Does video-mediastinoscopy improve the results of conventional mediastinoscopy?

Ziel der Arbeit: Die Mediastinoskopie ist bis heute der Goldstandard für das mediastinale Staging des Lungenkarzinoms. Die Einführung der videoassistierten Mediastinoskopie führte zu einer Verbesserung der Methode durch eine bessere Visualisierung und die Möglichkeit beidhändigen Arbeitens. Zum ersten Mal soll hier in einem direkten Vergleich geprüft werden, ob die videoassistierte Mediastinoskopie (Video-MS) gegenüber der konventionellen Mediastinoskopie (konv. MS) auch zu einer höheren Qualität der Gewebeproben und einer besseren Aussagekraft führt. Methoden: Alle zwischen Juni 2003 und Dezember 2005 an der Evangelischen Lungenklink Berlin (ELK) durchgeführten Mediastinoskopien (n=366) wurden analysiert. Durch den Operateur wurde unmittelbar postoperativ nach einem einheitlichen Schema die Art der verwendeten Technik sowie die Anzahl und Lokalisation der untersuchten Lymphknoten dokumentiert. Zur Analyse der Qualität der Gewebsproben wurden für jede Lymphknotenstation Untersuchungs- und Resektionsraten berechnet. Zur Berechnung der Aussagekraft der Mediastinoskopien erfolgte der Vergleich mit dem Ergebnis der systematischen Lymphadenektomie bei 179 thorakotomierten Patienten. Positive mediastinoskopische Befunde wurden auch bei nicht durchgeführter Thorakotomie als richtig gewertet. Zusätzlich erfolgte eine Analyse der Aussagekraft der Mediastinoskopien je nach computertomographischem Befund. Die Berechnungen erfolgten für die Mediastinoskopien insgesamt und für konv. MS und Video-MS getrennt. Ergebnisse: Von 366 Mediastinoskopien wurden 132 (36,1%) mit der konventionellen und 234 (63,9%) mit der videoassistierten Methode durchgeführt. Im Median wurden 7 Lymphknoten pro Patient untersucht (konv. MS 6, Video-MS 7). Lymphknoten aus den Lymphknotenstationen 2 links und rechts, 4 links und rechts und 7 wurden in 61,3%, 87,5%, 89,2%, 91,3% bzw. 95,1% untersucht und in 28,9%, 44,3%, 42,2%, 38,3% bzw. 33,1% komplett reseziert. Durch die Video-MS wurden gegenüber der konv. MS signifikant höhere Resektionsraten an allen Lymphknotenstationen erreicht, während die Untersuchungsraten sich nicht signifikant unterschieden. Die Prävalenz metastasierter Lymphknoten, die Sensitivität und der NPV (negative predictive value) betrugen bei der Mediastinoskopie insgesamt 43,8%, 72,7% bzw. 82,5%. Signifikante Unterschiede zwischen konv. MS und Video-MS waren nicht nachzuweisen. Die entsprechenden Werte bei computertomographischem N0/1-Befund betrugen 19%, 43,5% bzw. 88,3% und bei computertomographischem N2/3-Befund 68%, 80% bzw. 70,2%. Interpretation: Die vorliegenden Daten weisen eine Verbesserung der Qualität der Gewebsproben durch die Verwendung der Video-MS nach. Lymphknoten werden in signifikant höherem Anteil komplett reseziert. Dies spricht für die Etablierung der Video-MS als Standard bei der Mediastinoskopie. Insgesamt lagen Sensitivität und NPV für das Staging des Lungenkarzinoms bei der konv. MS und bei der Video-MS jedoch unter den für alternative Stagingmethoden (EUS-FNA, EBUS-FNA) in der Literatur angegebenen Werten. Um die Stellung der Video-MS beim Staging des Lungenkarzinoms zu bewerten, sind weitere Studien notwendig.Objective: Despite new technologies, mediastinoscopy remains the gold standard for mediastinal staging of lung cancer. Since the introduction of video- mediastinoscopy the quality of mediastinal lymph node staging has improved due to improved visualisation and bimanual working. However, video-mediastinoscopy has not been directly compared to standard mediastinoscopy. The objective of this study was to prove whether the use of video-assisted mediastinoscopy (VM) compared to conventional mediastinoscopy (CM) results in a higher quality of lymph node tissue and a higher accuracy. Methods: All mediastinoscopies (n= 366) performed in the Evangelische Lungenklinik Berlin (ELK) from June 2003 to December 2005 were analysed. Via a standardized protocol surgeons documented location of lymph node stations, number of lymph nodes resected or biopsied and the technique (VM or CM) utilized. Examination rates and resection rates per lymph node station were calculated to assess the quality of lymph node tissue. To analyse the accuracy of the results of the mediastinoscopy, they were compared with the results of lymphadenectomy in 179 patients who underwent thoracotomy. Postive findings in mediastinoscopy were assigned to be correct even if there was no thoracotomy performed. Additionally, an analysis of the accuracy of mediastinoscopies in relation to the results of computed tomography was performed. Calculations were carried out for all mediastinoscopies and for CM and VM seperately. Results: Out of 366 mediastinoscopies, 132 (36,1%) were performed as CM and 234 (63,9%) as VM. The median of lymph nodes resected was 7 (CM: 6, VM: 7). Lymph nodes from the lymph node stations 2 left and right, 4 left and right and 7 were examined in 61,3%, 87,5%, 89,2%, 91,3% and 95,1%, respectively, and completely resected in 28,9%, 44,3%, 42,2%, 38,3% and 33,1%, repectively. Resection rates were significantly higher for all lymph node stations when VM was used. There was no significant difference in the examination rates of VM compared to CM. Prevalence of lymph node metastasis, sensitivity and negative predictive value (NPV) for all mediastinoscopies were 43,8%, 72,7% and 82,5%, respectively. There were no significant differences between CM and VM. The corresponding results in patients without enlarged lymph nodes in computed tomography (cN0/1) were 19%, 43,5% and 88,3%, respectively, and in patients with enlarged lymph nodes in computed tomography (cN2/3) 68%, 80% and 70,2%, respectively. Conclusion: This study indicates a higher quality of lymph node tissue resected when the video-assisted method is used. Lymph nodes are completely resected in a significantly higher number. These findings support the use of VM as the standard method. Nevertheless, the sensitivity and NPV of both CM and VM were lower than other studies have found for alternative staging methods such as EUS-FNA and EBUS-FNA.
