Distribution of genes encoding resistance to macrolides and lincosamides among staphylococcal islates collected from greek hospitals

In Greece, as in many other countries, methicillin-resistant staphylococcal strains (S. aureus, coagulase-negative staphylococci) are the major cause of nosocomial infections. The treatment of these infections is difficult, as many of the methicillin-resistant staphylococci exhibit cross-resistance to a broad spectrum of antimicrobial agents. Clones of these multiresistant staphylococcal strains can easily predominate in the nosocomial environment and spread in different hospital care units. Macrolides (such as erythromycine) and lincosamides (such as clindamycine), which are chemically distinct antimicrobial agents but share a similar mode of action, are widely used in the treatment of staphylococcal infections. However, resistance to macrolides and lincosamides is common among staphylococci and mostly among those that exhibit resistance to methicillin. Three main mechanisms of resistance to macrolides and lincosamides have been described in staphylococci: target modification (ribosomal modification by 23S rRNA methylase mediated by ermA, ermB and ermC genes), the presence of multicomponent macrolide-efflux pumps (encoded by msrA, msrB genes) and the enzymatic inactivation of the macrolides by enzymes such as esterases EreA and EreB (encoded by ereA and ereB genes, respectively) and phosphorotransferases type II (encoded by mphC gene) and enzymatic inactivation of lincosamides by enzymes encoded by lnuA/Α΄. The expression of the first mechanism of resistance can be constitutive (resistance to all MLSB antibiotics) or inducible (resistance to 14, 15-macrolides but susceptibility to 16-macrolides, lincosamides and streptogramins B which is phenotypically presented by a flattening of the zone of inhibition, the D shaped zone, around the clindamycine disc when a disc of erythromycine is placed near a disc of clindamycine). The occurrence of efflux pump system causes resistance to macrolides and streptogramins B (MSB resistance). The aim of the present study was to determine the incidence of macrolides and lincosamides resistance among methicillin-resistant and methicillin-susceptible staphylococcal isolates in Greek hospitals by determining the minimum inhibitory concentrations of these antibiotics. It was also attempted to determine the phenotypes of resistance and the molecular mechanisms of resistance as well the genetic relatedness of isolates in order to mode of spread among multi-resistant staphylococcal strains in Greek hospitals. The material of the study was consisted of 400 staphylococcal strains (278 S. aureus and 122 CNS) isolated from patients with clinical infections in 6 Greek hospitals (3 were located in Athens, 2 were located in Thessaloniki and the General Hospital of Korinthos) during 2001-2004. Antimicrobial susceptibility was performed by the agar microdilution method. Among these isolates, 207 (51,8%) were resistant to oxacillin and characterized as methicillin-resistant, 259 isolates (64,8%) were resistant to erythromycin and 162 (40,5%) to clindamycin. Furthermore, 5 isolates (1,3%) presented intermediate susceptibility to erythromycin. Intermediate susceptibility to clindamycine was not detected in any isolate. The phenotypes of resistance were detected by the double-disk diffusion method on agar plate. The MLSB constitutive phenotype was detected in 39,8% of the isolates (159 isolates: 121 S. aureus and 38 CNS), the MLSB inducible phenotype in 17,8% of the isolates (71 isolates: 47 S. aureus and 24 CNS), the MSB phenotype in 8,0% of the isolates (32 isolates:16 S. aureus and 16 CNS). The phenotype of resistance to lincosamides was found only in 3 S. aureus isolates (0,8%). Detection of the macrolide and lincosamide resistance genes by molecular assays was performed in 265 staphylococcal isolates that exhibited resistance or intermediate susceptibility to at least one of the antibiotics (erythromycin and clindamycin). The most prevalent gene of resistance was ermA (54,7% of isolates) followed by ermC (24,9% of isolates) and msrA/msrB (11,7% of isolates). Less frequently was detected lnuA/A΄ (1,1% of isolates). The ermB, ereA, ereB and mphC genes were not detected in any of the tested staphylococci. Eleven (4,2%) of the isolates were negative for all resistance genes. Among S. aureus isolates, ermA was the prevalent resistance gene (67,4%); it was more frequently in MRSA than in MSSA (74,5% in MRSA versus 45,7% in MSSA) and especially among those that exhibited the constitutive type of MLSB resistance. On the other hand, among CNS isolates the most prevalent gene was ermC (42,3%); it was detected mostly in MSCNS (48,3% in MSCNS versus 38,8% in MRCNS) and especially among those with the inducible type of MLSB resistance. The msrA/msrB genes were more prevalent in CNS (20,5%) than in S. aureus isolates (8,0%). The lnuA/A΄ gene was detected only in 3 S. aureus isolates (1,3%), but not in CNS isolates.Στην Ελλάδα, καθώς και σε πολλές άλλες χώρες, τα μεθικιλλίνη-ανθεκτικά στελέχη σταφυλοκόκκων (Staphylococcus aureus και κοαγκουλάση-αρνητικοί σταφυλόκοκκοι) αποτελούν την κύρια αιτία νοσοκομειακών λοιμώξεων. Οι λοιμώξεις αυτές είναι δύσκολο να αντιμετωπισθούν, καθώς πολλά από τα μεθικιλλίνη-ανθεκτικά στελέχη σταφυλόκοκκων παρουσιάζουν διασταυρούμενη αντοχή και σε ένα μεγάλο φάσμα άλλων αντιμικροβιακών παραγόντων. Κλώνοι αυτών των πολυανθεκτικών στελεχών, μπορούν εύκολα να επικρατήσουν στο νοσοκομειακό περιβάλλον και να εξαπλωθούν σε διαφορετικές μονάδες νοσηλείας. Οι μακρολίδες (όπως η ερυθρομυκίνη) και οι λινκοσαμίδες (όπως η κλινδαμυκίνη), που αποτελούν χημικά διακριτές ομάδες αντιβιοτικών με παρόμοιο όμως μηχανισμό δράσης, χρησιμοποιούνται ευρέως στη θεραπεία των σταφυλοκοκκικών λοιμώξεων. Ωστόσο, η αντοχή στις μακρολίδες και τις λινκοσαμίδες είναι συχνή στους σταφυλόκοκκους και ιδιαίτερα σε αυτούς που είναι ανθεκτικοί στη μεθικιλλίνη. Τρεις κύριοι μηχανισμοί αντοχής στις μακρολίδες και τις λινκοσαμίδες έχουν περιγραφεί στους σταφυλόκοκκους: η τροποποίηση του στόχου δράσης (ριβοσωμική τροποποίηση από μια 23S rRNA μεθυλάση που κωδικοποιείται από τα ermA, ermB και ermC γονίδια), η ύπαρξη συστήματος ενεργού εκροής των μακρολίδων (που κωδικοποιείται από τα msrA/msrB γονίδια) και η ενζυματική αδρανοποίηση των μακρολίδων από ένζυμα όπως είναι: οι εστεράσες EreA και EreΒ (που κωδικοποιούνται από τα γονίδια ereA και ereB) και οι φωσφοροτρανσφεράσες τύπου ΙΙ (που κωδικοποιούνται από τα γονίδια mphC) καθώς και ενζυματική αδρανοποίηση των λινκοσαμίδων από ένζυμα που κωδικοποιούνται από τα γονίδια lnuA/A΄. Η έκφραση του πρώτου μηχανισμού αντοχής, μπορεί να είναι σταθερή (αντοχή στις μακρολίδες-λινκοσαμίδες-στρεπτογραμμίνες τύπου Β, MLSB) ή επαγώγιμη (αντοχή στις 14,15-μακρολίδες αλλά ευαισθησία στις 16-μακρολίδες, τις λινκοσαμίδες και τις στρεπτογραμμίνες τύπου Β με φαινοτυπική παρουσία μια ζώνης αναστολής σχήματος D γύρω από το δίσκο της κλινδαμυκίνης που έχει τοποθετηθεί πλησίον δίσκου ερυθρομυκίνης). Η ύπαρξη συστήματος ενεργού εκροής προκαλεί αντοχή στις μακρολίδες και στρεπτογραμμίνες τύπου Β (MSB αντοχή). Σκοπός της παρούσας μελέτης, ήταν ο προσδιορισμός της επίπτωσης της αντοχής στις μακρολίδες και τις λινκοσαμίδες ανάμεσα στα μεθικιλλίνη-ανθεκτικά και μεθικιλλίνη-ευαίσθητα στελέχη σταφυλόκοκκων σε διάφορα ελληνικά νοσοκομεία μέσω του προσδιορισμού των ελάχιστων ανασταλτικών πυκνοτήτων των συγκεκριμένων αντιβιοτικών. Επίσης, έγινε ο καθορισμός των φαινοτύπων αντοχής και η διερεύνηση των μοριακών μηχανισμών αντοχής καθώς και της γενετικής συγγένειας των ανθεκτικών στελεχών με σκοπό να προσδιοριστεί ο τρόπος διασποράς των ανθεκτικών κλώνων στα ελληνικά νοσοκομεία. Το υλικό της μελέτης αποτέλεσαν 400 στελέχη σταφυλόκοκκων (278 S. aureus και 122 CNS) που απομονώθηκαν από ασθενείς με λοιμώξεις που νοσηλεύονταν σε 6 ελληνικά νοσοκομεία (3 στην περιοχή της Αθήνας, 2 στην περιοχή της Θεσσαλονίκης και στο Γενικό Νοσοκομείο Κορίνθου), κατά την χρονική περίοδο 2001-2004. Η ευαισθησία των στελεχών στα αντιβιοτικά προσδιορίσθηκε με τη μέθοδο αραιώσεων σε άγαρ. Από τα στελέχη των σταφυλόκοκκων της μελέτης, τα 207 στελέχη (51,8%) παρουσίασαν αντοχή στην οξασιλλίνη και χαρακτηρίστηκαν ως μεθικιλλίνη-ανθεκτικά, τα 259 (64,8%) ήταν ανθεκτικά στην ερυθρομυκίνη και τα 162 (40,5%) στην κλινδαμυκίνη. Επιπλέον, 5 στελέχη (1,3%) παρουσίασαν ενδιάμεση ευαισθησία στην ερυθρομυκίνη. Ενδιάμεση ευαισθησία στην κλινδαμυκίνη δεν παρατηρήθηκε σε κανένα στέλεχος. Οι φαινότυποι αντοχής ανιχνεύθηκαν με τη μέθοδο διάχυσης των δίσκων σε στερεό άγαρ. Ο σταθερός τύπος της MLSB αντοχής ανιχνεύθηκε στο 39,8% των στελεχών (159 στελέχη: 121 S. aureus και 38 CNS), ο επαγώγιμος τύπος στο 17,8% των στελεχών (71 στελέχη: 47 S. aureus και 24 CNS) και ο MSB φαινότυπος στο 8,0% των στελεχών (32 στελέχη: 16 S. aureus και 16 CNS). Ο φαινότυπος αντοχής μόνο στις λινκοσαμίδες βρέθηκε μόνο σε 3 (1,3%) στελέχη S. aureus. Ο μοριακός έλεγχος για την ανίχνευση των γονιδίων αντοχής στις μακρολίδες και τις λινκοσαμίδες, πραγματοποιήθηκε σε 265 στελέχη σταφυλόκοκκων που παρουσίαζαν αντοχή ή ενδιάμεση ευαισθησία σε ένα τουλάχιστον από τα αντιβιοτικά ερυθρομυκίνη και κλινδαμυκίνη
