Thrombotic risk factors in patients with primary billiary cirrhosis and non alcoholic steatohepatitis: correlation with the severity of hepatic fibrosis

Background/Aims: Presence of thrombotic risk factors have been frequently detected in patients with chronic viral hepatitis and have been associated with more advanced fibrosis. We evaluated the prevalence of thrombotic risk factors in non-alcoholic fatty liver disease (NAFLD) and primary billiary cirrhosis (PBC) and their possible associations with more advance fibrosis. We also compared our findings with previous data from another study in patients with chronic hepatitis B or C. Methods: We included 60 consecutive patients and 19 patients with histologically documented NAFLD or PBC respectively and a historical cohort of 90 patients with chronic hepatitis B (n=39) or C (n=51). Thrombophilic factors were evaluated on the day of the liver biopsy. Results: 37% and 12% of NAFLD patients had ³1 or ³2 thrombotic risk factors respectively, being less frequently present than in chronic viral hepatitis patients (68% and 37% respectively, P?0.001). Among NAFLD cases, ³1 thrombotic risk factor was significantly more frequently present in NASH than simple fatty liver [56% vs 8%, odds ratio (OR): 13.8 (2.8-67.4), P<0.001]. Among patients with PBC, thrombotic risk factors were rarely detected (<6%) and their presence was not associated with advanced fibrosis. In multivariate analysis, NASH was independently associated with moderate/severe steatosis (adjusted OR: 24,3, P=0.001) and presence of ³1 thrombotic risk factor (adjusted OR: 38,7, P=0.002). Fibrosis stage was worse in NASH patients with than without thrombotic risk factor(s) (2.5±1.1 vs 1.3±1.1, P=0.002). Conclusions: Thrombotic risk factor(s) are frequently present in patients with NAFLD, particularly NASH and rarely in patients with PBC. In NASH, presence of thrombotic risk factor(s) is associated with more advanced fibrosis. Such an association may have significant clinical implications, even though it is not clear yet whether it represents a primary or secondary phenomenon.Εισαγωγή-Σκοπός: Παρουσία θρομβοφιλικών παραγόντων έχει ανιχνευθεί συχνά σε ασθενείς με χρόνια ιογενή ηπατίτιδα και έχει συσχετισθεί με πιο σοβαρή ίνωση. Στην παρούσα μελέτη εκτιμήθηκε η συχνότητα θρομβοφιλικών παραγόντων στη μη αλκοολική λιπώδη νόσο του ήπατος (NAFLD) και την πρωτοπαθή χολική κίρρωση (PBC), η πιθανή συσχετισή τους με τη σοβαρότητα της ίνωσης και συγκρίθηκαν τα αποτελέσματα με τα αντίστοιχα ευρήματα σε ομάδα ασθενών με χρόνια ιογενή ηπατίτιδα. Μέθοδοι: Συμπεριλήφθηκαν 60 διαδοχικοί ασθενείς με ιστολογικά επιβεβαιωμένη NAFLD και 19 ασθενείς με PBC και μια ομάδα 90 ασθενών με χρόνια ιογενή ηπατίτιδα Β (n=39) ή C (n=51).
