαν σε αυτή την ερευνητική εργασία. Η μέτρηση των τριών δεικτών VEGF,VEGFR1 και VEGFR2 στον ορό και στο βρογχικό έκπλυμα έγινε με την διπλή (sandwich) μέθοδο ενζυμικής ανοσοαπορρόφησης (enzyme-linked immunosorbent assay, ELISA). Για τον ανοσοιστοχημικό προσδιορισμό χρησιμοποιήθηκαν ειδικά αντισώματα έναντι VEGF (Biogenex, San Ramon, CA). Αποτελέσματα: Η συγκέντρωση του VEGF στον ορό των ασθενών σχετίζεται με τοπικά εκτεταμένη νόσο όπως αυτή ορίζεται με το παράγοντα Τ του συστήματος σταδιοποίησης (Ρ=0.021). Το ίδιο και ο λόγος VEGF/VEGFR2 στον ορό και στο βρογχικό έκπλυμα (Ρ= 0.03. και Ρ=0.040 αντίστοιχα). Από τους ασθενείς που έλαβαν χημειοθεραπεία όσοι είχαν χαμηλές συγκεντρώσεις VEGF είχαν στατιστικά καλύτερες ανταποκρίσεις στην θεραπεία (Ρ
