Development of the reporting system of adverse events in patient safety in the Pirkanmaa Hospital District

The objective of this Master’s thesis was to research the development of how to handle and report safety related incidents in health care to improve the operation and safety in in the Pirkanmaa Hospital District (PSHP). By systematic analysing and reporting of safety related incidents, the organization can learn various things such as how to better understand and manage the tasks of the units and the core task of the organization. The organization can also learn the way the organization works and hence provide efficient and safe care. In this way the problems and errors in the operations of the organization can be recognized so that the deviations can be prevented in the future and the consequences can be minimized. Before all, the organization needs shared procedures that all people are aware of and committed to them. Analysing safety related incidents means organizing and structuring them so, that the organization can get a better idea of all the factors that have affected the incidents. The aim is to get a very profound view of the operation of the organization by getting acquainted with the amount, the type, and the consequences of the kinds of incidents that happen. The increasing amount of reported incidents should be examined as whole at all hierarchical levels in the organization. The thesis is divided into two parts. In the literature study part, issues related to the reporting system of adverse events and the safety in healthcare are explored. In the empirical part of the thesis, tools and procedures are planned to put into practice the analysis of the safety related incidents that have been observed. Literature about the modifications and obstacles of performance measurement in public sector as well as the use of different managerial models in healthcare organisations were considered in order to take account the characteristics of the case organisation. The research was conducted by making observations and analyzing material concerning the individual departments of the district, conducting interviews among the management and employees, and going through the district’s databases by analysing and visualization of the information. The developed tools and procedures were built in several stages including the clarification of the vision and strategy, specification of the critical success factors, definition of the preliminary tools and procedures and final decision of the tools chosen. On the grounds of positive feedback from the final interviews, the research should be continued. The tools and procedures developed can be applied into similar operation units in healthcare for some parts. By doing so the case unit's characteristics and measures' linkage to strategy must be taken into account. The importance of the research is notable due to its timeliness and there is a big need for follow-up research since the examination of the mass of incidents can lead to a wide beneficial risk analysis of the operation of the organization.Diplomityön tavoitteena on toteuttaa tutkimus potilasturvallisuuteen liittyvien vaaratapahtumien raportoinnin kehittämisestä Pirkanmaan sairaanhoitopiirissä (PSHP). Työssä tarkastellaan terveydenhuollon vaaratapahtumien käsittelyä ja raportointia organisaation toiminnan ja turvallisuuden parantamiseksi. Erilaisia vaaratapahtumia systemaattisesti käsittelemällä organisaatio voi muuan muassa paremmin ymmärtää ja hallita yksiköiden työtehtäviä sekä koko organisaation perustehtävää. Organisaatio voi oppia omasta toiminnastaan ja siten kehittää turvallisen ja vaikuttavan hoidon toteutumista sekä tunnistaa toiminnan ongelmakohtia, joihin perehtymällä voidaan ehkäistä poikkeaman toistuminen tai lieventää niiden seurauksia. Ennen kaikkea organisaatio tarvitsee yhteisesti sovitut ja selkeät menettelytavat käsittelylle. Vaaratapahtumien analysointi tarkoittaa tapahtumien jäsentämistä ja luokittelua siten, että saadaan parempi käsitys siitä, millaiset tekijät tapahtuman syntyyn ovat vaikuttaneet. Tarkoituksena on saada mahdollisimman perusteellinen kuva organisaation toiminnasta perehtymällä muun muassa tapahtumien määrään, tyyppiin sekä seurauksiin. Kertynyttä vaaratapahtumien joukkoa on tarpeellista tarkastella kokonaisuutena yksikkö-, vastuualue-, toimialue sekä organisaatiotasolla. Kirjallisuustutkimusosassa selvitetään turvallisuuden osuutta terveydenhuollossa toiminnan kehittämiseksi sekä vaaratapahtumien raportointimenettelyä. Kokeellisessa osuudessa suunnitellaan ja toteutetaan käsittelyprosessin suorituskyvyn mittari sekä analyysityökalut. Kohdeyksikön toiminnan luonteen vuoksi tutkimuksen teoreettinen viitekehys käsittelee myös julkisen sektorin ominaispiirteitä ja haasteita, sekä erilaisten johtamismallien soveltamista terveydenhuollon organisaatioissa. Kokeellinen osuus pohjautuu pääasiallisesti pilotti- ja vertailyksiköitä käsittelevään materiaaliin, vaaratapahtumien käsittelijöiden haastatteluihin, toiminnan havainnointiin sekä PSHP:n tietokantojen aineistojen käsittelyyn, jäsentelyyn ja hyödyntämiseen. Tutkimuksessa kehitettiin erilaisia näkymiä ja analysointitapoja, joilla saatiin esitettyä sairaalan vaaratapahtumien raportteja uudella tavalla. Nämä toteutettiin vaiheittain etenemällä PSHP:n strategian tunnistamisen kautta tavoitteiden toteutumista kuvaavien esimerkkien ja työkalumallien alustavaan määrittelyyn ja valintaan. Loppuhaastatteluiden perusteella saatu positiivinen palaute kehottaa jatkamaan tutkimusta eteenpäin. Toteutettuja työkaluja voidaan tulevaisuudessa hyödyntää muissa vastaavissa terveydenhuollon toimiyksiköissä. Tällöin on kuitenkin otettava huomioon kohdeyksikön toiminnan ominaispiirteet, sekä varmistettava, että työkalut kuvaavat kohdeyksikön strategisten tavoitteiden toteutumista. Työ on hyvin ajankohtainen ja jatkotutkimuksen tarve on suuri, koska vaaratapahtumien tarkastelu voi innostaa myös monipuoliselle toiminnan riskien ennakoivalle tarkastelulle. /Kir1
