Investigation into the pathogenesis of masticatory and skeletal muscle atrophy in canine leishmaniosis (Leishmania infantum)

Canine polymyositides, which appear more often than the other myopathies, are mainly the result of infectious, parasitic, hereditary and immunological insults. Their diagnosis will be based on the concerted evaluation of the clinical picture (myopathic syndrome), serum activities of muscular enzymes, especially that of CK and the electromyographic (EMG) and histopathological findings in the affected muscles. According to Bohan and Peter (1975a) the aforementioned data constitute the diagnostic criteria for polymyositis, irrespective of its etiology. The fulfillment of 4, 3 and 2 of these criteria makes the diagnosis of this condition definite, possible and probable, respectively. Canine leishmaniosis (CL) is a protozoan disease caused by Leishmania infantum (MON-1), at least in the mediterranean countries. Apart from the various members of the canidae family, CL is transmitted to other mammals, including humans, by blood-sucking Phlebotomus sp sandflies (mediterranean countries). The clinical picture of CL is comprised by a wide variety of symptoms and lesions involving nearly every organ system, the muscular included. The purpose of the present study was to investigate the pathogenesis of muscular atrophy, a fairly common clinical manifestation of CL, that is more prominent and severe in temporalis muscles. To that cause, a panel of biochemical (serum CK and LDH activities), electrodiagnostic (EMGs, electroneurograms), histopathological, immunohistochemical and histochemical techniques was employed. The relevant results obtained in the masticatory muscles were compared with those in skeletal muscles to determine whether the myopathy is localized or generalized. The study material included 48 natural CL cases, the diagnosis of which was confirmed with serology (IFA, ELISA) and/or the detection of L. infantum amastigotes in lymph node aspirates. The mean age of all these animals, which belonged to 18 breeds (n:45) or were mongrels (n:3), was 4 years and the male to female ratio 1:2.7. Also, the dogs had no clinical or laboratory evidence of other concurrent diseases. After the physical examination, the animals were allocated into 3 groups of which group A (n:14) included those presented no muscular atrophy, group B (n:20) the animals with disproportional atrophy between masticatory and skeletal muscles and group C (n:14) the cohorts with proportional atrophy between the two muscle groups. Regarding the signalment parameters, no significant difference could be found between the groups. Four clinically healthy dogs (controls), that had been tested negative for L. infantum with the aid of serology and lymph node cytology, were also used simply for comparative reasons.Οι πολυμυΐτιδες στο σκύλο, που εμφανίζονται συχνότερα απ΄ό,τι οι άλλες μυοπάθειες, διακρίνονται σε λοιμώδεις, παρασιτικές, συγγενείς-κληρονομικές και ανοσολογικές, με βάση την αιτιολογία τους. Η διάγνωση των πολυμυΐτιδων βασίζεται στη συνεκτίμηση της κλινικής εικόνας (μυοπαθητικό σύνδρομο), της δραστηριότητας των μυϊκών ενζύμων και ιδιαίτερα της CK στον ορό του αίματος, των ευρημάτων από το ηλεκτρομυογράφημα (ΗΜΓ) και εκείνων της ιστοπαθολογικής εξέτασης των βιοψιών από τους προσβλημένους μυς. Τα παραπάνω σύμφωνα με τους Bohan και Peter (1975a) αποτελούν διαγνωστικά κριτήρια της πολυμυΐτιδας, ανεξάρτητα από την αιτιολογία της. Η εκπλήρωση 4, 3 και 2 κριτηρίων, καθιστούν τη διάγνωση της νοσολογικής αυτής οντότητας αναμφίβολη, δυνατή και πιθανή, αντίστοιχα. Η λεϊσμανίωση του σκύλου (ΛΣ) είναι πρωτοζωϊκό νόσημα που οφείλεται στη Leishmania infantum (MON-1), στις παραμεσόγειες τουλάχιστον χώρες. Η ΛΣ, εκτός από τα διάφορα είδη της οικογένειας των κυνιδών μεταδίδεται και σε άλλα θηλαστικά, μεταξύ των οποίων και ο άνθρωπος, με τα νύγματα των αιματοφάγων σκνιπών του γένους Phlebotomus sp. (παραμεσόγειες χώρες). Η κλινική εικόνα της ΛΣ περιλαμβάνει μεγάλο αριθμό συμπτωμάτων και αλλοιώσεων από διάφορα όργανα και συστήματα στα οποία περιλαμβάνονται και οι μύες. Σκοπός της μελέτης αυτής ήταν η διερεύνηση της παθογένειας της μυϊκής ατροφίας που παρατηρείται συχνά στη ΛΣ και είναι ιδιαίτερα εμφανής και εντονότερη στους κροταφίτες μυς. Για το λόγο αυτό έγιναν διάφορες βιοχημικές (δραστηριότητα CK και LDH στον ορό του αίματος), ηλεκτροδιαγνωστικές (ΗΜΓ, ηλεκτρονευρογράφημα), ιστοπαθολογικές, ανοσοϊστοχημικές και ιστοχημικές εξετάσεις. Στη συνέχεια, τα αποτελέσματα του ΗΜΓ και της ιστοπαθολογικής, ανοσοϊστοχημικής και ιστοχημικής εξέτασης συγκρίθηκαν μεταξύ των μασητήριων και των σκελετικών μυών προκειμένου να εξακριβωθεί αν η υποκείμενη μυοπάθεια εντοπίζεται αποκλειστικά στους πρώτους ή είναι γενικευμένη
