Degression and cognitive disorders in Parkinson's disease: a study with neuropsychological and psychophysiological methods

The aim of this doctoral thesis is the study of depression and cognitive disorders in Parkinson’s disease (PD). 43 patients with identified PD were examined with the following neuropsychological tests HAM-D₁₇, MMSE and Wechsler II and were divided into four groups: a) 11 patients without depression or cognitive disorders, b) 11 patients with depression, c) 11 patients with cognitive impairment and d) 10 patients with coexisting depression and cognitive decline. They were also examined with visual psychophysiological tests such as VEP, ERG and pupillometry. The PD patients had pathological pupillometric findings compared to normal control subjects, especially in Vcmax and Acmax. This fact may be attributed to a central cholinergic deficit. The patients with cognitive disorders exhibited the most pathological findings. Pupillometry can be used for the differential diagnosis between dementia and depressive “pseudodementia”.Ο στόχος της διατριβής είναι η διερεύνηση της κατάθλιψης και των γνωστικών διαταραχών στη νόσο του Πάρκινσον. Σε 43 ασθενείς με τη συγκεκριμένη νόσο χορηγήθηκαν οι εξής νευροψυχολογικές δοκιμασίες HAM-D₁₇, MMSE και Wechsler II, οπότε οι ασθενείς διαχωρίσθηκαν στις εξής 4 ομάδες α) 11 χωρίς κατάθλιψη ή γνωστικές διαταραχές, β) 11 με κατάθλιψη, γ) με γνωστικά ελλείμματα και δ) με συνυπάρχουσα κατάθλιψη και γνωστική έκπτωση. Οι παραπάνω ασθενείς εξετάσθηκαν με οπτικές ψυχοφυσιολογικές μεθόδους ΟΠΔ, ΗΑΓ και κορημετρία. Οι παραπάνω ασθενείς παρουσίαζαν χαμηλές τιμές, όσον αφορά τις παραμέτρους της κορημετρίας, κυρίως την Vcmax και την Acmax, γεγονός το οποίο αποδίδεται σε κεντρικό χολινεργικό έλλειμμα. Περισσότερο χαμηλές τιμές παρουσίαζε η ομάδα με γνωστικές διαταραχές. Με τη μέθοδο της κορημετρίας είναι επίσης δυνατός ο διαχωρισμός ανάμεσα στην αμιγή άνοια από τη καταθληπτικού τύπου «ψευδοάνοια»
