The contribution emphasizes the critical elements and complexity of regional health systems in Italy. In the context of a more complex research carried out in 2012 in Emilia Romagna, Lombardia, Puglia, Toscana, Veneto, some issues have raised especially to the topic of fragmentation of local programming, the lack of homogeneity of local contexts and different models of regulation. But the most critical concerns relevant contextual factors, exogenous to the regional health systems. These problems appear to play a major role, especially at a time when the economic crisis seems to strongly reduce the resources devoted to the welfare system as a whole
