A study on the effect of 2nd and 3rd generation mobile telephones on the auditory perception of digital hearing aid users

The provision of amplification is currently the only effective means for managing sensorineural hearing loss. However, the interaction between digital wireless and hearing aid technologies may result in audible electromagnetic interference, thus impeding the accessibility of hard-of-hearing people to the available information. The aim of the present thesis was to quantify the bystander effect of 2nd and 3rd generation mobile telephones on the auditory perception of digital hearing aid users. Potential differences in the susceptibility of behind-the-ear (BTE) compared to in-the-ear (ITE) hearing aids were also assessed. Patients/Methods: The methodology of the study was based on the Greek edition of the Bamford-Kowal-Bench (BKB) sentence lists (open sentence test). Five different lists, each of them comprising of 20 sentences, were used. Subject performance was assessed as the number of key-words (out of a total of 50 for each list) that were correctly identified, and was then expressed as % percentage by doubling the total score of the correctly identified key-words. The cut-off inclusion criterion regarding baseline aided word recognition score was 75%. A power analysis had revealed that at least 60 subjects with sensorineural hearing loss (30 in each group), using either BTE, or ITE digital hearing aids, were needed to find, at the p=0.05 level, with 90% certainty, a difference of at least 3% in speech discrimination. Sixty-four adults with sensorineural hearing-loss (38 males and 26 females) using digital hearing aids met the criterion and were finally included. Thirty of them were wearing BTE and 34 ITE hearing aids. The ITE group comprised of participants that were using cosmea top (CT) hearing devices, or smaller in-the-canal hearing aids (cosmea small-CS), or even completely-in-the-canal hearing devices (CIC). The hearing handicap of each participant was calculated, according to the guidelines of the American Academy of Otolaryngology. The participants were tested with a functioning 2nd generation mobile telephone at almost physical contact with their ear. According to the research protocol, 3rd generation mobile telephones were not ultimately employed in the study, as the mean differences in word recognition with and without the functioning mobile telephone were less than 10% for both groups. The SPSS 16.0 statistical package was used to compare variance within the ITE group. Paired student’s t-test was performed to assess the measured differences in speech perception within each group after the activation of the mobile telephone. Simple t-test was used to compare the measured differences in the mean aided word recognition scores between the examined groups before and after the activation of the mobile telephone. Statistical importance was accepted at a level of 0.05. Results: The initial aided word recognition scores showed a baseline difference between the two examined categories of digital hearing aids; ITE digital hearing aids scored better than BTE under aided conditions (p< 0.01)...Η ακουστική ενίσχυση και αποκατάσταση παραμένει το μοναδικό αποτελεσματικό μέσο για την αντιμετώπιση της νευροαισθητήριας βαρηκοΐας και η εφαρμογή των ακουστικών βαρηκοΐας συνδυάζεται με στατιστικώς σημαντική βελτίωση στην ποιότητα ζωής των βαρήκοων ατόμων, ελαττώνοντας τις αρνητικές ψυχολογικές, κοινωνικές και συναισθηματικές επιπτώσεις που σχετίζονται με τη βαρηκοΐα τους. Παρά ταύτα, η εισαγωγή της ψηφιακής ασύρματης τεχνολογίας δημιούργησε νέα δυνητικά εμπόδια στην προσπέλαση των διαθέσιμων πληροφοριών από τα βαρήκοα άτομα, λόγω της πιθανής υπάρξεως ακουστών ηλεκτρομαγνητικών παρεμβολών από την αλληλεπίδραση κινητών τηλεφώνων και ακουστικών βαρηκοΐας. Ο σκοπός της παρούσας μελέτης ήταν να εξετάσει ποσοτικά την πιθανή έμμεση (παρεμπίπτουσα) επίδραση των κινητών τηλεφώνων δεύτερης και τρίτης γενιάς στην ακουστική αντίληψη ασθενών που φέρουν ψηφιακά ακουστικά βαρηκοΐας. Εξετάστηκαν επίσης οι πιθανές διαφοροποιήσεις στην επιδεκτικότητα των οπισθοωτιαίων, σε σχέση με τα ενδοωτιαία ακουστικά βαρηκοΐας, στις ακουστές ηλεκτρομαγνητικές παρεμβολές. Ασθενείς/Μέθοδος: Η μεθοδολογία της μελέτης βασίστηκε στην καταγραφή της ακουστικής αντιλήψεως λέξεων-κλειδιών σε προτάσεις της δοκιμασίας BKB (Bamford-Kowal-Bench) μεταφρασμένης στην ελληνική γλώσσα (ανοιχτή δοκιμασία αναγνωρίσεως προτάσεων). Οι προς αναγνώρισιν λέξεις ήταν ενσωματωμένες σε πέντε διαφορετικές, απλές και εύκολα κατανοητές ιστορίες (σενάρια), αποτελούμενες από 20 διαφορετικές προτάσεις η καθεμιά. Σε κάθε σενάριο υπήρχαν 50 λέξεις-κλειδιά και για την εξαγόμενη βαθμολογία υπολογιζόταν ο συνολικός αριθμός αυτών που είχαν ειπωθεί σωστά από τον εξεταζόμενο, αφού διπλασιαζόταν για να μετατραπεί στο αντίστοιχο ποσοστό επί τοις εκατό (%). Εξετάστηκαν ασθενείς με ποσοστό διακρίσεως τουλάχιστον 75% με ακουστικό (? 75%). Η ανάλυση ισχύος (power analysis), που προηγήθηκε της ενάρξεως της μελέτης, προσδιόρισε, ότι ο μικρότερος δυνατός αριθμός συμμετεχόντων στη μελέτη, που ήταν αναγκαίος για να βρεθεί μια διαφορά στη διάκριση της ομιλίας μεταξύ των δύο εξεταζομένων ομάδων της τάξεως του 3%, στο επίπεδο στατιστικής σημαντικότητας του 5% και με 90% βεβαιότητα, ανερχόταν στους 60 (30 σε κάθε ομάδα). Στη μελέτη συμπεριελήφθησαν 64 ενήλικες πάσχοντες από νευροαισθητήρια βαρηκοΐα, που χρησιμοποιούσαν ψηφιακό ακουστικό βαρηκοΐας (38 άνδρες και 26 γυναίκες). Το ποσοστό της ακουστικής αναπηρίας καθενός εκ των συμμετεχόντων υπολογίστηκε σύμφωνα με τη μέθοδο εκτιμήσεως της Αμερικανικής Ακαδημίας Ωτορινολαρυγγολογίας. Τριάντα ασθενείς έφεραν οπισθοωτιαία ακουστικά και 34 ενδοωτιαία ακουστικά βαρηκοΐας. Στην ομάδα των φερόντων ενδοωτιαία ακουστικά συμπεριελήφθησαν ασθενείς φέροντες τόσο μικρού μεγέθους ενδοκαναλικά ενδοωτιαία ακουστικά (cosmea top - CT), όσο και μικρότερου μεγέθους (cosmea small - CS), όπως επίσης και πλήρως ενδοκαναλικά (completely-in-the-canal - CIC) ενδοωτιαία ακουστικά βαρηκοΐας..
