The prognostic significance of angiogenesis for colorectal cancer

Anhang 111Die prognostische Bedeutung der Gefäßdichte und Expression des basic Fibroblast Growth Factor (bFGF) wird anhand von 146 kolorektalen Karzinomen von Patienten, die kurativ operiert wurden, untersucht. Die Tumorschnitte wurden mit dem Antikörper JC70 und dem anti-bFGF-Antikörper immunhistochemisch gefärbt und die Gefäßdichte ermittelt. Gefäßdichte wie auch Expression von bFGF stellen sich als unabhängig von Geschlecht, Tumorstadium, Tumorrezidiv und Gesamtüberlebenszeit heraus. Unter den Patienten mit Tumorrrezidiv ist die Gesamtüberlebenszeit in der Gruppe mit hoher Gefäßdichte signifikant kürzer als in der mit niedriger Gefäßdichte. Eine multivariate Analyse weist der Gefäßdichte eine unabhängige prognostische Bedeutung für die Gesamtüberlebenszeit von Patienten mit Tumorrezidiv aus. Aus den Ergebnissen wird gefolgert, daß die Gefäßdichte zwar den zeitlichen Ablauf der Tumorrezidiventstehung beeinflußt, jedoch läßt sich aus ihrer immunhistochemischen Ermittlung kein Prognosefaktor für den klinischen Alltag ableiten.The prognostic relevance of vascular density and expression of the basic Fibroblast Growth Factor (bFGF) in colorectal cancer has been studied, examining 146 patients treated surgically for cure. Tumor sections were immunostained with JC70 and an monoclonal anti-bFGF-antibody, and the vessel density determined. Both, vessel density and bFGF-Expression were found to be independent of sex, stage of disease, tumor recurrence, and overall survival. Overall survival in the subgroup of patients with tumor recurrence was significantly shorter with tumors of greater than those of less vessel density. Multivariate analysis shows microvessel count to be an independent prognostic factor for the overall survival rate of patients with tumor recurrence; and a significant difference in the relapse-free survival rates between the hypovascular and the hypervascular groups. The findings suggest that microvessel density of the primary tumor determines the speed of tumor recurrence when metastastic disease has been triggered by other mechanism. Although tumor vascularization can be linked to the aggressiveness of colorectal cancer, it has no value as a new prognostic marker in clinical practice
