Real time elastography: first study to measure the basic tissue elastic properties of the cervix

Einleitung: In dieser Arbeit wurden erstmalig altersabhängige/tragzeitabhängige Eigenschaften und Veränderungen der Gewebeelastizität bei prä- und postmenopausalen Frauen sowie Schwangeren untersucht. Die Ergebnisse der Normalkollektive wurden mit den Gruppen mit Pathologien (Zervixläsionen bis Karzinom bzw. Zervixinsuffizienz) verglichen. Material und Methoden: insgesamt wurden 165 Patientinnen, davon 52 Schwangere, transvaginal mit einem Schallkopf mit 9 MHz untersucht. Eine ROI wurde umfahren und markiert; in allen Fällen umfasste sie die gesamte Zervix uteri einschließlich des Zervikalkanals. Im Dualmodus wurde das konventionelle B-Bild farbcodiert überlagert. Die Anteile an Rot (weiches Gewebe), Grün (mittelhartes/ intermediäres Gewebe) und Blau (hartes/ festes Gewebe) wurden mit einem speziellen Softwareprogramm ermittelt und nach ihrem prozentualen Anteil am Gesamtgewebe eingeteilt. Zusätzlich erfolgte eine Beurteilung durch zwei unabhängige Betrachter mittels einer Analogskala von 1-5. Zur Anwendung kamen die Softwareprogramme StatXact, SAS, SPFC und SPSS sowie Anova, Wilcoxon-Test, Pearsons Korrelationskoeffizient, Mann-Whitney Test und Mc Nemars-Test. Ergebnisse: Die Farbverteilung im Normalkollektiv der nicht schwangeren Patientinnen zeigte keine signifikanten Veränderungen in Bezug auf das Alter. Der Grünanteil war vorherrschend sowohl beim Normalkollektiv (67+/- 13%) als auch beim Pathologienkollektiv (64+/-15%). Einzig in der Subgruppe der Zervixkarzinome war eine Zunahme des Blauanteils als Ausdruck eines festeren Gewebes zu verzeichnen (34+/- 15% vs. 26+/-13%). Im Schwangerenkollektiv ohne Pathologien zeigte sich ebenfalls eine Dominanz des Grünanteils (67,1%). Die Gewebeelastizität war unabhängig von der Schwangerschaftsdauer, zeigte aber Veränderungen mit dem Alter. Zervixinsuffizienzen waren mit einer Erhöhung des Rot- und Grünanteils vergesellschaftet und waren daher „weicher“ darstellbar. Zusammenfassung: Die computergestützte und die subjektive Auswertung der Zervixelastizität stimmte gut überein. Zervixkarzinome konnten sicher erkannt werden, Zervixläsionen (CIN) dagegen nicht. Eine engmaschige Überwachung von Risikoschwangerschaften in Bezug auf drohende Frühgeburt sind mit der real time Sonoelastografie gut durchführbar. The normal findings were compared with the results in a group of patients with focal pathology of the cervix and with cervical insufficiency. Materials and Methods: a total of 165 patients, among them 52 pregnant women, underwent transvaginal ultrasound using a 9 MHz high-end US device with a vaginal probe. The examiner placed a rectangular ROI in the scans to include the cervix and the cervical canal. The real-time elastography information was color-coded and superimposed on the B-mode scan. A special morphometry software calculated the percentages of red (soft tissue), green (medium hard) and blue (hard tissue) relative to the total area. In addition, two independend readers evaluated subjectively on an analogue scale from 1-5. Statistical analysis was performed using StatXact, SAS, SPFC and SPSS and ANOVA, Wilcoxons test, Pearsons correlation, Mann-Whitney test and the McNemars test. Results: the color spectrum showed green to be predominant in the nomal group of non pregnant women and in the group with cervical pathology, without a difference in age. Significant differences in the blue color spectrum (hard tissue) were only found between the cervical cancer patients and the normal group. In the normal group of pregnant women were no significant changes in color distribution (percentage of green) with duration of pregnancy, but with patient age. Cervical insufficiency showed higher proportions of green and red as a sign of softer tissue. Conclusion: computer- assisted and subjective evaluation of cervical elastography have a good agreement and allows differentiation of malignancy from normal findings. CIN can not be identified with this modality. A close monitoring of high-risk pregnancy for premature delivery is able and easy to perform.
