Investigation of carbapenem resistance mechanisms in multidrug resistance in acinetobacter

Surveillance and control of hospital infections caused by Gram negative bacteria has been a major concern over the last few years for the developed countries, especially after the intorduction of wide-spectrum antibiotics. Among Gram negative genera, Acinetobacter has the lead in colonization and infection of hospitalized patients. Besides causing bacteremia, urinary tract infection and secondary meningitis, the genus is highly associated with nosocomial pneumonia. Such infections are usually difficult to treat due to the pandrug phenotype that the clinical isolates of genus Acinetobacter exhibit. In addition, these isolates have the potency to survive for long period within the nosocomial settings and thus might spread not only via the healthcare workers but also the inanimate material. The first report of imipenem-resistant A. baumannii was from New York of the United States in 1994 and eversince several relative studies have been published worlwide. To date, however, the underlying molecular mechanisms of carbapenem-resistance in that species remain unknown. Most studies focus on the production of b-lactamases (e.x. AmpC, OXA-51, ΟΧΑ-58, VIM and IMP type) but none of the characterized genes code for an enzyme with strong hydrolytic activity capable of causing high level carbapenem resistance. Other mechanisms such as low permeability of outer membrane due to repression of porins (e.x. porin CarO) seem to act in synergy with b-lactamases and contribute to high level carbapenem resistance. Also, the presence of efflux pumps or the repression of penicillin-binding proteins in the cell wall is a field yet poorly studied. The initiative of this study was the epidemiological report on the dissemination of pandrug A. baumannii genotypes among clinical isolates. In that term, the study included isolates of several wards of three different and distant Greek hospitals. For each one of the recovered genotypes, the presence and expression of genes coding for known carbapenemases was examined. Also, outer mebrane protein profiles as well as the presence of efflux pumps was tested in order to postulate on the relative contribution of the identified molecular mechanisms to the resistant phenotype of the genotypes. The majority of the isolates of the study were pandrug resistant and particularly for the carbapenems, resistance rates were high (67.8% for imipenem and 40.6% for meropenem respectively) but meropenem appeared to be more effective than imipenem. The isolates belonged at least to 13 genotypes which is indicative of the multiclonality of the endemic isolates of A. baumannii in Greece. Eventhough the relation of the recovered genotypes to the European Clones Ι and ΙΙ was not investigated, this is anticipated due to the geographical location of the hospitals of the study relatively to other european healthcare settings. As for the molecular mechanisms of carbapenem resistance, the prevalence of alleles blaΟΧΑ-66 and blaΟΧΑ-69 coding for type ΟΧΑ-51 oxacillinases was detected while no association with insertion sequence ISAba1 was found in any of the isolates of the study. Type ΟΧΑ-58 oxacillinase was found in most of the isolates (65.4%) and in all of them was adjacent to insertion sequence ISAba3. That might explain the positive correlation of gene blaOXA-58 with higher level of carbapenem reistance relatively to the isolates that were positive only for gene blaOXA-51. Also, no variation in gene blaOXA-58 expression was detected among isolates of the same genotype exhibiting different susceptibilty status to carbapenems. It is plausible that ΟΧΑ-58 might not contribute to intragenotypic discrepancies of carbapenem resistance. In type VIM-1 metallo-b-lactamase positive isolates, gene blaVIM was found to be part of a class 1 integron and in three cases the variable region was similar to that widely disseminated among Gram negative pathogens in Greek hospitals. Two isolates were negative for metallo-b-lactamase production by phenotypic assays but positive for VIM-1 by PCR. That discripancy was partially due to the lower expression of gene blaVIM-1 in these two cases relatively to the first three cases. Particularly for the cell wall related mechanisms, no differences were found in the outer membrane protein profile or no presence of efflux pump was detected that could explain carbapenem resistance. Finally, the detected heteroresistance of carbapenem susceptible A. baumannii clinical isolates is of major concern. Population analysis revealed heteroresistant subpopulations exhibiting carbapenem MIC as far as 16-fold greater than the native’s population. Bactericidal assays utilizing imipenem showed low effect of the antimicrobial against these heteroresistant isolates. However, the in vivo efficacy of the carbapenems for the treatment of infections caused by such isolates is yet unknown.Ο έλεγχος των νοσοκομειακών λοιμώξεων που προκαλούνται από Gram αρνητικά βακτήρια απασχολεί τα τελευταία χρόνια όλες τις ανεπτυγμένες χώρες ιδιαίτερα μετά την εισαγωγή νέων, ευρέος φάσματος αντιβιοτικών. Από τα βακτήρια αυτά, το γένος Acinetobacter πρωταγωνιστεί στον αποικισμό και στις λοιμώξεις των νοσηλευόμενων στα νοσοκομεία. Κυρίαρχο ρόλο διαδραματίζει στη νοσοκομειακή πνευμονία ενώ είδη του γένους αυτού ενοχοποιούνται για ένα εύρος νοσοκομειακών λοιμώξεων όπως βακτηριαιμία, ουρολοίμωξη και δευτερογενής μηνιγγίτιδα. Τέτοιου είδους λοιμώξεις είναι συχνά εξαιρετικά δύσκολες στη θεραπευτική τους αντιμετώπιση λόγω του ότι τα μικροβιακά στελέχη που εμπλέκονται εμφανίζουν πολυανθεκτικό φαινότυπο αντοχής στις κυριότερες κατηγορίες των αντιβιοτικών. Το γεγονός αυτό επιβαρύνεται ακόμα περισσότερο από το ότι τα μικρόβια αυτά μπορούν να επιβιώσουν για μεγάλο διάστημα στο νοσοκομειακό περιβάλλον και έχουν τη δυνατότητα διασποράς τόσο μέσω του ανθρώπινου δυναμικού όσο και του νοσοκομειακού υλικού. Η πρώτη εμφάνιση ανθεκτικών στην ιμιπενέμη στελεχών A. baumannii αναφέρθηκε στη Νέα Υόρκη των ΗΠΑ το 1994 και έκτοτε οι σχετικές αναφορές άρχισαν να πληθαίνουν παγκοσμίως. Μέχρι και σήμερα όμως, οι υποκείμενοι μοριακοί μηχανισμοί παραμένουν άγνωστοι παρά τη συστηματική μελέτη τους. Οι περισσότερες μελέτες αναφέρονται στη παραγωγή β-λακταμασών (π.χ. τύπου AmpC, OXA-51, ΟΧΑ-58, VIM και IMP) αλλά κανένα από τα γονίδια που έχουν περιγραφεί μέχρι σήμερα δεν κωδικοποιεί για ένζυμο με τόσο ισχυρή δραστικότητα καρβαπενεμάσης ώστε να δικαιολογεί υψηλά επίπεδα αντοχής. Άλλοι μηχανισμοί όπως η μειωμένη διαπερατότητα του κυτταρικού τοιχώματος λόγω μειωμένης έκφρασης πορινών (π.χ. η CarO πορίνη) φαίνεται να δρουν συνεργικά με τις β-λακταμάσες και να προσδίδουν υψηλά επίπεδα αντοχής. Επίσης, η ύπαρξη αντλιών ενεργού εκροής καθώς και η μειωμένη έκφραση πενικιλινο-δεσμευτικών πρωτεϊνών στο τοίχωμα της πεπτιδογλυκάνης διερευνάται συστηματικά σε σχέση με το πολυανθεκτικό φαινότυπο του είδους. Αρχικός σκοπός της παρούσας μελέτης ήταν η επιδημιολογική διερεύνηση της διασποράς στον ελλαδικό χώρο πολυανθεκτικών γονοτύπων A. baumannii. Για το λόγο αυτό τα κλινικά στελέχη της μελέτης συλλέχθηκαν από κλινικές τριών γεωγραφικά απομακρυσμένων ελληνικών νοσοκομείων. Επίσης, για καθέναν από τους γονοτύπους της μελέτης, ελέγχθηκε η φορεία και η έκφραση γνωστών καρβαπενεμασών καθώς και το πρότυπο των πρωτεϊνών της εξωτερικής μεμβράνης όπως και η ύπαρξη αντλιών εκροής, με στόχο τη προκαταρκτική διερεύνηση της σχετικής συνεισφοράς στην αντοχή καθενός από τους αναζητούμενους μοριακούς μηχανισμούς. Τα στελέχη της μελέτης επιλέχθηκαν στην πλειοψηφία τους πανθεκτικά και ιδιαίτερα για τις καρβαπενέμες, τα παρατηρούμενα ποσοστά αντοχής (67.8% για την ιμιπενέμη και 40.6% για τη μεροπενέμη αντίστοιχα) ήταν αρκετά υψηλά. Ωστόσο, η μεροπενέμη βρέθηκε να υπερέχει θεραπευτικά της ιμιπενέμης. Η πολυκλωνικότητα των στελεχών που ενδημούν στα ελληνικά νοσοκομεία φάνηκε από τους τουλάχιστον 13 γονοτύπους που απομονώθηκαν από τις τρεις γεωγραφικές περιοχές της μελέτης. Εάν και δε μελετήθηκε η συσχέτιση των γονοτύπων που ανευρεθήκαν με τους Ευρωπαϊκούς κλώνους Ι και ΙΙ, ωστόσο αναμένεται η αντιστοιχία ορισμένων από αυτούς εξαιτίας της γεωγραφικής συσχέτισης των νοσοκομείων της μελέτης με τα υπόλοιπα ευρωπαϊκά κέντρα. Όσον αφορά στους μοριακούς μηχανισμούς αντοχής στις καρβαπενέμες, για τη τύπου ΟΧΑ-51 οξακιλινάση παρατηρήθηκε η επικράτηση των blaΟΧΑ-66 και blaΟΧΑ-69 αλληλομόρφων ενώ σε κανένα από τα στελέχη της μελέτης το γονίδιο αυτό δε βρέθηκε να σχετίζεται με την ISAba1 αλληλουχία εισδοχής. Η ΟΧΑ-58 οξακιλινάση ανιχνεύθηκε στη πλειοψηφία των στελεχών (65.4%) και σε όλα αυτά τα στελέχη το blaOXA-58 γονιδίο βρέθηκε να σχετίζεται με την ISAba3 αλληλουχία εισδοχής. Στους ισχυρούς υποκινητές που παρείχε το ISAba3 στο εν λόγω γονιδίο ενδέχεται να οφείλεται η θετική συσχέτιση που βρέθηκε μεταξύ της φορείας του blaOXA-58 γονιδίου και της υψηλότερης αντοχής στις καρβαπενέμες σε σχέση με τα στελέχη που έφεραν μόνο το blaOXA-51 γονίδιο. Επίσης, δεν ανιχνεύθηκαν ποσοτικές διαφορές στην έκφραση του γονιδίου μεταξύ στελεχών που ανήκαν στον ίδιο κλώνο και εμφάνιζαν διαφορετικό επίπεδο αντοχής
