Sexual and sexuality, cancer, adolescents and young adults. : A qualitative study on a population cared in an AYA unit.

Les progrès considérables de la médecine face au cancer ont fait émerger l'enjeu de la qualité de vie après les traitements, dans laquelle la vie sexuelle revêt une place importante. Elle se pose avec acuité pour les adolescents et les jeunes adultes compte-tenu de l'avènement des nouvelles potentialités génitales et procréatrices qui caractérisent cette période de la vie. Les publications issues du champ biomédical ont clairement établi que la survenue d'un cancer heurte le déploiement du processus d'adolescence. Peu ont mis en évidence ses effets dans une perspective psychodynamique qui défend la spécificité de la psychosexualité humaine, que nous privilégierons ici. La revue critique de la littérature entend démontrer la validité de cette approche et argumenter son inscription dans le champ théorique de la psychanalyse, dans laquelle la notion de sexuel est entendue comme centrale et organisatrice de la vie psychique.Toutefois, se déroulant sur un terrain clinique hospitalier, cette étude cherche à privilégier un dialogue avec la médecine, champ disciplinaire qui lui est exogène. Sous-tendue par cette ambition épistémologique, elle utilise la théorisation ancrée, méthodologie qualitative qui permet d'interroger la faisabilité d'une recherche en psychanalyse.Cliniquement, cette thèse défend l'hypothèse d'une compatibilité fantasmatique entre le traitement psychique du corps malade et la résistance inhérente du psychisme à intégrer le sexuel et son étrangeté. Elle vise à dégager comment cette compatibilité vient servir ou desservir le déploiement du processus pubertaire ce contexte, et éclairer la spécificité de organisations psycho-sexuelles qui en découlent. Elle étudie comment la maladie engage un rapport dialectique, sur les plans somatique et psychique, entre le corps érogène adolescent et le corps malade en danger létal, qui va colorer singulièrement les réorganisations psychiques et libidinales du travail d'adolescence ordinaire.The considerable progress in the light against cancer raises the question of the quality of life after treatments and in particular the quality of patients' sex life. To study the impact on the sexuality of adolescents and young adults seems particularly salient in respect to the emerging genital and reproductive potentialities characteristic of this period of life. Biomedical research has clearly established that the process of adolescence is upset by the onset of cancer. Few publications explore a psycho-dynamic perspective which refers to the specificity of human psycho-sexuality, which is the approach we have chosen for this study. Our literature review strives to demonstrate the validity of this approach and argues that this study is rooted in psychoanalytical theory, the concept of sexuality being understood as central to and structuring of the human psyche. Our research was conducted in a hospital setting and therefore also establishes a dialog with medicine, an exogenous discipline. This epistemological ambition led us to resort to grounded theory, a qualitative methodology which allows us to question the feasibility of psychoanalytical research, aiming at results that might be considered by a wider scientific community.From a clinical point of view, this thesis puts forward the hypothesis that the suddenly imposed mental processing of the sick body is fantasmatically compatible with the psyche's inherent resistance to integrating sexuality and its strangeness. It seeks to demonstrate how this compatibility serves or hinders the process of puberty in its coalescence with serious illness, and to elucidate resulting psycho-sexual adjustments. With this aim, we study how the illness installs a dialectical relationship both on a mental and somatic level between an erogenous adolescent body and the lethally threatened sick body, thus singularly characterising the mental and libidinal adjustments and organisations inherent to the ordinary adolescent process
