Mini and narrow diameter dental implants as mandibular overdenture retainers. A systematic review

Relevance of the problem and aim of the work. When standard dental implants may not be used for edentulous mandible prosthetic rehabilitation, mini and narrow diameter dental implants can be used. The aim of this review is to evaluate the reliability and patient satisfaction of mini and narrow diameter dental implants as mandibular overdenture retainers. Material and the methods. Literature search was conducted from October 2018 to February 2019 on PubMed/Medline, Cochrane Library, Willey Online Library Google Scholar databases, according to the PRISMA statement. The methodological quality of publications was evaluated using Down and Black‘s checklist. Results: 20 publications, that examined 1443 mini and 862 narrow diameter dental implants in period of 6 months to 5 (7) years after the implantation, were selected for systemic review. Survival and success rates were better in narrow diameter than mini dental implants and were 97.68%, 93.89% and 93.05%, 89.92% respectively. On average, vertical alveolar bone resorption around mini dental implants was greater by 0,5 mm after 6 months and by 0,6 mm after 1 year compared to narrow diameter dental implants. Prosthetic complications were more frequent in the mini dental implant retained overdentures: matrix replacement was more frequent by 1.43%, relining by 23.34% and denture fracture by 6.28% compared to overdentures retained on narrow diameter dental implants. Overall satisfaction with prosthesis and patient quality of life using overdenture prosthesis retained by mini dental implants improved by 4,5% more compared to overdentures retained by narrow diameter dental implants. Conclusions. In a period up to 5 years mini and narrow diameter dental implants can be used to retain mandibular overdenture as an alternative to standard dental implants
