Magnetic resonance imaging based morphologic evaluation of the pineal gland for suspected pineoblastoma in retinoblastoma-patients and age-matched controls

Das Trilaterale Retinoblastom (Trb) ist eine äußerst seltene Erkrankung, bei der zusätzlich zum Retinoblastom ein Pineoblastom auftritt. Die Inzidenz beträgt ca. 3% und die MRT-basierte Diagnostik stellt eine Herausforderung dar. Das Ziel dieser Studie ist die MRT-basierte radiomorphologische Untersuchung der Glandula pinealis bei Patienten mit Retinoblastom (mit und ohne Pineoblastom) und in einer alterskorrelierten Kontrollgruppe um die Identifizierung des Pineoblastoms zu verbessern. Material und Methodik 80 Patienten mit Retinoblastom (Rb) sowie 80 alterskorrelierte Kontrollpatienten wurden in dieser retrospektiven, durch die Ethikkommission genehmigten Kohortenstudie untersucht. Das Haupteinschlusskriterium war das Vorhandensein einer neurocraniellen MRT. Zwei unabhängige Radiologen analysierten die Glandula pinealis in Bezug auf folgende Parameter: Signalintensität inT1- und T2-gewichteten Aufnahmen, Muster der Kontrastmittelanreicherung, die Begrenzung der Glandula pinealis, das Vorhandensein zystischer Anteile, die Größe der Glandula pinealis sowie die Größe der Pinealiszyste, falls vorhanden. Ein dritter Radiologe analysierte alle MRT-Aufnahmen in Bezug auf das Vorliegen eines Pineoblastoms. Ergebnisse Drei Patienten hatten die Diagnose eines Pineoblastoms (Trb-Kohorte) und 77 Patienten (Nicht-Trb- Kohorte) hatten kein Pineoblastom. Der Mittelwert der Größe der Glandula pinealis (ap, ml, cc) bei Nicht-Trb Patienten war deutlich kleiner (6.4 x 5.2 x 4.6 mm) als bei Trb Patienten (11.1 x 8.6 x 7.8 mm). In der Kontrollgruppe betrug der Mittelwert der Größe 6.1 x 5.2 x 4.4 mm, womit kein signifikanter Unterschied zu Nicht-Trb Patienten feststellbar ist. Bis auf das Muster der Kontrastmittelanreicherung zeigten alle weiteren untersuchten Parameter keine relevanten Unterschiede. Der dritte Radiologe identifizierte das Pineoblastom mit einer Sensitivität von 100% (3 von 3) und einer Spezifität von 94% (72 von 77). Schlussfolgerung Die Größe der Glandula pinealis ist das wichtigste Kriterium für die Erkennung eines Pineoblastoms im MRT. Zwischen Nicht-Trb Patienten und der alterskorrelierten Kontrollgruppe bestehen keine Unterschiede bezüglich der Größe der Glandula pinealis und der Größe der Pinealiszysten. Die ermittelten Größen der Glandula pinealis sowie der Pinealiszysten können als Vergleichsgrundlage für die Beurteilung der Glandula pinealis bei zukünftigen Patienten dienen.Trilateral retinoblastoma (Trb) is a very rare disease in which retinoblastoma (Rb) patients develop a pineoblastoma. The pupose of this study was to evaluate the morphologic MR characteristics of the pineal gland in retinoblastoma patients without and with pineoblastoma in comparison to age- matched controls to improve early identification of a pineoblastoma. Methods and Materials 80 patients with retinoblastoma (Rb cohort) and 80 age-matched controls who had undergone brain MRI were included in this retrospective ethical review board approved cohort study. Two radiologists independently analyzed the following MR characteristics of the pineal gland: Signal intensity on T1- and T2 weighted images, enhancement pattern, delineation of the gland, presence of cystic components, size of pineal gland and size of pineal cyst, respectively. A third radiologist assessed all images for presence or absence of pineoblastoma. Results Three patients were positive (Trb-cohort) and 77 were negative for pineoblastoma (non-Trb- cohort). The mean size (ap, ml, cc) of the pineal gland in non-Trb patients was considerably smaller (6.4 x 5.2 x 4.6 mm) compared to the Trb-patient-cohort (11.1 x 8.6 x 7.8 mm). In the control group the mean size of the pineal gland measured 6.1 x 5.2 x 4.4 mm (no statistically significant difference compared to the non-Trb-cohort). The third radiologist identified pineoblastomas with a sensitivity of 100% (3 of 3) and a specificity of 94% (72 of 77). Conclusion The size of the pineal gland is the most important diagnostic parameter for the identification of pineoblastomas. Our non-Trb patients did not show significant differences in the size of the pineal gland and pineal gland cysts compared to age-matched controls. The presented data can serve as a size reference of normal pineal gland and pineal cysts in the diagnostic work-up of Rb patients with suspected pineoblastoma
